Vulvar disorders in the prepubertal female.

Inspection of the vulva should be a routine part of well child care. Detection of poor perineal hygiene permits the establishment of good hygiene practices, which may prevent development of vulvovaginitis. Condylomata acuminata, molluscum contagiosum, herpetic vulvitis, and vulvovaginitis secondary to Neisseria gonorrhoeae, Gardnerella vaginalis, Chlamydia trachomatis, and Trichomonas vaginalis arouse suspicion of child sexual abuse, which must be addressed. Atopic dermatitis, psoriasis, lichen planus, and lichen sclerosis et atrophicus are often chronic and early diagnosis with appropriate tr

A case of lingual lymphoma in a dog treated with oclacitinib.

Lingual canine T-zone lymphoma (TZL) is a rare form of extranodal canine lymphoma. Seven-years and 5 months old, spayed female toy poodle presented with a red swollen mass on the tongue. A punch biopsy of the lesion was conducted, followed by histopathological and immunohistochemical analyses. Despite the absence of a lymph node excisional biopsy, the case was diagnosed as canine lingual TZL. Although there is no established treatment for lingual TZL, treatment often follows TZL protocols. In this case, the tongue lesion improved after the administration of oclacitinib, a drug approved for can

Skin, gut, and lung barrier: Physiological interface and target of intervention for preventing and treating allergic diseases.

The epithelial barriers of the skin, gut, and respiratory tract are critical interfaces between the environment and the host, and they orchestrate both homeostatic and pathogenic immune responses. The mechanisms underlying epithelial barrier dysfunction in allergic and inflammatory conditions, such as atopic dermatitis, food allergy, eosinophilic oesophagitis, allergic rhinitis, chronic rhinosinusitis, and asthma, are complex and influenced by the exposome, microbiome, individual genetics, and epigenetics. Here, we review the role of the epithelial barriers of the skin, digestive tract, and ai

Bacterial Metabolites and Inflammatory Skin Diseases.

The microbiome and gut-skin axis are popular areas of interest in recent years concerning inflammatory skin diseases. While many bacterial species have been associated with commensalism of both the skin and gastrointestinal tract in certain disease states, less is known about specific bacterial metabolites that regulate host pathways and contribute to inflammation. Some of these metabolites include short chain fatty acids, amine, and tryptophan derivatives, and more that when dysregulated, have deleterious effects on cutaneous disease burden. This review aims to summarize the knowledge of weal

Case report: Dupilumab for the treatment of bullous pemphigoid.

Bullous pemphigoid (BP) is an inflammatory subepidermal blistering disease with rising prevalence in the elderly individuals. Cytokines associated with Th2-type immune response play an important role in the pathogenesis of BP. The traditional glucocorticoids and immunosuppressants are recommended as first-line drugs, but their therapeutic use is limited by numerous adverse effects. Dupilumab is a humanized monoclonal antibody that targets IL-4 and IL-13. It has recently been approved for the treatment of atopic dermatitis and may be an attractive therapeutic option for BP as well. Herein, we r

Targeted Molecular Therapies in Allergy and Rhinology.

Biologic agents, monoclonal antibodies that target highly-specific molecular pathways of inflammation, are becoming integrated into care pathways for multiple disorders that are relevant in otolaryngology and allergy. These conditions share common inflammatory mechanisms of so-called Type 2 inflammation with dysregulation of immunoglobulin E production and eosinophil and mast cell degranulation leading to tissue damage. Biologic agents are now available for the treatment of chronic rhinosinusitis with nasal polyps (CRSwNP), asthma, eosinophilic granulomatosis with polyangiitis (EGPA), atopic d

4-month-old boy coinfected with COVID-19 and adenovirus.

A 4-month-old boy with a history of muscular ventricular septal defect and atopic dermatitis presented with decreased oral intake, loose stools, stuffy nose, mild cough and diaphoresis. The patient had an in-home exposure to COVID-19. The initial respiratory pathogen panel was positive for adenovirus, consistent with his symptoms. The following day, the COVID-19 PCR was also positive. The patient was treated with supportive care, isolation precautions were implemented and the patient was discharged on day 4. This case demonstrates the importance of testing for COVID-19 even if a patient tests

JAK/STAT pathway modulation: Does it work in dermatology?

Janus kinase-signal transducer and activator of transcription (JAK/STAT)-is an intracellular signaling pathway, which plays a key role in downstream transmission of extracellular signals from cell membrane to the cell nucleus. This pathway is activated by cytokines, which participate in inflammation, innate and acquired immune responses, and also cell growth. Recent studies point out possible disturbances in JAK/STAT pathway in various inflammatory and autoimmune skin diseases, such as atopic dermatitis, psoriasis, alopecia areata. Several molecules that modulate-inhibit-this pathway are curre

Regulatory T cells in inflammatory skin disease: from mice to humans.

The skin is the largest organ in the body and one of the primary barriers to the environment. In order to optimally protect the host, the skin is home to numerous immune cell subsets that interact with each other and other non-immune cells to maintain organ integrity and function. Regulatory T cells (Tregs) are one of the largest immune cell subsets in skin. They play a critical role in regulating inflammation and facilitating organ repair. In doing so, they adopt unique and specialized tissue-specific functions. In this review, we compare and contrast the role of Tregs in cutaneous immune dis

Molecular and cellular discoveries in inflammatory dermatoses.

It was no earlier than 1986 that T helper (Th)1 and Th2 cells were described for the first time, opening the field of lymphocyte diversity and the investigation of the physiopathology of inflammatory diseases such as atopic dermatitis and psoriasis. Since that time, much research has been carried out showing a very complex communication network leading to inflammatory responses. Nowadays, understanding the cellular and molecular components of the inflammatory network and of the different crosstalks not only for groups of diseases but also for the individual patient is mandatory for developing

The Role of Intereukin-31 in Pathogenesis of Itch and Its Intensity in a Course of Bullous Pemphigoid and Dermatitis Herpetiformis.

Itch which is one of the major, subjective symptoms in a course of bullous pemphigoid and dermatitis herpetiformis makes those two diseases totally different than other autoimmune blistering diseases. Its pathogenesis is still not fully known. The aim of this research was to assess the role of IL-31 in development of itch as well as to measure its intensity. Obtained results, as well as literature data, show that lower concentration of IL-31 in patients' serum may be correlated with its role in JAK/STAT signaling pathway which is involved in development of autoimmune blistering disease. Intens

Filaggrin - revisited.

Profilaggrin (proFLG) and its processing products are critical to the health and appearance of skin. The recent identification of loss-of-function filaggrin (FLG) mutations as a predisposing factor in ichthyosis vulgaris and atopic dermatitis has lead to a resurgent interest in this enigmatic protein. Here, we review the literature on the structure and many functions of proFLG, from its role as a filament-aggregating protein and a source of natural moisturizing factor (NMF), to the more recent discoveries of its role in epidermal barrier formation and its more speculative functions as an antim

Recurrent Wells' syndrome associated with allergic asthma exacerbation.

Wells syndrome is an inflammatory eosinophilic dermatosis of unknown pathogenesis characterized by clinical polymorphism, a suggestive but nonspecific histopathologic traits, usually with a recurrent course and inconstant response to therapy. It seems to be an unspecific hypersensitivity reaction in response to various exogenous and endogenous stimuli, such as insect bites, infections, drug eruption or underlying internal disorders. We present a patient with allergic asthma and atopic dermatitis in whom a skin eruption developed in the sequence of allergic asthma exacerbation, which was clinic

Mast cells: novel clinical perspectives from recent insights.

Mast cells are still generally viewed as mediators of type I allergic or pseudoallergic reactions. Research over the past 10 years revealed that our view was too small and that mast cells are of key importance in innate immunity and also types II, III and IV adaptive immune reactions. Understanding their role in modulating and amplifying of inflammatory responses provides important insights into the pathogenesis of skin diseases such as psoriasis, atopic dermatitis, bullous pemphigoid or the control of infections. This helps us to understand the course of these diseases, their trigger mechanis

Tacrolimus in rheumatoid arthritis.

Tacrolimus is an immunosuppressive drug that has been used widely in organ transplantation and topically for atopic dermatitis. Tacrolimus exerts its immunosuppressive effects by the inhibition of calcineurin, leading to interference with T-cell activation. As T-cell activation plays a major role in the pathogenesis of rheumatoid arthritis, there has been an interest in the use of tacrolimus for the treatment of rheumatoid arthritis. The pharmacological properties of tacrolimus have the potential of suppressing the production of inflammatory cytokines, improvement of joint inflammation, improv

Standardized Clinical Photography for Improving Dermatological Treatment Adherence (Scoping Review): Evidence Landscape and Research Gaps.

Treatment adherence remains suboptimal across chronic dermatological conditions, contributing to poor clinical outcomes and increased healthcare burden. Despite the established role of standardized clinical photography in clinical documentation, its specific application as an adherence-promoting intervention remains poorly characterized. This scoping review aimed to systematically map the literature on photographic feedback in dermatology, identify research gaps, and propose methodological recommendations for future randomized controlled trials (RCTs). We conducted a scoping review according t

Postbiotics as Emerging Therapeutics for Allergic Diseases: A Novel Approach Beyond Live Biologics.

Allergic diseases, including asthma, atopic dermatitis, allergic rhinitis, and food allergies, are characterized by immune dysregulation, epithelial barrier dysfunction, and exaggerated type 2 inflammation. Although current therapeutic strategies have improved disease management, many treatments remain symptomatic, costly, and insufficiently effective in a substantial proportion of patients. Increasing recognition of the gut-immune axis has shifted attention toward microbiome-derived therapeutic approaches. However, safety concerns and inconsistent clinical outcomes associated with live probio

Engraftment of staphylococcal strains on human skin can competitively displace native staphylococci.

Topical probiotic formulations containing live bacteria are being explored as treatments for skin disorders such as atopic dermatitis and acne vulgaris. Certain bacterial strains with antimicrobial and/or anti-inflammatory properties show promise as bacterial therapeutics that could improve these conditions and serve as alternatives to antibiotics, which are increasingly compromised by rising antimicrobial resistance. However, little is known about the engraftment efficacy of such bacterial strains or their impact on the native skin microbiome. In this study, we applied two different coagulase
