Development of a clinical prediction rule for identifying women with tension-type headache who are likely to achieve short-term success with joint mobilization and muscle trigger point therapy.

OBJECTIVE: To identify prognostic factors from the history and physical examination in women with tension-type headache (TTH) who are likely to experience self-perceived clinical improvement following a multimodal physical therapy session including joint mobilization and muscle trigger point (TrP) therapies. BACKGROUND: No definitive therapeutic intervention is available for TTH. It would be useful for clinicians to have a clinical prediction rule for selecting which TTH patients may experience improved outcomes following a multimodal physical therapy program. METHODS: Women diagnosed with p

The Safety of Cardiac and Thoracic Magnetic Resonance Imaging in Patients with Cardiac Implantable Electronic Devices.

RATIONALE AND OBJECTIVES: Studies reporting the safety of magnetic resonance imaging (MRI) in patients with a cardiac implantable electronic device (CIED) have mostly excluded examinations with the device in the magnet isocenter. The purpose of this study was to describe the safety of cardiac and thoracic spine MRI in patients with a CIED. MATERIALS AND METHODS: The medical records of patients with a CIED who underwent a cardiac or thoracic spine MRI between January 2011 and December 2014 were reviewed. Devices were interrogated before and after imaging with reprogramming to asynchronous paci

Autonomic cardiovascular control in Paralympic athletes with spinal cord injury.

INTRODUCTION: Disruption of autonomic control after spinal cord injury (SCI) results in life-threatening cardiovascular dysfunctions and impaired endurance performance; hence, an improved ability to recognize those at risk of autonomic disturbances is of critical clinical and sporting importance. PURPOSE: The objective of this study is to assess the effect of neurological level, along with motor, sensory, and autonomic completeness of injury, on cardiovascular control in Paralympic athletes with SCI. METHODS: Fifty-two highly trained male Paralympic athletes (age, 34.8 ± 7.1 yr) from 14 coun

Cervical pyogenic spinal infections: are they more severe diseases than infections in other vertebral locations?

PURPOSE: Cervical spine pyogenic infections are unusual compared to other vertebral segments, but they can be associated to worse clinical outcomes. We compared all patients with cervical spine pyogenic infections to those with thoracolumbar involvement in terms of epidemiology, prognostic factors and clinical outcomes. METHODS: We retrospectively reviewed all patients discharged from our institution with diagnosis of pyogenic spinal infections (PSI) during a 14-year period. Patients' demographics, etiologic agent, co-morbidities, site of infection, white blood cell count, erythrocyte sedimen

Effects of corticospinal tract stimulation on renal sympathetic nerve activity in rats with intact and chronically lesioned spinal cords.

Sympathetic preganglionic neurons and interneurons are closely apposed (presumably synapsed upon) by corticospinal tract (CST) axons. Sprouting of the thoracic CST rostral to lumbar spinal cord injuries (SCI) substantially increases the incidence of these appositions. To test our hypothesis that these additional synapses would increase CST control of sympathetic activity after SCI, we measured the effects of electrical stimulation of the CST on renal sympathetic nerve activity (RSNA) and arterial pressure (AP) in alpha-chloralose-anesthetized rats with either chronically intact or chronically

Spinal cord injury in children: long-term urodynamic and urological outcomes.

PURPOSE: Urological complications in adults with spinal cord injuries (SCIs) are well documented. We sought to determine the effect of SCI on the developing bladder and kidneys in the child. MATERIALS AND METHODS: We retrospectively reviewed cases of SCI with 1-year minimum followup. We identified 42 children with an average age at injury of 5.3 years (range 1 day to 14 years). Mean followup was 5.5 years (range 1 to 15.5). Videourodynamics, sonograms, infection, medications and continence were reviewed. Safe bladder capacity was defined as the pressure specific volume at 40 cm water or less.

The use of allograft bone and cages in fractures of the cervical, thoracic, and lumbar spine.

The advantages of allograft tissues and cage devices in anterior spinal reconstruction for trauma is the absence or minimization of donor site morbidity and unlimited choices of graft shapes and sizes. Osteoinductive matrices often are added to these grafting alternatives to improve healing rate and success. Allografts and cages seem to be the most frequent grafting materials used in the thoracolumbar region. Currently, autologous iliac crest is the most popular grafting material in the cervical region although cylindrical cages are gaining popularity. As the material properties of cage device

[Radical surgery and vertebral body replacement in aneurysmal bone cysts of the spine].

6 cases of aneurysmal bone cyst of the spine are presented. The clinical picture, diagnostic procedures and operative treatment will be discussed. Although aneurysmal bone cyst is a benign, non-neoplastic lesion its spreading tumor growth with destruction of important bony structures of the vertebral body, consecutive instability and cord compression presents a considerable menace to the mostly teenage-patient. Spinal computerized tomography is the diagnostic method of choice. The goal of surgical therapy of the aneurysmal bone cyst of the spine should include radical tumor excision in order t

In-hospital outcomes following surgery versus conservative therapy in elderly patients with C2 fractures: a propensity score-matched analysis.

OBJECTIVE: Second cervical vertebrae (C2) fractures are a common traumatic spinal injury in the elderly population. Surgical fusion and nonoperative bracing are two primary treatments for cervical instability, but the former is often withheld in the elderly due to concerns for poor postoperative outcomes arising from patient frailty. This study sought to evaluate the in-hospital differences in mortality, outcomes, and discharge disposition in elderly patients with C2 fractures undergoing surgical intervention compared with conservative therapy. METHODS: The National Trauma Data Bank was queri…

Comparative proteomics analysis for identifying the lipid metabolism related pathways in patients with Klippel-Feil syndrome.

BACKGROUND: Klippel-Feil syndrome (KFS) represents the rare and complex deformity characterized by congenital defects in the formation or segmentation of the cervical vertebrae. There is a wide gap in understanding the detailed mechanisms of KFS because of its rarity, heterogeneity, small pedigrees, and the broad spectrum of anomalies. METHODS: We recruited eight patients of Chinese Han ethnicity with KFS, five patients with congenital scoliosis (CS) who presented with congenital fusion of the thoracic or lumbar spine and without known syndrome or cervical deformity, and seven healthy control…

Volume-outcome relationship in halo vest utilization for C2 fractures.

BACKGROUND CONTEXT: The prevalence of C2 fractures has increased in recent years. The treatment of these fractures include halo-vest immobilization (HVI), rigid cervical collar, or spinal fusion. There is controversy regarding the management of these fractures with different institutions having their own protocols based on individualized experience. The volume-outcome relationship of HVI use for C2 fractures has not been studied. Evaluation of such relationships are important as they suggest that patients may benefit from referral to and treatment at high-volume institutions. PURPOSE: To eval

Size of the C7-T1 articular processes is overestimated on lateral radiographs when compared to standing cone beam computed tomography.

BACKGROUND: Radiography of the caudal cervical vertebral column is commonly performed in horses, but image distortion may occur due to factors such as scapular superimposition, divergent beam, and the distance between anatomy and detector. These variables can falsely suggest enlargement of the articular processes (APs) at the seventh cervical and first thoracic vertebrae (C7-T1). OBJECTIVES: To assess the presence of magnification of C7-T1 APs as determined by AP:vertebral body (VB) height ratio on lateral radiographs compared to cone beam computed tomography (CBCT) as a gold standard, and to
