Management of Hyperhemolysis in β-thalassemia With Multiple Immunosuppressives, Including Complement Blockade.

Hyperhemolysis is a life-threatening condition of exaggerated hemolysis of red blood cells which occurs in patients receiving chronic transfusion therapy. We present a 19-year-old male with the β-thalassemia major with an episode of hyperhemolysis. Hemolysis was initially unresponsive to immunosuppression but responded after the addition of eculizumab. Several weeks after stabilization, hemolysis returned; which was also managed with immunosuppression and eculizumab. Hyperhemolysis presents significant challenges in β-thalassemia due to the underlying dysfunctional erythropoiesis and transfusi

The Liver in Sickle Cell Disease.

Patients with sickle cell disease can develop liver disease as a result of intrahepatic sickling of erythrocytes, viral hepatitis and iron overload secondary to multiple blood transfusions, and gallstone disease as a result of chronic hemolysis. The spectrum of clinical liver disease is wide and often multifactorial. Some patients develop cirrhosis that may progress to end-stage liver failure. Limited evidence exists for medical treatments. Exchange blood transfusions may improve outcomes in the acute liver syndromes. Liver transplantation may be an option for chronic liver disease. The role f…

Joint effusions and purpura in multiply-transfused adult beta-thalassemia- clinical pointers to diagnosis of scurvy.

Periodic transfusions and effective chelation have ensured that thalassemics survive in to adulthood but their life is punctuated by peculia … esponded dramatically to vitamin C supplementation. It is imperative to keep in mind that recurrent blood transfusions are associated with a state of subclinical vitamin C deficiency and overt scurvy may manifest as cumulative number of transfusions increase, as in adult thalassemics.

Granulocyte transfusions--a review for the intensive care physician.

In order to achieve the maximum therapeutic benefit in the treatment of malignancies, doses of chemotherapeutic agents are pushed to the point of severe marrow toxicity. This aggressive therapy can lead to iatrogenic complications including haemorrhage and sepsis due to the depletion of platelets and granulocytes. Prior to the advent of platelet transfusions, haemorrhage was the leading cause of death in these patients. Advances in blood banking and the availability of platelet transfusions have markedly decreased the incidence of fatal haemorrhage. As a result, infection has become the leadin

Anaphylactic transfusion reaction associated with a possible anti-A2m(1).

A 42-year-old leukaemic black male developed shortness of breath, itching, hot flushes, substernal pain, marked hypotension, and cardiorespiratory arrest after the infusion of 50 ml of pooled platelet concentrates. The patient had had one previous and uneventful red blood cell transfusion. Serologic studies of this patient's post-transfusion serum showed the presence of an antibody against one denatured IgA2 protein with the phenotype A2m(1 + 2-). All of the platelet donors were A2m(1) positive. The patient was successfully transfused with platelets from donors with the phenotype A2m(1 - 2+).

The role of freeze-dried plasma in a world of whole blood: a Canadian prehospital and transport perspective.

BACKGROUND: Hemorrhage is the leading preventable cause of trauma death, and timely transfusion remains a major challenge in Canada, particularly in rural and remote regions where long transport times create "blood deserts." Whole blood has re-emerged as the gold standard for resuscitation, but donor limitations, short shelf-life, and cold-chain requirements restrict its universal deployment. ROLE OF FREEZE-DRIED PLASMA: Freeze-dried plasma (FDP) offers a complementary solution with unique advantages for Canadian prehospital and transport systems. FDP is pathogen-reduced, shelf-stable for up

Perioperative Hyper-coagulation and Thrombosis: Cost Analysis After Congenital Heart Surgery.

Thrombosis, a major adverse event of congenital heart surgery, has been associated with poor outcomes. We hypothesized that in CHD patients undergoing cardiac surgery, increased perioperative use of pro-coagulant products may be associated with postoperative thrombosis in the setting of hyperfibrinogenemia, leading to greater hospital and blood product costs. Single-center retrospective study. Data from Boston Children's Hospital's electronic health record database was used in this study. All patients undergoing congenital heart surgery between 2015 and 2018 with postoperative fibrinogen level

How I treat patients who are refractory to platelet transfusions.

Patients with thrombocytopenia requiring ongoing platelet transfusion support may develop inadequate platelet count increments, referred to as platelet refractoriness (PR), which further complicates their care. The underlying etiologies of PR can be broadly divided into immune and nonimmune causes. A high index of suspicion is required to initiate testing for alloimmunization, and the leading culprit in immune PR is the development of class I HLA antibodies. The approach to diagnosis of immune PR has changed over recent years with new technologies, but questions regarding the clinical signific

Beyond the human eye: Artificial intelligence revolutionizing plasma quality control.

BACKGROUND AND OBJECTIVES: Plasma quality control (QC) in transfusion medicine relies on subjective visual inspections, which vary depending on the technologist's experience and lighting conditions, potentially compromising safety. We developed an automated system to standardize the detection of plasma colour and turbidity abnormalities, and aimed to evaluate its accuracy and reliability in a transfusion setting. MATERIALS AND METHODS: At the Blood Transfusion Center, Khon Kaen University, Thailand, three experienced technologists classified 973 plasma bags. The system was trained on 789 bags…

Acute myeloid leukemia: Current understanding and management.

Although relatively rare, acute myeloid leukemia (AML) is the most common type of acute leukemia in adults. AML is associated with poor 5-year overall survival and prompt treatment is critical. Classifying AML based on World Health Organization criteria is important for determining prognosis and applying a risk-adapted treatment approach. Throughout therapy, patients require comprehensive supportive care measures with blood product transfusions, antimicrobial treatment, and frequent monitoring for chemotherapy-related complications. This article provides an overview of AML and its treatments.

Dosing of tranexamic acid in trauma.

PURPOSE OF REVIEW: Tranexamic acid is routinely used as part of the management of traumatic bleeding. The dose recommendation in trauma was extrapolated from other clinical settings and the results of pragmatic randomized trials rather than pharmaco-kinetic and -dynamic evaluations. The review addresses current evidence on dosing of tranexamic acid in traumatized patients with a focus on efficacy, safety and risk-benefit profile. RECENT FINDINGS: A majority, but not all, of existing randomized clinical trials reports a reduction in mortality and/or blood loss with tranexamic acid administrati

What's new in whole blood resuscitation? In the trauma bay and beyond.

PURPOSE OF REVIEW: Transfusion therapy commonly supports patient care during life-threatening injury and critical illness. Herein we examine the recent resurgence of whole blood (WB) resuscitation for patients in hemorrhagic shock following trauma and other causes of severe bleeding. RECENT FINDINGS: A growing body of literature supports the use of various forms of WB for hemostatic resuscitation in military and civilian trauma practice. Different types of WB include warm fresh whole blood (FWB) principally used in the military and low titer O cold stored whole blood (LTOWB) used in a variety

[Iron in pre-operative stage and transfusion in patients undergoing hysterectomy].

BACKGROUND: The correction of preoperative anemia is part of the patient blood management program, in order to improve the patient's clinical results by reducing the number of transfusions in surgery. Uterine fibroids can cause anemia, so the application of iron before hysterectomy could reduce transfusion. OBJECTIVE: To evaluate the impact of iron treatment in the preoperative stage on the need for transfusion in patients with anemia secondary to myomatosis in the trans and postoperative stage of hysterectomy. MATERIAL AND METHODS: Patients with uterine myomatosis who presented with microcy

Novel device for automating exchange transfusions through umbilical venous route in neonates.

UNLABELLED: Manually performed double-volume exchange transfusion (DVET) is tedious, error-prone, and may incur the risk of embolism. We aimed to develop a device that automates the DVET procedure performed through the umbilical venous route. We evaluated changes in blood passing through the device during DVET. We developed an electro-mechanical device with accessories (tubing and valve assembly) to perform a complete DVET. It comprises two syringes driven by a common pump that moves back and forth to withdraw aliquots of the patient's blood and infuse equal volumes of donor blood. In tandem,

Sideroblastic anaemia in a patient with sickle cell disease.

Sideroblastic anaemia is a rare condition. We report a unique case of concomitant sideroblastic anaemia in a patient with sickle cell disease with long-standing blood transfusion history. Due to a low prevalence of sideroblastic anaemia, the diagnosis of sideroblastic anaemia is often difficult, especially when coexisting with common types of anaemia, including sickle cell disease. This case highlights the detrimental effects of anchoring bias. Rare causes of refractory anaemia should be considered in patients with haemoglobin disorders as the therapeutic approaches for these conditions are di…
