BACKGROUND: Obesity is a rising public health issue worldwide. Guidelines regarding maternal body mass index (BMI) and gestational weight gain (GWG) are missing in Cameroon where maternal mortality rate remains very high. We hypothesized that obesity and inappropriate GWG are associated with poor pregnancy outcomes. We aimed at assessing associations of BMI and GWG with pregnancy outcomes.
METHODS: This was a retrospective cohort study at the Yaoundé Central Hospital. We included women with term singleton deliveries in the post-partum ward. The World Health Organisation classification of BMI …

BACKGROUND: Obstetric hemorrhage (OH), which includes hemorrhage from multiple etiologies during pregnancy, childbirth, or postpartum, is the leading cause of maternal mortality and accounts for one-quarter of global maternal deaths. The Non-pneumatic Anti-Shock Garment (NASG) is a first-aid device for obstetric hemorrhage that can be applied for post-partum/post miscarriage and for ectopic pregnancies to buy time for a woman to reach a health care facility for definitive treatment. Despite successful field trials, and endorsement by safe motherhood organizations and the World Health Organizat…

Leon C. Chesley's first paper opened with the title 'Pregnancy in the patient with hypertensive disease'(1). Leon C. Chesley PhD, John E. Annitto, MD, MSc (Med), Jersey City, NJ, USA (from the Margaret Hague Maternity Hospital) [Am J Obstet Gynecol 1947;53:372-381]. We quote some lines from this important paper and pioneering work: "We have thought it worth while to survey our experience with pregnancy in women with hypertensive disease. There are relatively few such studies based upon any considerable series, and in most studies extant there has been a selective factor in that therapeutic abo

A retrospective analysis of neonatal morbidity and mortality was conducted over a ten-year period (1981-1990) at a tertiary hospital in Ilesa, Nigeria, to determine the trends in neonatal morbidity and mortality in relation to places of delivery. 7,225 babies were admitted into the neonatal unit during the period wherein 3,232 (44.7%) were inborns and 3,993 (55.3%) outborns. Places of delivery of outborn babies were government hospitals/maternity centres (44.1%), home (28.5%), private hospitals/clinics (18.8%), and mission houses (8.7%). Major indications for admission among inborns were neona

The prevalence of maternal obesity is steadily increasing, affecting a significant proportion of pregnancies. Beyond well-known obstetric complications, this condition exposes the newborn to immediate risks (perinatal mortality, congenital malformations, macrosomia) and the child to long-term consequences (obesity, metabolic disorders, and neurodevelopmental issues). Since lifestyle interventions during pregnancy show limited efficacy on child outcomes, prevention strategies must focus on the preconception period and the promotion of breastfeeding. This is an underestimated public health issue

Preeclampsia constitutes one of the leading causes of maternal morbidity and mortality in the United States. Preeclampsia-related neurological disorders are well-established and associated with a broad spectrum of manifestations, including headaches, visual symptoms like blurred vision, photopsia, field defects, and other major clinical events. However, cranial nerve disorders are rare in preeclampsia, which is inadequately researched. Here, we present a 26-year-old primigravida woman with an isolated abducens nerve palsy as the first sign of preeclampsia at 35 weeks of gestation.

Hypertensive disorders of pregnancy are among the leading causes of maternal morbidity and mortality in the US. Preeclampsia (PreE) which includes hypertension and proteinuria during pregnancy, is thought to result from placental ischemia. Risk factors for PreE parallel those for cardiovascular disease, and recent studies point to hyperlipidemia specifically, hypertriglyceridemia, as a risk factor for PreE. Current practice does not routinely include lipid testing pre-conception or during pregnancy. Professional, societal recommendations should advocate for hyperlipidemia screening, followed b

IN BRIEF: Preeclampsia is a common serious disorder that can occur during pregnancy. This study uses integrative analysis of preeclampsia transcriptomes and single-cell transcriptomes to predict cell type-specific contributions to preeclampsia.
ABSTRACT: Preeclampsia is a devastating pregnancy disorder and a major cause of maternal and perinatal mortality. By combining previous transcriptomic results on preeclampsia with single-cell sequencing data, we here predict distinct and partly unanticipated contributions of decidual stromal cells and uterine natural killer cells in early- and late-ons

Both pheochromocytoma and amniotic fluid embolism (AFE) are important causes of maternal mortality. We present a case of a 29-year-old woman who developed cardiac arrest after Caesarean section, complicated by both pheochromocytoma crisis and AFE. After resuscitation, the patient developed multiple organ dysfunction, rhabdomyolysis and disseminated intravascular coagulation (DIC). After institution of multidisciplinary interventions (including the use of an intra-aortic balloon pump, extracorporeal membrane oxygenation, continuous hemodiafiltration, and neuroprotective therapeutic hypothermia)…

We report an intrauterine fetal death that occurred less than 24 h after transabdominal amniocentesis. Chorioamnionitis was confirmed by amniotic fluid culture which showed multiple enteric organisms and postmortem examination of the fetus that showed Clostridium perfringens. The patient was treated with intravenous broad-spectrum antibiotics and uterine evacuation and her condition rapidly improved. Intra-amniotic infection after amniocentesis requires a high index of suspicion and prompt aggressive treatment with broad spectrum antibiotics and uterine evacuation to prevent maternal sepsis-re

Pneumonia contributes to between 750,000 and 1.2 million neonatal deaths and an unknown number of stillbirths each year world wide. The aetiology depends on time of onset. Gram negative bacilli predominate in the first week of life, and Gram positive bacteria after that. Streptococcus pneumoniae probably causes about 25% of neonatal pneumonia. Interventions that would reduce mortality from this condition would have a large range of beneficial effects: improved maternal health, better management of other common neonatal conditions, and reduced long term childhood and adult morbidity.

BACKGROUND: Maternal and fetal morbidity and mortality associated with varicella pneumonia in the peripartum period are increased further with multiple complications.
CASE: The patient had varicella pneumonitis acquired at 32 weeks' gestation and complicated by bacterial superinfection, adult respiratory distress syndrome, endotoxin shock, and bronchiolitis obliterans organizing pneumonia.
CONCLUSION: Aggressive, timely management of respiratory failure, control of infection, correction of endotoxin shock, treatment of biopsy-proven bronchiolitis obliterans organizing pneumonia, and obstetri

A retrospective review of pregnancy outcome in hemoglobin (Hbsc) patients managed at the University College Hospital, Ibadan over a 5-year period (1984-1988) was carried out. The main antenatal complications included anemia (51.2%), bacterial infection (22.0%), bone pain crisis (7.3%) and preeclampsia (2.4%). Intrapartum complications included anemia (29.2%), bone pain crisis (12.2%) and pseudotoxemia (4.9%). The maternal and perinatal mortality rate were 48 and 195 per 1000, respectively. The duration of labor and operative delivery rate were not different from the general population.

BACKGROUND: The rate of incidental pregnancy in trauma patients and the incidence of associated fetal mortality, to our knowledge, have not been previously reported. The early diagnosis of pregnancy in trauma patients has become even more difficult because rapid pregnancy screens have been eliminated because of quality control issues. We determined the rate of incidental pregnancy and the sequelae of delayed diagnosis, including fetal radiation exposure and mortality.
STUDY DESIGN: Data were analyzed retrospectively on all patients in whom pregnancy was diagnosed during a trauma admission dur

BACKGROUND: Birth asphyxia remains a leading cause of neonatal mortality in low- and middle-income countries, particularly in rural settings with limited access to skilled intrapartum and neonatal care. Evidence on survival patterns and predictors of birth asphyxia-related neonatal mortality in rural Ghana is limited. This study assessed neonatal mortality among neonates admitted with birth asphyxia and predictors among neonates admitted with birth asphyxia in rural Ghana.
METHODS: An institution-based multicentre retrospective cohort study was conducted using routinely collected clinical rec

BACKGROUND: Neonatal deaths now account for 47% of all deaths in children younger than 5 years globally. More than a third of newborn deaths are due to preterm birth complications, which is the leading cause of death. Understanding the causes and factors contributing to neonatal deaths is needed to identify interventions that will reduce mortality. We aimed to establish the major causes of preterm mortality in preterm infants in the first 28 days of life in Ethiopia.
METHODS: We did a prospective, cross-sectional, observational study in five hospitals in Ethiopia. Study participants were pret
