Serial echocardiographic assessment of neo-aortic regurgitation and root dimensions after the modified Ross procedure.

BACKGROUND AND AIM OF THE STUDY: Concern exists regarding progressive root dilatation after the modified Ross procedure. The present prospective echocardiographic study aimed to provide further insight into neo-aortic regurgitation (nAR) and neoaortic root dimensions over time in adult Rotterdam Ross root patients, and to study potential risk factors for nAR and dilatation. METHODS: All Rotterdam Ross patients aged > or = 16 years at surgery were subjected to a prospective biennial standardized echocardiographic protocol. Analysis over time of nAR according to the jet length and jet diameter

Valve-sparing aortic root replacement in bicuspid aortic valves: a reasonable option?

OBJECTIVES: Aortic dilatation occurs in many patients with bicuspid aortic valves. We have added root replacement using the remodeling technique originally designed for tricuspid aortic valves to bicuspid aortic valve repair for treatment of the dilated root. We compared the results of remodeling in bicuspid aortic valves with those in tricuspid aortic valves. METHODS: From October 1995 through January 2004, 60 patients underwent root remodeling for bicuspid aortic valves (group A), and 130 patients underwent root remodeling for tricuspid aortic valves (group B). Correction of cusp prolapse w

Frequency analyses of CSF flow on cine MRI in normal pressure hydrocephalus.

Our objective was to clarify intracranial cerebrospinal fluid (CSF) flow dynamics in normal-pressure hydrocephalus (NPH). Frequency analyses of CSF flow measured with phase-contrast cine MRI were performed. The CSF flow spectra in the aqueduct were determined in patients ( n=51) with NPH, brain atrophy or asymptomatic ventricular dilation (VD), and in healthy volunteers (control group; n=25). The changes in CSF flow spectra were also analyzed after intravenous injection of acetazolamide. Moreover, a phase transfer function (PTF) calculated from the spectra of the driving vascular pulsation and

Bell-bottom aortoiliac endografts: an alternative that preserves pelvic blood flow.

OBJECTIVE: Dilated common iliac arteries that complicate aortic aneurysm stent grafting usually have been managed with endograft extension across the iliac artery bifurcation with internal iliac artery (IIA) occlusion. We studied 25 patients with significant common iliac artery (CIA) dilation treated with two methods: endograft extension across the iliac bifurcation or a new approach with a flared cuff within the CIA that preserves the IIA. METHODS: Of 86 patients with abdominal aortic aneurysm (AAA) who underwent bifurcated endovascular stent grafting (ESG), 25 (29.1%) had at least one dilat

Sonographic diagnosis of small intestinal anisakiasis.

PURPOSE: We studied the utility of sonography in the early diagnosis of anisakiasis of the small intestine. METHODS: The imaging findings and clinical data of 12 patients with proven small intestinal anisakiasis were retrospectively reviewed. RESULTS: Dominant sonographic findings were ascites, small bowel dilatation, and focal edema of Kerckring's folds. CONCLUSIONS: Patients who have acute abdomen after recent ingestion of seafood and in whom ascites, small bowel dilatation, and focal edema of Kerckring's folds are seen by sonography should be suspected of having small intestinal anisakia

[Dolichoectatic intracranial arteries. Advances in images and therapeutics].

Dolichoectasia of intracranial arteries is an infrequent disease with an incidence less than 0.05% in general population. It represents 7% of all intracranial aneurysms. Commonly seen in middle age patients with severe atherosclerosis and hypertension, the affected arteries include the basilar artery, supraclinoid segment of the internal carotid artery, middle, anterior and posterior cerebral arteries; males are more frequently affected. The clinical features of these fusiform aneurysms are divided in three categories: ische-mic, cranial nerve compression and signs from mass effect. Hemorrhage

Urethral catheterization in spinal surgery: a randomized prospective study.

In a prospective randomized study the effect of the use of an intraoperative indwelling urethral catheter (IUC) on urinary complications was investigated in patients undergoing spinal fusion. Two groups were formed; 16 patients received an intraoperative IUC and 16 patients had no intraoperative catheter (NC). All patients were, if necessary, intermittently catheterized in the postoperative period. Seven of the patients in the IUC group had positive cultures, defined as > or = 100,000 CFU/ml, compared with two patients in the NC group (n.s.). Another four patients in the NC group had cultures

Retinal ischemia in diabetic retinopathy.

Eight patients with proliferative diabetic retinopathy developed extensive retinal arteriolar and capillary obstruction. Ophthalmoscopy showed many white, thread-like retinal arterioles associated with capillary and venous dilatation. Widespread retinal arteriolar and capillary nonperfusion was demonstrated by fluorescein angiography. Ischemic maculopathy resulted in severe loss of visual acuity in some eyes. The severe degree of retinal ischemia was accompanied by optic disc pallor and neovascularization and a high incidence of rubeosis iridis with neovascular glaucoma. Patients with this var

Chlordecone-induced potentiation of carbon tetrachloride hepatotoxicity: a light and electron microscopic study.

Previous studies have shown that a chlorinated pesticide, chlordecone (Kepone), greatly potentiates carbon tetrachloride (CCl4) hepatotoxicity and lethality (Curtis, L.R., Williams, W.L., and Mehendale, H.M. (1979). Toxicol. Appl. Pharmacol. 51, 283-293; Curtis, L.R., and Mehendale, H.M. (1980). Drug Metab. Dispos. 8, 23-27). The present study describes sequential morphologic changes which occurred in livers of rats given a "nontoxic" level of chlordecone (10 ppm for 15 days) followed by a single injection of CCl4 (0.1 ml/kg). The hepatic alterations were examined 1 to 36 hr after exposure of

Diagnosis of Nipple Discharge: Value of Magnetic Resonance Imaging and Ultrasonography in Comparison with Ductoscopy.

BACKGROUND: Pathologic nipple discharge, which is a common reason for referral to the breast imaging service, refers to spontaneous or bloody nipple discharge that arises from a single duct. The most common cause of nipple discharge is benign breast lesions, such as solitary intraductal papilloma and papillomatosis. Nevertheless, in rare cases, a malignant cause of nipple discharge can be found. AIMS: To study the diagnostic value of ultrasonography, magnetic resonance imaging, and ductoscopy in patients with pathologic nipple discharge, compare their efficacy, and investigate the importance

Sexual Devices and Erotica.

INTRODUCTION: Sexual wellness is directly associated with overall physical and mental health. Sex span, which is the period spent enjoying a satisfying, vibrant, and joyful sexual life, is directly correlated to quality of life. Gynecologic surgeons are well-positioned to discuss sexual function with their patients as it pertains to a variety of pathologic pelvic conditions, including pelvic pain disorders, urinary/bowel incontinence, pelvic organ prolapse, as well as urogenital and non-urogenital cancers and other related conditions. OBJECTIVE: Describe sexual devices, tools, and erotica com

A Rare Cause of Small Bowel Obstruction: Malignant Metastatic Sarcoma.

Small bowel obstruction is a common reason for hospital admission and is most frequently caused by postoperative adhesions. Malignancy is a less common etiology and typically results from a focal obstructing lesion. We present the case of a 67-year-old female who developed small bowel obstruction due to diffuse sarcomatous implants throughout the abdomen and pelvis. Computed tomography demonstrated dilated small bowel with a pelvic transition point. After failure of conservative management, the patient underwent surgical exploration, which revealed hundreds of small malignant lesions involving

Takayasu Arteritis Complicated with Vertebral Artery Dissection Aneurysm Treated Endovascularly: Report of One Case.

Takayasu arteritis (TA) is a chronic granulomatous large-vessel vasculitis that primarily affects the aorta and its major branches, such as the subclavian, carotid, and renal arteries. Pathologically, it is characterized by transmural infiltration of lymphocytes and macrophages with granulomatous inflammation, leading to intimal fibrosis and thickening, destruction of medial elastic fibers, and subsequent luminal stenosis or occlusion. Aneurysmal dilation may occur in some cases. We here report a case of TA complicated by a vertebral artery dissecting aneurysm. Our patient was a 47-year-old ma

A Novel Mouse Model for Venous High-Load Remodeling Induced by Unilateral Jugular Vein Transection.

Chronic venous disease (CVD) is a prevalent peripheral vascular disorder characterized by vascular remodeling driven by hemodynamic overload, yet available animal models are limited by high surgical difficulty and unstable induction efficiency. Here, we developed a novel mouse venous high-load remodeling model by selectively ablating the unilateral cervical venous drainage system. After 4 weeks, compensatory contralateral jugular veins were assessed via ultrasonography, histology, Western blot, and bulk RNA sequencing. The model displayed marked luminal dilation, wall hypertrophy, and excessiv

Cystic Neutrophilic Granulomatous Mastitis: Imaging Features With Histopathologic Correlation.

Cystic neutrophilic granulomatous mastitis (CNGM) is a rare type of granulomatous lobular mastitis (GLM) with a distinct histologic pattern characterized on histopathology by clear lipid vacuoles lined by peripheral neutrophils ("suppurative lipogranulomas"), often containing gram-positive bacilli and strongly associated with Corynebacterial infection (in particular, Corynebacterium kroppenstedtii). Cystic neutrophilic granulomatous mastitis has a distinct histopathologic appearance, but the imaging appearance is less well described and has been limited to case reports and small case series pu

Cardiology's best friend: Using naturally occurring disease in dogs to understand heart disease in humans.

Heart diseases are a leading cause of death globally. Laboratory and preclinical animal models of disease have been critical in advancing our understanding of the mechanisms of pathology, creating diagnostic tools, and developing therapeutic interventions. However, fundamental biological dissimilarities between humans and rodents limits their usefulness in research, and the induction of disease in an otherwise healthy animal creates unrealistic conditions under which diseases are typically studied. Dogs are at high risk of acquiring and dying from several naturally occurring heart disorders th

Perivascular adipose tissue inflammation in abdominal aortic aneurysm: An underrecognized contributor.

Abdominal aortic aneurysm (AAA) is a life-threatening vascular disorder characterized by localized dilation of the abdominal aorta and a high mortality rate once rupture occurs. At present, therapeutic approaches for AAA are primarily confined to operative repair. Thus, a comprehensive understanding of its pathogenesis is crucial for the development of novel pharmacological interventions. Although the underlying mechanisms remain incompletely defined, several critical pathological features have been identified, including inflammation, extracellular matrix degradation, and loss of vascular smoo
