[Rejection of corneal allografts].

Corneal graft rejection is the major cause of penetrating keratoplasty failure. It is a complex immunological process that involves recognition of alloantigens from the corneal graft by the host's immune system, leading to an efferent immune response against the graft. Each layer of the cornea can undergo rejection, endothelial rejection being the most severe form. In some cases, rejection will lead to corneal graft failure. Many donor- and host-related risk factors contribute to corneal graft rejection. Corticosteroid therapy, topical or systemic, is the gold-standard in the preventive and cu

Responses of airway epithelium to environmental injury: role in the induction phase of childhood asthma.

The pathogenesis of allergic asthma in childhood remains poorly understood. Environmental factors which appear to contribute to allergic sensitisation, with development of a Th2-biased immunological response in genetically predisposed individuals, include wheezing lower respiratory viral infections in early life and exposure to airborne environmental pollutants. These may activate pattern recognition receptors and/or cause oxidant injury to airway epithelial cells (AECs). In turn, this may promote Th2 polarisation via a "final common pathway" involving interaction between AEC, dendritic cells,

[Stem cells from fatty tissue : A new resource for regenerative medicine?].

While stem cells derived from the bone marrow are well-known in clinical medicine, fatty tissue as a source of mesenchymal stem cells is still the subject of recent research. However, adipose-derived stem cells (ASC) are not only harvested less invasively, i.e. via minimally invasive liposuction, but also yield higher numbers of multipotent stem cells.Due to cell-cell interactions and also because of the very favorable secretion profile of growth factors and cytokines ASCs displayed an extraordinary regenerative potential in recent preclinical and clinical applications and achieved a significa

High levels of CD57+CD28- T-cells, low T-cell proliferation and preferential expansion of terminally differentiated CD4+ T-cells in HIV-elite controllers.

The study of clinical and demographic characteristics related to virus control and disease progression in patients who spontaneously control HIV vireamia (HIV-controllers) is of major interest. A particular cause of HIV control has not been found and the scenario could be partially explained by special homeostatic and immunological features. In this study, CD57+CD28- phenotype, T-cell activation and levels of proliferating T-cells in elite-controllers were studied in relation to spontaneous virus control. In HIV-controllers, 9% were AIDS-diagnosed and there was a high proportion of women. In e

Both viremia and cytokine levels associate with the lack of severe disease in secondary dengue 1 infection among adult Chinese patients.

Secondary dengue infections are frequently associated with increased risk for dengue hemorrhagic fever and dengue shock syndrome. Surprisingly, we observed no dengue hemorrhagic fever cases among 353 hospitalized dengue-infected patients including 212 with primary, and 141 with secondary dengue 1 infection in China. To explore virological and immunological mechanisms which may account for this unexpected clinical observation, we assessed dengue viremia, type I interferon and inflammatory cytokine levels in these patients. While the levels of viremia and inflammatory cytokines are indistinguish

Influence of genes, sex, age and environment on the onset of autoimmune hepatitis.

The pathogenesis of autoimmune hepatitis (AIH) is complex. However, it is believed that a susceptible individual, owing to his genetic background, sex and age, can develop the disease following exposure to an environmental trigger. Autoimmune hepatitis does not follow a Mendelian pattern of inheritance; hence no single causative genetic locus has been identified. However, several genes, inside and outside the HLA locus, have been linked to an increased susceptibility to AIH. Epidemiological evidence also suggests that the sex and age of the patient plays a role in AIH pathogenesis as the disea

Oral lichen planus: a review.

Oral lichen planus (OLP) is a common chronic inflammatory disorder thought to have a low risk of associated malignancy. A genetic predisposition linked to Th1 cytokine polymorphisms may promote the T cell-mediated immunological response to an induced antigenic change that is supposed to lead to OLP lesions. Some putative etiologic factors, mainly amalgam restorations and hepatitis C virus, have been studied in detail. The diagnosis of OLP can be made from the clinical features if they are sufficiently characteristic, but biopsy is recommended to confirm the diagnosis, exclude dysplasia and mal

Hochu-ekki-to combined with interferon-gamma moderately enhances daily activity of chronic fatigue syndrome mice by increasing NK cell activity, but not neuroprotection.

The purpose of this study was to evaluate the beneficial effect of Hochu-ekki-to (TJ-41) combined with interferon-gamma (IFN gamma) on daily activity, immunological and neurological alternation in a mouse model of chronic fatigue syndrome (CFS). CFS was induced by 6 times of repeated injection of Brucella abortus antigen every 2 weeks. Both single TJ-41 and TJ-41 combined with IFN gamma increased running activity and thymus weight of CFS mice, while thicker thymic cortex together with elevation of natural killer cell activity was only found in the combined treatment group. No significant impro

How to design effective vaccines: lessons from an old success story.

Evaluation of: Gaucher D, Therrien R, Kettaf N et al. Yellow fever vaccine induces integrated multilineage and polyfunctional immune responses. J. Exp. Med. 205(13), 3119-3131 (2008). Despite the successful development of vaccines that are able to elicit potent and protective immune responses, the majority of vaccines were developed empirically and the mechanistic events leading to protective immune responses are often poorly understood. This impedes the development of new prophylactic as well as therapeutic vaccines for infectious diseases and cancer. Gaucher et al. took advantage of the effe

Mast cells and mastocytosis.

Mast cells have been recognized for well over 100 years. With time, human mast cells have been documented to originate from CD34+ cells, and have been implicated in host responses in both innate and acquired immunity. In clinical immunology, they are recognized for their central role in IgE-mediated degranulation and allergic inflammation by virtue of their expression of the high-affinity receptor for IgE and release of potent proinflammatory mediators. In hematology, the clinical disease of mastocytosis is characterized by a pathologic increase of mast cells in tissues, often associated with

CD4 memory T cells divide poorly in response to antigen because of their cytokine profile.

Immunological memory is a hallmark of adaptive immunity, and understanding T cell memory will be central to the development of effective cell-mediated vaccines. The characteristics and functions of CD4 memory cells have not been well defined. Here we demonstrate that the increased size of the secondary response is solely a consequence of the increased antigen-specific precursor frequency within the memory pool. Memory cells proliferated less than primary responding cells, even within the same host. By analyzing the entry of primary and memory cells into the cell cycle, we found that the two po

[The immunomodulatory and anti-inflammatory properties of different antimicrobial agents].

One of the major achievements in modern medicine has been the development of research into antimicrobial agents. These drugs are now widely used in the treatment of human and animal infectious diseases caused by bacterial pathogens. However, some antibacterial agents, mainly macrolides, tetracyclines, and sulfonamides, have both anti-inflammatory and immunomodalotory properties. They can down-regulate prolonged inflammation, increase mucus clearance, prevent bacterial biofilm formation, and stimulate or impair the activation of the host immune system. It is possible that these drugs are able i

[Nitric oxide--an important signalling molecule in normal and pathological pregnancy].

The numerous studies on the multifunctional nature of the nitric oxide (NO) have suggested the wide variety of functions mediated by this molecule in vital processes in cardiovascular, nervous, gastrointestinal, urogenital system and during immunological reactions. In this review we present the results of experiments carried out on animal models, as well as the results of clinical studies considering the role of NO and its metabolites during pregnancy and delivery. During pregnancy NO is one of the most important relaxing factors of myometrium, and also no less important in the control of bloo

The inflammatory disease of the subcutaneous fatty tissue are called panniculitis. The panniculitis has got a septal and a lobular histopathologic type. In the opinion of the etiologic classification occurs idiopathic (primary) or secondary entity. The secondary illnesses develop as the result of physical, chemical, biochemical, immunological, degenerative and proliferative reactions. In a part of panniculitis the etiologic factor is unknown. The authors demonstrate a rare type of the panniculitis, the Weber-Christian syndrome. This disease is an acute relapsing panniculitis with unknown etiol

Congenital atrioventricular block: histological aspects.

It is known that maternal immunological factors such as systemic disease are involved in the genesis of cardiac conduction problems in the fetus but the histologic changes in the conduction system are less documented. We report the case of a 33-year-old woman with no significant medical history. Her first pregnancy was induced by Clomifene. At 17 weeks of gestation, the fetus presented sonographic abnormalities characteristic of a complete atrioventricular block. Biological investigations found anti-SSA and -SSB antibodies. Clinical history search for systemic disease was positive for photosen

Management of Crohn's disease--a practical approach.

Crohn's disease is a chronic inflammatory disorder of the gastrointestinal tract that affects up to 480,000 persons in the United States. Symptoms include abdominal pain, diarrhea, fever, malaise, and arthralgias, and cause considerable morbidity. Speculation about genetic, environmental, dietary, infectious, and immunologic etiologies has led to treatment modalities directed at each theoretic cause, but therapy guidelines are determined by the severity of disease. Use of salicylates and/or antibiotics can be effective in mild to moderate disease, while steroids are the accepted therapy for mo

Classification of gastritis--yesterday, today and tomorrow.

The major landmark in the recent history of gastritis was the discovery of Helicobacter pylori as the cause for approximately 90% of cases of chronic gastritis. This was followed by an attempt to rationalise classification from the many conflicting nomenclatures in existence to one, the Sydney System, that might gain more universal acceptance and allow studies from around the world to be compared. In the decade since its inception, including an appropriate update, the system has partially achieved this aim. It is based on documenting the topography of the gastritis, this reflecting the range o

Sézary syndrome, T-helper 2 cytokines and accessory factor-1 (AF-1).

Mycosis fungoides and the Sézary Syndrome are characterized by clonal accumulation of well differentiated T-helper memory cells in the skin and, in the case of the Sézary Syndrome, also in the blood and lymph nodes. Well known immunological abnormalities in patients with MF and SS include reduced delayed type hypersensitivity, decreased proliferation upon stimulation with mitogens of the peripheral blood mononuclear cells, elevated IgE or IgA serum levels and eosinophilia. These abnormalities can be explained by the predominance of T-helper 2 cytokines. Clonal T-lymphocytes purified from the p

Understanding the pathogenesis of HSV-associated erythema multiforme.

Erythema multiforme (EM) is a polymorphic, often recurring eruption caused by exposure to medication or various infections, notably herpes simplex virus (HSV). Understanding the pathogenesis of HSV-associated EM (HAEM) is essential for patient management. We suggest that HAEM results from the combination of viropathic effects mediated by HSV proteins, notably DNA polymerase (Pol), and an immunological reaction to viral antigens. Presumably, viral DNA and proteins ingested by macrophages at HSV lesion sites undergo fragmentation and processing for presentation to T cells with HSV memory. HSV DN
