A singular case is described in which a pateint with a Celestin endoesophageal tube in place for 10 months died of complications from small bowel perforation resulting from disruption of the tube. The lower part of the tube lying within the stomach had deteriorated and become detached except for a single strand of nylon monofilament. This fragment passed into the small intestine, where it remained tethered at the level of the distal jejunum, acting first as an obscure cause of intermittent small bowel obstruction and later as the cause of jejunal perforation. VIP is a 28 amino acid peptide found in highest concentration in central and peripheral nervous tissue. This study measured VIP in pure populations of peripheral blood cells to determine the presence or absence of VIP in noninnervated tissues. Cell populations were purified by Ficoll-hypaque gradient centrifugation followed by dextran sedimentation or differential adherence to culture plates. Platelets were purified by differential centrifugation. VIP was isolated by acid-ethanol extraction and quantified by radioimmunoassay. A mean value of 1.1 +/- 0.6 ng of VIP per 10(8) cells was extracted

Large scale studies were performed to determine the prevalence of various intestinal helminth infections in urban and rural areas of Khuzestan Province, southwest Iran. A total of 16,361 stool samples from people in 105 villages and 14 small towns were examined by the formalin-ether concentration method. Megacystis-microcolon-intestinal hypoperistalsis syndrome was observed in a male infant. Review of the literature failed to turn up any other cases involving males. The sonogram revealed megacystis, hydronephrosis, and a normal amount of amniotic fluid. The hypoperistalsis failed to respond to several pharmacological agents, including metoclopramide and caerulein.

A 33-year-old man had a sudden onset of night blindness 3 1/2 years after intestinal bypass surgery for morbid obesity. Ocular examination disclosed a probable Bitot's spot and electrophysiologic testing showed markedly impaired dark adaptation and subnormal electroretinography. After therapy with orally administered vitamin A, the night blindness disappeared and the dark adaptation curve and electroretinogram returned to normal.

A revision of the children which during the last five years have undergone abdominal surgery is presented. 2% showed postoperative intestinal obstruction due to adhesions. Intestinal plication has been performed in children with a great amount of adhesions. A comparison is made of the Childs-Phillips and the Noble plication operation. Results in both cases showed to be good. The greater facility and the shorter duration of the surgery in the Childs-Phillips plication operation make this last technic more advisable.

Acute small-intestinal obstruction is not an uncommon complication following excisional operation for inflammatory disease of the bowel. In the Monash series the most common cause was adhesion formation. Stoma problems accounted for a small number. There was a special tendency for the complication to appear soon after the excisional surgery. A significant mortality rate accompanied obstructive complication and, over the long term, one in five patients needed further surgery for a recurrence.

Fifty hospital patients in Port Moresby (mean age 30 yr) without evidence of malnutrition or gastro-intestinal disease, who came from many parts of Papua New Guinea (P.N.G.), were given 50 g lactose orally after an overnight fast. In 49 blood glucose rise was less than 1.1 mmol 1(-1), indicating hypolactasia. Thirty-three gave a clear history of symptoms, usually diarrhoea, after lactose. Adult hypolactasia approaches 100% in P.N.G.
