[Variability of blood pressure and baroreceptor function; clinical and scientific relevance].

The human arterial blood pressure shows not only exogenic alterations, but also spontaneous fluctuations. This blood pressure variability has both scientific and clinical relevance. A well-known phenomenon is the so-called "white coat hypertension" which may severely affect diagnosis and therapy of essential hypertension. Furthermore, several studies have provided evidence that the degree of end-organ damage in hypertensive patients is closely related to the circadian blood pressure variability. Therefore, it is tempting to suggest that an optimal antihypertensive treatment should take an impr

[Fungal pneumonia].

Concomitant with advances in medicine, involving the development and use of antibiotics, immunosuppressive drugs, organ/stem cell transplantation and pre-existing illnesses such as AIDS, the frequency of fungal infection has increased dramatically. These factors determine the susceptibility of the lung for an acute or chronic course of disease. Early detection, characterisation and follow-up of the pulmonary infiltrates is the major task of the radiologist. The correct timing to perform an investigation and adequate technique are an additional essential factor in this setting. Furthermore, rad

Adherence mechanisms of Candida albicans.

The yeast Candida albicans is an opportunistic fungal pathogen that is capable of inducing a range of superficial and systemic diseases in the immunocompromised host. Although it displays a variety of virulence factors, one--the ability to adhere to host tissue--is considered essential in the early stages of colonisation and tissue invasion. Adherence is achieved by a combination of specific (ligand-receptor interactions) and non-specific (electrostatic charge, van der Waals forces) mechanisms which allow the yeast to attach to a wide range of tissue types and inanimate surfaces. Conventional

Effects of atropine and L-NAME on cutaneous blood flow during body heating in humans.

We sought to investigate further the roles of sweating, ACh spillover, and nitric oxide (NO) in the neurally mediated cutaneous vasodilation during body heating in humans. Six subjects were heated with a water-perfused suit while cutaneous blood flow was measured with a laser-Doppler flowmeter. After a rise in core temperature (1. 0 +/- 0.1 degrees C) and the establishment of cutaneous vasodilation, atropine and subsequently the NO synthase inhibitor N(G)-nitro-L-arginine methyl ester (L-NAME) were given to the forearm via a brachial artery catheter. After atropine infusion, cutaneous vascular

[What tests should be conducted for secondary arterial hypertension in hypertensive patients resistant to treatment?].

UNLABELLED: The aim of this study was to determine the prevalence of secondary hypertension in a population of refractory hypertension without initial clinical or biological findings suggesting identifiable causes of hypertension. METHODS: A survey included 200 consecutive hypertensive patients referred to hypertension specialists for refractory hypertension (BP > 140/90 mmHg on at least two antihypertensive drugs). Prior inclusion, the permanent elevation of blood pressure was confirmed by ambulatory monitoring. In each subject an extensive work-up was performed to detect a secondary cause f

Hypertension and the kidneys--inter-relationship and therapeutic approach.

Over the last decade, a great body of evidence has pointed towards the role of kidneys in the genesis and maintenance of a hypertensive state. There exists a relationship between a rise in the blood pressure and a proportionate increase in the urinary excretion of sodium and water called the pressure-natriuresis curve. Chronic renal diseases are the most common causes of secondary hypertension accounting for 2-5% of all cases of secondary hypertension. The prevalence rate of renovascular hypertension based upon referral patterns range from 0.2% to 10% of hypertensive population. The data provi

Angiotensin II receptor blockers. A new class of antihypertensive drugs.

Angiotensin II (AII) receptor antagonists, a new class of antihypertensive agents, recently became available for the treatment of clinical hypertension. These agents have a unique mechanism of action: they selectively block the AII receptor type I thereby blocking all known physiological actions of AII that are relevant to hypertension. Controlled clinical trials have shown losartan potassium, the first of the AII receptor type I antagonists approved for clinical use, to be effective, providing long-term control of blood pressure in once daily oral doses in patients who have mild to moderate e

[Study and treatment of varicose veins. Truths and counter-truths].

Varicose veins are not the only symptomatology of chronic venous insufficiency of the lower limbs, which has other clinical manifestations: purely symptomatic problems, varicosities, edema and trophic skin lesions. The management of varicose veins is base upon thorough clinical examination. Doppler and ultrasonography are useful in cases of varicose veins in which radical treatment is envisaged. Ultrasound investigation is essential in the presence of cutaneous ulceration in order to avoid mistaking a trophic lesion due to incompetence of deep veins, most often of post-thrombotic origin, for a

[Treatment of algodystrophies. The point of view of an anesthetist].

Algodystrophy is a complex and heterogenous syndrome, better defined as a subtype of Reflex Sympathetic Dystrophy syndrome (RSD). The pathophysiological theory of RSD has been supported by basic studies and clinical efficacy of sympathetic blocks. Results may be good, but some of RSD are not responsive to sympathetic blocks, and distinction between sympathetic dependent and independent pain has been proposed as subtypes of RSD. Physiotherapy is an essential part of RSD treatment. Sympathetic blocks and other analgesic therapies are performed in order to permit painless action of the physiother

The effects of a monoclonal antibody to interferon-gamma on experimental autoimmune thyroiditis (EAT): prevention of disease and decrease of EAT-specific T cells.

CBA/J mice immunized with thyroglobulin (Tg) develop an experimental autoimmune thyroiditis (EAT) with lymphocytic infiltration of the thyroid glands, autoantibodies to Tg and occurrence of EAT-specific T cells. When these mice were treated for 4 weeks after immunization with 1 mg/week of a monoclonal antibody (mAb) that neutralizes the activity of interferon-gamma (IFN) a beneficial effect on the onset of EAT was observed. Characteristic features of EAT were significantly reduced, including the lymphocytic infiltrations of the thyroid glands and the serum levels of autoantibodies to Tg. Moreo

Diagnosis and management of chronic hypertension in pregnancy.

Pregnancies complicated by chronic hypertension are at increased risk for the development of superimposed preeclampsia, abruptio placentae, and poor perinatal outcome. The frequency of these complications is particularly increased in patients with severe hypertension and those with preexisting cardiovascular and renal disease. Such women should receive appropriate antihypertensive therapy and frequent evaluations of maternal and fetal well-being. In contrast, in patients with mild essential chronic hypertension, the maternal and perinatal benefits from antihypertensive medications are highly c

Modern techniques in the diagnosis and treatment of gastrointestinal and biliary tree parasites.

Parasitic infestations are endemic in tropical and subtropical areas, but rarely occur in temperate zones, and are imported by tourists, immigrants and expatriates. Gastrointestinal and biliary tree parasites are the commonest helminthics in humans. Previously these were diagnosed only by stool examinations, but recently other diagnostic techniques have been used. These include fibreoptic endoscopies for upper or lower gastrointestinal tract and biliary tree. Endoscopy plays an important role in diagnosis, treatment and follow-up as in gastric anisakiasis, chronic giardiasis, strongyloides, he

Narcolepsy update.

Narcolepsy, a disorder of excessive daytime sleepiness that affects more than 125,000 people in the United States, is technically defined as a daytime mean sleep latency (time elapsed before falling asleep) of less than 5 minutes in conjunction with verification of rapid eye movement sleep in at least two of five daytime nap periods. Cataplexy, hypnagogic hallucinations, and sleep paralysis are frequently associated with narcolepsy. Currently, overnight polysomnography and multiple sleep latency testing in a sleep disorders laboratory are used to diagnose narcolepsy. Standard pharmacologic the

[Acute respiratory distress syndrome caused by tuberculosis].

The adult respiratory distress syndrome (ARDS) is rarely due to tuberculosis. Two new cases are reported here. Both were female patients, aged 33 and 41 years. The first, of North African origin, was admitted for epigastralgia, hyperpyrexia and intestinal problems. She underwent an exploratory laparotomy, which only showed oedematous mesenteries. Hepatic and lymph node biopsies revealed an ongoing tuberculosis. On the 4th postoperative day, she developed ARDS. Despite an initial period of improvement after proper treatment (antituberculous drugs, steroids, positive pressure ventilation) had be

Antihypertensive efficacy of pinacidil--automatic ambulatory blood pressure monitoring.

Forty-three patients with mild essential hypertension were randomized into two double-blind studies: pinacidil vs. placebo or pinacidil vs. hydralazine. Pinacidil (62 +/- 18 mg/day) decreased office systolic and diastolic blood pressures from 145 to 137 mm Hg and from 98 to 89 mm Hg, respectively, after 6 weeks of therapy. Similarly, hydralazine (128 +/- 28 mg/day) reduced supine systolic blood pressure from 140 to 134 mm Hg and supine diastolic blood pressure from 93 mm Hg to 84 mm Hg. Significant tachycardia was not noted with either drug. Ambulatory blood pressure was monitored for 24 h dur

Suppression of drinking by naloxone in rats homo- and heterozygous for diabetes insipidus.

The effects of the opiate antagonist, naloxone, alone and in combination with morphine, were examined on drinking induced by water deprivation in homo- and heterozygous Brattleboro rats manifesting an inherited diabetes insipidus. Both naloxone and a structurally-related congener, naltrexone (0.01-10 mg/kg), attenuated water consumption is a dose-related fashion of 1 hr water-deprived homozygotes, which exhibit a complete absence of vasopressin. Drinking was also reduced by the two drugs in 24 hr water-deprived heterozygotes, which have detectable levels of vasopressin. Morphine pretreatment e

Hypertension in general practice. Part III. Treatment of hypertensive heart failure and hypertensive emergencies.

The therapy of hypertension with heart failure is a classic illustration of the value of agents reducing the afterload, being a specialized example of the general principle of the modern treatment of heart failure by vasodilator therapy. Agents such as diuretics and vasodilators are used in the therapy of heart failure in general and also in the therapy of hypertensive heart failure. However, drugs such as digoxin, which primarily and directly increase the inotropic state of the heart, should not be used without careful consideration, because the basic problem is not the defective inotropic st

Drug repurposing in oncology: Bridging computational discovery to clinical application.

Drug repurposing, the identification of new therapeutic applications for existing drugs, has emerged as a pragmatic and cost-efficient strategy to accelerate oncology drug discovery. Faced with rising development costs, protracted timelines, and high attrition rates associated with traditional de novo drug development, repurposing leverages known pharmacokinetics, safety profiles, and manufacturing processes to expedite clinical translation. This review synthesizes current advances in computational and experimental methodologies and mechanistic insights that drive drug repurposing for cancer t
