Changes in regional brain glucose metabolism measured with F-18-FDG-PET in essential tremor.

BACKGROUND: There is growing evidence that essential tremor (ET) is a multiple-system disorder. Previous PET studies in ET typically have me … the mean FTM score was 15.1 ± 4.9. Brain FDG-PET analysis demonstrated hypometabolism in the medial frontal lobe, medial temporal lobe, and the precuneus of parietal lobe. However, there was no significant difference of glucose metabolism in the cerebellum. CONCLUSION: We propose that motor symptom of ET are caused by electrophysiological disturbances within …

The effect of sex and handedness on white matter anisotropy: a diffusion tensor magnetic resonance imaging study.

Diffusion tensor magnetic resonance imaging provides a way of assessing the asymmetry of white matter (WM) connectivity, the degree of anisotropic diffusion within a given voxel being a marker of coherently bundled myelinated fibers. Voxel-based statistical analysis was performed on fractional anisotropy (FA) images of 42 right- and 40 left-handers, to assess differences in underlying WM anisotropy and FA asymmetry across the whole brain. Right-handers show greater anisotropy than left-handers in the uncinate fasciculus (UF) within the limbic lobe, and WM underlying prefrontal cortex, medial a

Reorganized cerebral metabolic interactions in temporal lobe epilepsy.

Patients with left temporal lobe epilepsy demonstrate language impairments that are not well understood. To explore abnormal patterns of brain functional connections with respect to language processing, we applied a principal component analysis to resting regional cerebral metabolic data obtained with positron emission tomography in patients with right- and left-sided temporal lobe epilepsy and controls. Two principal components were expressed differentially among the groups. One principal component comprised a pattern of metabolic interactions involving left inferior frontal and left superior

Demyelinating Neurobrucellosis Presenting With Neuropsychiatric Manifestations.

Neurobrucellosis with psychiatric manifestations is an uncommon manifestation of brucellosis. Herein, we report a 31-year-old male complaining of dizziness, nausea, malaise, imbalance, amnesia, psychosis, delusion, and impulsive and disorganized behavior. He was not responsive to antipsychotic treatment, and his brain MRI showed demyelinating hyperintensity foci in the periventricular and paraventricular areas and the subcortical areas of the right temporal and frontal lobes. His serum and cerebrospinal fluid serology were in favor of brucellosis, and he dramatically responded to anti-Brucella…

Update on meningioma: Clinical-radiological and radio-pathological correlation.

Meningiomas are tumors that originate in the arachnoid villi and are the most common non-glial neoplasm in the central nervous system. The clinical manifestations associated with meningioma depend, fundamentally, on its location. The location in the cerebral convexity is the most frequent, especially in the frontal lobes, manifesting with headache, motor disturbances, seizures and even neurocognitive disorders. There are 15 histologic subtypes of meningioma and three histologic grades. Within these, grades two and three have a worse prognosis and a higher rate of recurrence, as well as a radio…

Giant nasofrontal meningioma: endoscopic and transcranial approach followed by microvascularized vastus laterallis flap reconstruction.

A 12-year-old girl presented with headache, nasal voice, and anosmia. Magnetic resonance imaging demonstrated a 11 × 9 × 8-cm tumor extending from the nasal cavity to the frontal lobes. Histological analysis was consistent with transitional meningioma WHO grade I. As far as we know, this is the first reported case of a giant olfactory groove meningioma invading the nasal cavity through the anterior cranial base. Complete surgical removal was accomplished with excellent result via a staged endoscopic and transcranial approach. A microvascularized vastus laterallis flap was used for reconstructi…

Primary dural diffuse large B cell lymphoma mimicking parafalcine meningioma.

A 58-year-old immunocompetent woman presented with complaints of recent onset recurrent generalised tonic-clonic seizures and left hemiparesis. On MRI a dural-based heterogeneously enhancing mass was seen along the falx cerebri mimicking parafalcine meningioma, causing brain parenchymal compression and vasogenic oedema in the right frontal and parietal lobes. The tumour was completely excised and on subsequent investigation it was diagnosed as diffuse large B cell type primary dural lymphoma. Chemotherapy with CHOP (cyclophosphamide, hydroxyl doxorubicin, vincristine and prednisone) regimen wa

Ruptured posterior ethmoidal artery aneurysm and Moyamoya disease in an adult patient. Case report.

The association between Moyamoya disease and intracranial aneurysms is well described. In our case, we describe a unique aneurismal location and its management. We report the case of a 74-year-old woman affected by a Moyamoya disease who displayed a frontal lobe hematoma. The origin of the bleeding came from the rupture of a posterior ethmoidal artery aneurysm that was treated surgically with favourable outcome. This case of a ruptured posterior ethmoidal artery aneurysm in a Moyamoya patient illustrates the polymorphism of the vascular complications encountered in this disease. It stresses th…

Hypertrophic Pachymeningitis as a Potential Cause of Headache Associated with Temporal Arteritis.

We herein describe a rare case of temporal arteritis associated with hypertrophic pachymeningitis. An 81-year-old man presented with a right temporal headache that had persisted for one month. A right superficial temporal artery biopsy revealed intimal hypertrophy with increased elastic fibers, consistent with temporal arteritis. Brain MRI using gadolinium enhancement showed thickened dura mater on the right frontal and temporal lobes, which led to the diagnosis of hypertrophic pachymeningitis. Intravenous methylprednisolone and oral prednisolone improved the patient's symptoms. According to o

Pontocerebellar hypoplasia type 3 with tetralogy of Fallot.

We report a male infant with pontocerebellar hypoplasia type 3 and tetralogy of Fallot. He showed optic nerve atrophy, progressive microcephaly, severe psychomotor developmental delay, and vesicoureteral reflux. Magnetic resonance imaging revealed severe hypoplasia of the cerebellar vermis and hemisphere, and of the brainstem including the pons, and simplified gyral patterns in bilateral frontal lobes. An unknown etiology differing from other cases of PCH type 3 might have caused not only optic nerve atrophy and hypoplasia of the cerebellum and brainstem, but also cerebral and visceral malform…

[A case of A3-A3 bypass and proximal clipping for a ruptured left A1 dissecting aneurysm].

A 58-year-old woman was admitted to our hospital with sudden onset of headache. CT and angiogram revealed subarachnoid hemorrhage due to a dissecting aneurysm at the left A1 segment. ACoA was not identified. We performed proximal clipping with A3-A3 bypass for preventing the recurrence of the hemorrhage and for reducing ischemia in the territory of the left ACA. After the operation, CT revealed the infarctions in the corpus callosum, the left medial frontal lobe and the genu of the internal capsule. But the postoperative angiogram demonstrated no aneurysm and the opacification of the recurrent…

To lose the frame of action: a selective deficit in avoiding unpleasant objects following a unilateral temporal lobe lesion.

Studies on emotion and its neurobiology have been far more focused on the recognition of emotion than on actions that are caused by emotional states. We investigate the performance of a patient, HS, with a unilateral lesion to the left temporal pole and orbito-frontal cortex (OFC) (including left amygdala), on a well-established approach/avoid task that taps into emotion-driven action. The striking finding of the present study is a remarkable, and selective, slowing of HS's avoidance of unpleasant items in her (impaired) contralesional field. This finding suggests that the left temporal lobe a

Thinning of sensorimotor cortices in children with Tourette syndrome.

The basal ganglia portions of cortico-striato-thalamo-cortical (CSTC) circuits have consistently been implicated in the pathogenesis of Tourette syndrome, whereas motor and sensorimotor cortices in these circuits have been relatively overlooked. Using magnetic resonance imaging, we detected cortical thinning in frontal and parietal lobes in groups of Tourette syndrome children relative to controls. This thinning was most prominent in ventral portions of the sensory and motor homunculi that control the facial, orolingual and laryngeal musculature that is commonly involved in tic symptoms. Corre

[Vascular ischemic dementia].

Observations are reported of the course and other interrelations between clinical pattern and computer tomography results in 36 patients with vascular ischaemic dementia. Attention is called to the frequency of transient dementia-like disturbances following stroke and to the importance of the middle gyrus of the left frontal lobe in the development of dementia manifestations. In cases with slow progression of dementia symptoms and only scant neurological signs not infrequently long-standing improvement or even complete remission of dementia symptoms occur which sets them apart from mixed forms…

Primary progressive apraxia in Pick's disease: a clinicopathologic study.

A 62-year-old right-handed man gradually experienced increasing difficulty with speech and manual dexterity. He had apraxia of speech, buccofacial apraxia, and complex limb apraxia as well as terminal dementia. At autopsy, focal cortical atrophy, neuronal loss, and neuropil rarefaction in the second and third cortical layers were most prominent in the left opercular, lower precentral, superior parietal, and left temporal pole. Numerous Pick bodies were diffusely present in the temporal and posterior frontal lobes and, to a lesser degree, in the superior parietal lobule. This report demonstrate…

Lectin histochemistry of dermoid and epidermoid cysts of the central nervous system.

The binding affinities of seven lectins to four epidermoid and three dermoid cysts were studied and the results were compared with the epidermis and the oral mucosa. The reactivity of four epidermoid cysts of the fourth ventricle and the frontal lobe, and one dermoid cyst in the sylvian fissure was consistent with that of the skin, whereas the binding profile of one parasellar dermoid cyst was identical to that of the keratinized oral mucosa. Another parasellar dermoid cyst also showed close similarity to the buccal mucosa. These results indicate that cells of these cysts develop same glycocon…

[A case of hemiballismus during cryptococcal meningitis].

We present a case of hemiballismus related to cryptococcal meningitis. A 23 year-old man was hospitalized because of involuntary movements of his left side, confusion, hyperpyrexia, neck stiffness, bilateral papilledema, right hemiparesis and bilateral pyramidal signs. Diagnosis was made by CSF examination demonstrating cryptococci by india ink. CT with contrast showed hyperdense lesions in the head of the right caudate nucleus, in the left internal capsule and in the frontal and occipital lobes. After treatment with amphotericin B, 5-fluorocytosine and haloperidol, he experienced rapid recove

Reye syndrome: computed tomographic documentation of disordered intracerebral structure.

Four patients with well documented Reye syndrome were examined with noncontrast computed tomography. Acute findings are those of diffuse cerebral edema with low density in deep white matter, increased gray-white matter differentiation, and evidence of secondary ventricular compression. The diffuse nature of the changes is unlike the more focal pattern often seen in true viral encephalitis. Residual changes after severe diseases are those of nonspecific cerebral degeneration and are most pronounced in the frontal lobes. Atrophic ventricular dilatation was observed in two cases. Computed tomogra…

Preliminary Report on Neuroanatomical Differences Among Reading Disabled, Nonverbally Gifted, and Gifted-Reading Disabled College Students.

Regional comparisons (cortical surface area and thickness) were performed on a well described sample of adults with reading disability alone (RD), nonverbal giftedness alone (G), and reading disability and nonverbal giftedness combined (GRD). These anatomical results are considered in relation to behavioral and functional work previously reported on this sample. GRD-RD regional differences were found in both hemispheres and were more common than GRD-G differences. Regional differences were found in the temporal, parietal, occipital and frontal lobes. While these data are preliminary given the …
