Out of 725 operative procedures at the gallbladder and bile duct there were 33 multiple interventions. The mortality rate of reoperations came up to 3 per cent. If feasible long-time T-drainage or ring-drainage is preferred. Biliodigestive anastomoses should be avoided.

Percutaneous transhepatic cholangiography (PTC) with subsequent external bile drainage by nonsurgically established percutaneous transhepatic intubation of bile ducts was performed in 105 patients with obstructive jaundice. In 460 cholecystectomies performed for acute cholecystitis 215 (47%) positive gallbladder bile cultures were obtained. The accuracy and possible clinical value of grey scale ultrasonography in the detection of gallstones has been prospectively studied in 100 unselected patients presenting with recurrent biliary colic, acute cholecystitis or acute pancreatitis. Adequate visualization of the gallbladder was obtained in 79 cases, with 3 false positive and no false negative reports. In order to provide a baseline for comparison with the results of the Kasai hepatic portoenterostomy, follow-up information was obtained for 89 infants who underwent surgical exploration alone for biliary atresia from 1956 through 1975. A simple score has been constructed to facilitate the selection of apparently normal newborns at risk or hidden congenital disorders. Components of the score are family history of previous malformations, stillbirth or infantile death of unknown origin, intrauterine growth retardation (2 points each), and six minor malformations: antimongoloid palpebral slant, hypertelorism, preauricular fistula, simian crease, mammillary and hallucal abnormality (1 point each). Two patients developed a focal mass in the head of the pancreas presumably following attempted endoscopic retrograde cholangiopancreatography (ERCP). These lesions grossly mimicked carcinomas of the pancreas. In one case a radical pancreatoduodenectomy was performed because of the misleading gross findings. A focal mass in the pancreas following ERCP may develop due to asymptomatic pancreatitis caused by the procedure.

In about 5-6% of all patients with malignant myeloproliferative disorders the gastrointestinal tract is involved with macroscopic lesions. The most common sites of digestive manifestations are stomach, small bowel, pancreas and colon. Multiple irregular ulcers, doughy infiltration of the bowel wall, multiple nodules and diffuse polyposis of the colon are classic patterns of secondary lymphoma of the gut. Prognosis is considerably impaired by gastroenterological symptoms.

Three hundred patients with acute pancreatitis have been studied. Pancreatitis was associated with alcoholism in 207, biliary tract disease in 51 and other conditions in 42.
