Facet joint orientation, facet and costovertebral joint osteoarthrosis, disc degeneration, vertebral body osteophytosis, and Schmorl's nodes in the thoracolumbar junctional region of cadaveric spines.

The disc degeneration in the thoracolumbar junctional region (T10-L1) of 37 male cadaveric spines was recorded with the use of discography. From 24 of these spines the facet joint orientation and degenerative findings of the facet and costovertebral joints, vertebral bodies (osteophytosis) and discs, and Schmorl's nodes were recorded directly from bones. At T11-12, the most common site for the transitional zone between thoracic and lumbar facet type, there was a marked variation in the orientation of facets. The occurrence of degenerative findings and Schmorl's nodes at the three levels in the…

Preoperative Staphylococcus aureus nasal screening and decolonization for the prevention of periprosthetic joint infection in elective total joint arthroplasty: A single-center retrospective comparative study.

INTRODUCTION: Staphylococcus aureus is the most frequently implicated pathogen in periprosthetic joint infections (PJIs) and surgical site infections (SSIs), with nasal colonization being a well-established risk factor for their onset. Despite this association, the role of routine preoperative screening and decolonization in reducing postoperative infection rates remains debated. METHODS: This retrospective study analyzed 2,507 elective joint arthroplasty procedures performed between January 2022 and December 2024 at a single center in Italy. Starting in 2024, patients underwent nasal swab sc

Comparison of Maximum Isometric Strength of the Hip Joint Abductor and Knee Joint Extensor Muscles between Knee Osteoarthritis Patients with and without Self-reported Instability.

Patients with knee osteoarthritis (KOA) often have impaired muscle function of the weight-bearing muscles, particularly in the knee and hip joints. This can lead to a significant loss of strength and power and may play a role in the perceived instability of the knee joint. The purpose of this study was to compare the maximum isometric strength of the hip abductor and knee extensor muscles between patients with KOA with and without perceived instability.Nineteen patients with KOA participated in this cross-sectional study and were divided into two groups. The first group (n = 10; women = 4, men…

A Bayesian Joint Model of Multiple Nonlinear Longitudinal and Competing Risks Outcomes for Dynamic Prediction in Multiple Myeloma: Joint Estimation and Corrected Two-Stage Approaches.

Predicting cancer-associated clinical events is challenging in oncology. In Multiple Myeloma (MM), a cancer of plasma cells, disease progression is determined by changes in biomarkers, such as serum concentration of the paraprotein secreted by plasma cells (M-protein). Therefore, the time-dependent behavior of M-protein and the transition across lines of therapy (LoT), which may be a consequence of disease progression, should be accounted for in statistical models to predict relevant clinical outcomes. Furthermore, it is important to understand the contribution of the patterns of longitudinal

Morphological factors influencing the progression of temporomandibular joint anterior disc displacement with reduction to without reduction after contralateral joint arthroscopic discopexy: a retrospective study.

The aim of this retrospective cohort study was to identify morphological risk factors associated with the progression of temporomandibular joint (TMJ) anterior disc displacement with reduction (ADDwR) to without reduction (ADDwoR) in the contralateral joint following arthroscopic discopexy. Among 80 patients, 26 divided into a 'progression' group versus 54 without progression based on the disc position in the contralateral joint at postoperative follow-up. Patient characteristics and the disc length, disc folding angle, anterior distance of the disc relative to the condyle, and angle between t

Biomechanics of the Human Knee Joint in Maximum Voluntary Isometric Flexion: Study of Changes in Applied Moment, Agonist-Antagonist Participations, Joint Center, and Flexion Angle.

Estimation of the knee joint strength by maximum voluntary isometric contraction (MVIC) is a common practice to assess strength, coordination, safety to return to work or engage in sports after an injury, and to evaluate the efficacy of treatment modalities and rehabilitation strategies. In this study, we utilize a previously validated coupled finite element-musculoskeletal model of the lower extremity to explore the sensitivity of output measures (posterior cruciate ligament [PCL]/muscle/contact forces and passive moments) in knee MVIC flexion exercises at seated position. To do so, at three

Postoperative Calcaneocuboid Joint Subluxation and Midtarsal Joint Changes in Pediatric Idiopathic Flexible Flatfoot Treated With Calcaneal Lengthening Osteotomy: A Midterm Follow-up Study.

BACKGROUND: Calcaneal lengthening osteotomy (CLO) is one of the main surgical options for treatment of pediatric idiopathic flexible flatfoot (FFF). Reportedly, calcaneocuboid (CC) joint subluxation occurs after CLO; however, its effect on the midfoot remains unclear. This study aimed to investigate the radiologic midterm results after CLO treatment in pediatric idiopathic FFF. METHODS: We evaluated 23 pediatric patients with idiopathic FFF aged ≥8 years, who underwent CLO from 1999 to 2017 owing to moderate to severe flatfoot deformity (assessed by visual inspection). Patients aged between 8

The Change of Serum Interleukin-6 Fails to Identify Subsequent Periprosthetic Joint Infection in Patients Who Have Two-Stage Revision for Periprosthetic Joint Infection.

BACKGROUND: The diagnosis of persistent infection prior to second-stage reimplantation in 2-stage exchanges for periprosthetic joint infection (PJI) can be challenging as there is no optimal diagnostic tool. This study investigates the usefulness of pre-reimplantation serum C-reactive protein (CRP) and interleukin-6 (IL-6) and its change between both stages to identify patients who have subsequent PJI. METHODS: There were 125 patients who underwent planned 2-stage exchange for chronic knee or hip PJI from a single center retrospectively identified. Patients were included if preoperative CRP a

Relationship between facet joint opening on CT and facet joint effusion on MRI in patients with lumbar spinal stenosis: analysis of a less invasive decompression procedure.

OBJECTIVE: Both facet joint opening (FJO) on CT and facet joint effusion (FJE) on MRI are reportedly indicators of segmental instability in the lumbar facet joints of patients with lumbar spinal stenosis (LSS). However, no study has investigated both parameters simultaneously. Therefore, the association between these findings and which parameter is better for predicting clinical outcomes after surgical treatment remains unclear. The purpose of this study was to investigate the relationship between FJO and FJE in patients who underwent less invasive decompression procedures for LSS and to inves…

Pitfalls during Sacroiliac Joint Arthrodesis for Patients with Severe Sacroiliac Joint Pain: Report of Three Cases with Sacral Dysmorphism Induced by Lumbosacral Transitional Vertebrae.

INTRODUCTION: Lumbosacral transitional vertebrae (LSTV) can cause sacral dysmorphism. Sacroiliac joint (SIJ) arthrodesis has been widely performed as the last resort for severe SIJ pain. We report three kinds of technical pitfalls identified in the surgical records of SIJ arthrodesis for the patients with sacral dysmorphism induced by LSTV. CASE REPORT: For the cases of a 34-year-old man with LSTV Castellvi's Type IV, anterior SIJ arthrodesis using a plate and screws was performed. The most cranial sacral foramen was larger and irregularly round, and the nerve root ran to the lateral side fro

Psychopathological manifestations of joint hypermobility and joint hypermobility syndrome/ Ehlers-Danlos syndrome, hypermobility type: The link between connective tissue and psychological distress revised.

Psychological distress is a known feature of generalized joint hypermobility (gJHM), as well as of its most common syndromic presentation, namely Ehlers-Danlos syndrome, hypermobility type (a.k.a. joint hypermobility syndrome - JHS/EDS-HT), and significantly contributes to the quality of life of affected individuals. Most published articles dealt with the link between gJHM (or JHS/EDS-HT) and anxiety-related conditions, and a novel generation of studies is emerging aimed at investigating the psychopathologic background of such an association. In this paper, literature review was carried out wi
