Biomechanical insights through finite element analysis of Bennett fracture fixation: a comparative study of four surgical techniques.

BACKGROUND AND OBJECTIVES: Bennett fractures are intra-articular fractures of the first metacarpal, typically resulting from axial loading. The optimal surgical method for fixation remains a topic of ongoing debate. This study aims to evaluate and compare the biomechanical properties of different fixation techniques. MATERIALS AND METHODS: Four fixation models were analyzed: the Lag Screw (LS) model, the Plate-screw (PS) model, and two K-wire models (Kw1 and Kw2). The biomechanical stability of each model was assessed by measuring the total displacement at the fracture line and the maximum vo

[Obesity, fat and bones: friends or foes ?].

Low fat mass is associated with an increased risk of fracture because of low bone mineral density (BMD) and altered bone micro-architecture. Conversely, overweight and obese patients also have an increased risk of fracture, particularly of the humerus and ankle, despite greater BMD. Visceral abdominal fat, which is the most metabolically active, may be associated with poorer quality of bone tissue properties, as suggested in diabetes. Other factors may contribute to higher fracture risk in overweight patients, notably higher frequency of falls and lower bioavailability of vitamin D stoked in f

A multi-domain computational framework investigating the short- and long-term viability of bioabsorbable magnesium fixation for tibial fractures.

This study presents a multi-domain computational framework to investigate the long-term performance of permanent and bioabsorbable magnesium fixation devices in orthopaedic fracture applications. The framework integrates a coupled model for bone fracture healing and remodeling, with an enhanced surface-based corrosion model to predict the performance of bioabsorbable magnesium devices. It was found that plated fracture fixation enabled fracture healing outcomes compare to non-plated models by facilitating direct fracture healing. During the fracture healing phase, it was found that the stiff t

Fractures in chronic kidney disease: pursuing the best screening and management.

PURPOSE OF REVIEW: Osteoporotic fractures are common and cause increased sickness and death. Men and women with chronic kidney disease (CKD) are at particularly high risk of osteoporotic fractures. Currently, however, there are no guidelines concerning noninvasive methods to assess fracture risk in CKD. Further, approved treatments to prevent fractures in otherwise healthy men and women are only recommended for use with caution in those with CKD. This review focuses on the recent data that support the use of noninvasive methods to assess fracture risk in CKD and highlights new therapies that c

Is intrasound vibration useful in the diagnosis of occult scaphoid fractures?

This study was designed to confirm the results of Finkenberg et al. (J Hand Surg 1993;18A: 4-7), who found a high sensitivity (100%) and specificity (95%) of the intrasound vibration method in diagnosing occult scaphoid fractures. These occult scaphoid fractures are not visible on x-ray films, but clinically the patients are suspected of having a scaphoid fracture. A vibratory apparatus is placed over the anatomical snuff-box and a vibration of 100 mW is emitted; a painful sensation is produced if the scaphoid is fractured. Thirty-seven consecutive patients with a clinically suspected scaphoid

Intraosseous myolipoma of the calcaneus.

A man in his 40s presented with an incidental finding of an osteolytic bone lesion. He sustained an ankle injury while inline skating, fracturing his lateral malleolus. Besides the fracture, radiographic imaging on the day of the injury incidentally revealed a well-defined solitary osteolytic lesion with a sclerotic rim within the right calcaneus. MRI showed an intraosseous, fat-containing lesion with focal contrast enhancement, assessed as an intraosseous lipoma with central necrosis. In the pathological analysis of a sample of the lesion an intraosseous myolipoma of the calcaneus was found-a

Aromatase inhibitors and bone.

The decrease in estrogen levels with the use of aromatase inhibitors results in an increase in the rate of bone remodelling. This result in an acceleration of bone loss, and probably to an increase in the risk of fractures. The risk of fracture is particularly high in the older woman and in the woman with a low bone mineral density. We have a number of proven treatments for the treatment of postmenopausal osteoporosis and it is likely that some of these, particularly bisphosphonates, could be effective at preventing bone loss with aromatase inhibitors.

Gout and Risk of Fracture in Women: A Prospective Cohort Study.

OBJECTIVE: Uric acid may be linked to bone health through its antioxidant or prooxidant effects, thereby affecting bone resorption and formation, or through its dual inhibition of vitamin D activation and parathyroid hormone production. Results of prior studies on the relationship between uric acid and bone mineral density have been conflicting. This prospective study was undertaken to examine the relationship between gout, a disease characterized by hyperuricemia and inflammation, and risk of hip or wrist fracture in women. METHODS: We conducted a prospective observational study of gout and

Wearable Electrical-impedance-tomography Armband for Noninvasive and Continuous Bone Monitoring.

Medical inspection systems such as digital radiography (DR) and computed tomography (CT) are limited in the continuous monitoring of fracture healing due to cumulative radiation exposure. This work presents a wearable 16-electrode armband incorporating electrical impedance tomography (EIT) for noninvasive, continuous monitoring of arm bones. The EIT armband applies stimulation current on the skin surface with a defined drive pattern and detects response voltages across electrodes. These acquired signals, combined with reference signals from an artificial model that only comprises skin, fat, an

A Systematic Review Comparing the Outcomes of Cemented Versus Uncemented Stems in Femoral Impaction Bone Grafting for Revision Hip Arthroplasty.

Femoral impaction bone grafting is a crucial technique in revision hip arthroplasty, addressing bone loss and ensuring implant stability. The choice between cemented and uncemented stems significantly influences the outcomes and long-term success of the procedure. This systematic review aims to compare the clinical outcomes of cemented versus uncemented stems in femoral impaction bone grafting. A comprehensive search of PubMed, MEDLINE Complete, and the Cochrane Library databases was conducted in accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidel

Osteoporosis and Fracture Risk among Older US Asian Adults.

PURPOSE OF REVIEW: This review summarizes the current knowledge regarding osteoporosis and fracture among older US Asian adults. RECENT FINDINGS: Asian adults have lower (areal) bone density than non-Hispanic White adults and thus are more likely to be diagnosed and treated for osteoporosis, despite their lower risk of hip fracture. The latter may relate to favorable characteristics in hip geometry, volumetric bone density, and bone microarchitecture; lower risk of falls; and other clinical factors. The fracture risk calculator FRAX accounts for the lower risk of hip fracture among US Asian a

Unsuspected lymphoma diagnosed with use of biopsy during kyphoplasty.

BACKGROUND: Vertebral augmentation procedures are currently widely performed to treat vertebral compression fractures. In selecting appropriate patients for these procedures, it is important to distinguish the pain caused by a fracture from other causes of back pain. The purpose of this study was to determine the frequency of underlying, previously unrecognized malignant tumors in a consecutive series of patients undergoing kyphoplasty to treat vertebral compression fractures. Our hypothesis was that an unsuspected malignant tumor will exist and that a bone-marrow aspiration from the iliac cre

Changes in non-enzymatic glycation and its association with altered mechanical properties following 1-year treatment with risedronate or alendronate.

SUMMARY: One year of high-dose bisphosphonate (BPs) therapy in dogs allowed the increased accumulation of advanced glycation end-products (AGEs) and reduced postyield work-to-fracture of the cortical bone matrix. The increased accumulation of AGEs in these tissues may help explain altered bone matrix quality due to the administration of BPs in animal models INTRODUCTION: Non-enzymatic glycation (NEG) is a posttranslational modification of the organic matrix that results in the formation of advanced glycation end-products (AGEs). In bone, the accumulation of AGEs play an important role in dete

[Guidelines for the development of anti-osteoporosis medications].

Osteoporosis is a general disorder of the skeleton characterised by a decrease in bone mass, with damage to the microarchitecture leading to an increase in bone fragility and fracture risk. The incidence of this illness will increase in the future because of the aging of the population and increasing risk factors. Many guidelines have been proposed by qualified authorities--those of the European Agency for the Evaluation of Medicinal Products (EMEA) being the latest published. The aim of treatment of the osteoporosis is to increase, maintain or improve bone mass as well as its strength, with a

Novel assessment of bone using time-resolved transcutaneous Raman spectroscopy.

UNLABELLED: With fragility fractures increasing as the population ages, there is a need for improved means to estimate risk of fracture. We recorded Raman spectra of both the mineral and organic phases of bone transcutaneously, a technology with potential to enhance bone quality and fracture risk assessment. INTRODUCTION: The current "gold standard" assessment of bone quality is BMD determined by DXA. However, this accounts for only 60-70% of bone strength. X-rays are absorbed by the mineral phase of bone, whereas the organic phase remains essentially invisible; however, bone strength is crit

Utility of Computed Tomography Angiography in Characterizing Vascular Injury From Sphenoid Fractures.

OBJECTIVES: Sphenoid fractures are associated with significant neurovascular risks, including blunt cerebrovascular injuries (BCVI). While computed tomography angiography (CTA) is commonly used to detect vascular injuries, its routine use in sphenoid fractures remains a debate. This study aimed to evaluate sphenoid fracture patterns associated with positive CTA findings to optimize imaging protocols. METHODS: This retrospective cohort study included 131 patients with sphenoid fractures from 2011 to 2023 at a tertiary care center. Of these, 88 patients underwent CTA. Fracture patterns were cat

Prevalence of Vitamin D Deficiency in Patients Admitted With Recent Fragility Hip Fracture: A Single-Center Retrospective Study.

Hip fractures are the most common low‑energy injury affecting older adults and place a large burden on healthcare systems. Vitamin D sufficiency is fundamental to healthy bone turnover, maintenance of muscular strength, and immune system homeostasis. We aimed to evaluate the prevalence of vitamin D deficiency among patients admitted with recent fragility hip fractures at a single orthopedic clinic over one year. This retrospective study included patients aged >50 years admitted to the General Hospital of Athens "KAT" with fragility hip fractures during 12 months. Preoperative assessment includ
