Re-revision of failed revision Total Hip Arthroplasty acetabular components.

While revision of total hip arthroplasty (THA) is being performed with increasing frequency, outcomes of repeated revisions have been rarely reported in the literature. The purpose of this study was to report mid-term outcomes of re-revision of failed revision THA acetabular components. We performed at least two revisions of the failed acetabular component in 57 patients (57 hips) between August 1996 and April 2008. Of these, 15 patients with infection were excluded and one died before 4-year evaluation. The final study cohort consisted of 41 patients (41 hips) with a mean age of 55.5 years (r…

Acute severe hip pain associated with labral calcific deposition disease.

Calcific tendinitis is a term used to describe radiographic evidence of calcific deposition within a tendon. This condition, also known as c … evidence of phagocytosis. The authors report a case of calcium deposition disease found within the hip labrum with a clinical presentation of acute, atraumatic, debilitating pain in a patient with underlying femoroacetabular impingement. This clinical picture is similar to that described during the resorptive phase seen in calcific tendonitis of the …

In vivo hip joint loading during post-operative physiotherapeutic exercises.

INTRODUCTION: After hip surgery, it is the orthopedist's decision to allow full weight bearing to prevent complications or to prescribe partial weight bearing for bone ingrowth or fracture consolidation. While most loading conditions in the hip joint during activities of daily living are known, it remains unclear how demanding physiotherapeutic exercises are. Recommendations for clinical rehabilitation have been established, but these guidelines vary and have not been scientifically confirmed. The aim of this study was to provide a basis for practical recommendations by determining the hip joi…

Effect of component design in retrieved bipolar hip hemiarthroplasty systems.

Primary articulation of bipolar hemiarthroplasty systems is at the femoral head-liner interface. The purpose of this study was to compare observed damage modes on 36 retrieved bipolar systems with implant, demographic, intraoperative, and radiographic data to elucidate the effects of component design, specifically locking mechanism, on clinical performance. Retrieved bipolar hip hemiarthroplasty systems of 3 different design types were obtained, disassembled, and evaluated macro- and microscopically for varying modes of wear, including abrasion, burnishing, embedding, scratching, and pitting. …

Hip dysplasia in patients with Hurler syndrome (mucopolysaccharidosis type 1H).

BACKGROUND: Hip dysplasia is common in patients with Hurler syndrome (HS). However, its prevalence and optimal management is not yet clear because of the rarity of the disease and the prior short life span of these patients. Recent advances in the management of these children using allogeneic hematopoietic cell transplant (HCT) has significantly increased their life expectancy, with many surviving into adulthood. This review was conducted to describe the experience of a single center with hip dysplasia in HS after HCT. METHODS: We performed a retrospective review of hip dysplasia in a consecu…

Dislocation after total hip arthroplasty: risk factors and treatment options.

OBJECTIVE: The aim of this study was to analyze the setting for dislocations and redislocations after primary and revision total hip arthroplasty (THA), identify risk factors and optimize treatment. METHODS: This study included 56 patients with a dislocated hip following THA (n=5,205) between 1984 and 2005 and a matched control group (n=55). Hospital charts and radiographs of all patients in both groups were analyzed. Thirty-one patients in the study group were followed both clinically and radiologically. RESULTS: The dislocation rate after primary THA was 1.1% (56/5,205) and the redislocati…

Alendronate treatment for hip osteoarthritis: prospective randomized 2-year trial.

We examined the clinical efficacy of alendronate treatment for hip osteoarthritis using multiple outcome measures. Fifty patients with symptomatic hip osteoarthritis were enrolled in this prospective trial. The patients were randomly assigned to an alendronate group (35 mg/week alendronate and 600 mg/day calcium lactate) or a control group (600 mg/day calcium lactate) for 2 years. The groups were compared with regard to the following five parameters. The primary outcome measures are the following: (1) the Western Ontario and McMaster Universities (WOMAC) osteoarthritis pain score and the visua…

Severe metal-induced osteolysis many years after unipolar hip endoprosthesis.

BACKGROUND: Modularity of the femoral head-neck junction provides increased intraoperative flexibility to the surgeon. Complications of this … crevice corrosion at these modular interfaces. These reports have been in association with a total hip prosthesis rather than a unipolar endoprosthesis. Revision of a unipolar endoprosthesis is most commonly attributable to stem loosening or acetabular erosion from the large femoral head articulating on the host acetabular cartilage and not owing to …

Bearing surface considerations for total hip arthroplasty in young patients.

Bearing selection for total hip arthroplasty in young patients is important because of the likely long service life of the implant. Careful consideration of the next operation is recommended when choosing components. No prospective, randomized studies exist that document the clear superiority of any bearing couple in young, active patients. Modern metals, ceramics, and polyethylenes all hold promise. Further long-term data on modern bearings are needed to determine the clinical performance of these bearings. This article summarizes the available data on various bearing couples in patients aged…

[Joint-preserving surgery of the adult hip. Intertrochanteric femoral osteotomy].

Due to advances in total joint replacement, intertrochanteric osteotomy (ITO) is performed more infrequently in spite of good clinical results. Nevertheless, there are several good indications for this joint-preserving procedure in adults. Detailed biomechanical knowledge and precise clinical examination are prerequisites for correct indications and planning of ITO. The main target of this surgical procedure is improvement of joint congruency and normalization of load transfer to protect damaged cartilage. Very good results can be obtained in hip dysplasia, non-union of the femoral neck and pr…

Missed obturator hip dislocation in a 19-year-old man.

Traumatic obturator hip dislocations are rare injuries that are typically diagnosed and managed acutely. We encountered a patient who presented with a painful hip 2 months after sustaining an undiagnosed traumatic obturator hip dislocation. After failed closed treatment, the hip was reduced with open reduction, utilizing a Kocher approach and a trochanteric osteotomy. At 15 months postoperatively, the patient maintained a functional range of motion without clinical or radiographic signs of posttraumatic arthritis or avascular necrosis.

Hip arthroplasty after failed fixation of trochanteric and subtrochanteric fractures.

BACKGROUND AND PURPOSE: Hip arthroplasty is an option for elderly patients with osteoporosis for the treatment of failure after fixation of trochanteric and subtrochanteric fractures, either as a total hip arthroplasty (THA) or as a hemiarthroplasty (HA). We analyzed the reoperation rate and risk factors for reoperation in a consecutive series of patients. METHODS: All patients (n = 88) operated from 1999 to 2006 with a THA (n = 63) or an HA (n = 25) due to failure of fixation of a trochanteric fracture (n = 63) or subtrochanteric fracture (n = 25) were included. Background data were collecte…

[Oblong acetabular cup, type TC, for revision total hip arthroplasty].

PURPOSE OF THE STUDY: Revision total hip arthroplasty is a demanding surgical procedure. It involves a massive loss of acetabular bone stock associated with primary implant removal, and a complicated revision cup fixation in the highly damaged surrounding bone. The authors describe the use of a novel oblong acetabular cup, type TC (Trč-Cingr), for replacement of a loose acetabular component. The aim of the study is to present this novel implant with evaluation of the first clinical results and to report on the authors' experience with revision arthroplasty assessed at 24 to 62 months of follow…

Improvement of hip abductor muscle weakness after lumbar decompressive surgery.

INTRODUCTION: Degenerative lumbar spinal disorder is common in Japan, and the L5 nerve root is commonly involved in this disorder. The symptoms of L5 radiculopathy are irradiating lateral leg pain, and numbness and weakness of tibialis anterior and the hip abductor muscle. There has been only one report on the results of surgery for hip abductor muscle weakness caused by degenerative lumbar spinal disorder. PATIENTS AND METHODS: In this study, we analyzed the strength of the hip abductor muscle before and after decompressive surgery in 26 cases and the relationship between the lumbar disc her…

Intraarticular abnormalities in residual Perthes and Perthes-like hip deformities.

BACKGROUND: Residual Perthes and Perthes-like hip deformities are complex and may encompass proximal femoral deformity, secondary acetabular dysplasia, and associated intraarticular abnormalities. These intraarticular abnormalities have not been well characterized but may influence surgical technique and treatment outcomes. QUESTIONS/PURPOSES: We (1) determined the characteristics of intraarticular disease associated with residual Perthes-like hip deformities; and (2) correlated these intraarticular abnormalities with clinical characteristics and radiographic parameters of hip morphology. ME…

Total hip arthroplasty in above-knee amputees: a case report.

There are previous case reports in the literature that describe total knee and total hip arthroplasty (THA) in below-knee amputees, but we could find no case reports on above-knee amputees (AKAs) who have severe osteoarthritis of the hip. We present a case involving an AKA who developed severe osteoarthritis of the ipsilateral hip. Out patient underwent THA with a satisfactory postoperative outcome. Technical considerations for AKAs undergoing THA are also reviewed.
