Varicella-zoster vaccine in the USA: success for control of disease severity, but what next?

In the period from 1990 to 1994, before the introduction of a varicella vaccine to the USA, approximately 100 deaths in otherwise healthy individuals, children and adolescents under 20 years of age, were attributable to varicella complications. The administration of a single-dose vaccine has now been widespread in the USA for nearly 10 years; however, since the effectiveness of a single dose in children under 13 years of age in an outbreak situation is approximately 80%, consideration of a second booster dose is in progress although not yet recommended. Licensure of a measles-mumps-rubella-var…

Transients and attractors in epidemics.

Historical records of childhood disease incidence reveal complex dynamics. For measles, a simple model has indicated that epidemic patterns represent attractors of a nonlinear dynamic system and that transitions between different attractors are driven by slow changes in birth rates and vaccination levels. The same analysis can explain the main features of chickenpox dynamics, but fails for rubella and whooping cough. We show that an additional (perturbative) analysis of the model, together with knowledge of the population size in question, can account for all the observed incidence patterns by…

[Medical travel counseling and vaccinations].

The art of travel advice is based on clear and regularly updated information. It aims at covering the specific needs of individual travellers and benefits from the experience of the professional advisor. Frequently occurring and life-threatening health problems are deal with in the first place. Recommendations on prophylactic measures are a challenge for the advisor as they require the active collaboration of the traveller. Vaccinations on the other hand require less compliance of the traveller. Routine vaccinations against hepatitis A and B, diphtheria and tetanus, and against measles are dis…

Qualification of long-stored samples of serum banks for seroepidemiological studies.

Serum samples were tested for antibodies against polio virus and measles virus from two serum banks after having been stored under different conditions for several years. The results are compared with those ones obtained immediately after sampling. Despite of small increases of antibody-negative samples and of small decreases of the titres the suitability of the samples stored could be shown. Special attention should be given to the establishment, e.g. in the framework of Expanded Programme on Immunization of the World Health Organization and in order to study new pathogens unknown to date, an…

Immunologic abnormalities accompanying acute and chronic viral infections.

A number of viral infections induce profound alterations in immune function; these changes may or may not be related to direct infection of lymphoid cells. The immunologic abnormalities that occur during acute systemic viral infections such as measles or infectious mononucleosis include suppression of responses to tuberculin skin tests in vivo and suppression of mitogen responses in vitro, evidence of activation of the immune system with up-regulation of activation-associated cell-surface markers and soluble cellular products, and evidence of altered immune regulation with autoimmune disease a…

Occupational hazards in hospitals: risk of infection.

In this review of the risk of infection to hospital staff, attention is drawn to the continuing risk presented by hepatitis B and pulmonary tuberculosis, which are more common than diseases such as typhoid fever, brucellosis, histoplasmosis, whooping cough, infectious gastroenteritis, measles, and parotiditis. Other items considered include the susceptibility of female hospital staff to rubella and the importance of their undergoing screening and vaccination; the risks currently presented by epidemic keratoconjunctivitis and by herpes viruses (herpes simplex, varicella zoster, and cytomegalovi…

A quantitation of myelin-associated glycoprotein and myelin basic protein loss in different demyelinating diseases.

The loss of myelin-associated glycoprotein (MAG) and myelin basic protein (MBP) was compared by quantitative immunocytochemistry in demyelinating lesions of measles encephalomyelitis (ME), multiple sclerosis (MS), and progressive multifocal leukoencephalopathy (PML). Serial sections from paraffin-embedded tissue were reacted with antisera for MAG and MBP, and areas of staining loss were compared morphometrically. Lesions in ME showed MAG loss equal to that of MBP, lesions of PML showed MAG loss greater than that of MBP, and MS lesions showed a mixture of patterns. These data demonstrate distin…

Immunization against viral infections. Successful, ongoing mode of defense.

The body protects itself from disease through a variety of innate defense mechanisms and through immunity acquired both passively and actively. Passive immunization is achieved through transfer of antibody or of sensitized lymphocytes or their soluble products from an immune donor, while active immunization is achieved through stimulation of the recipient's own protective mechanism by means of exposure to the disease or to pathogens or their toxoids. Among the diseases brought under control or eradicated by means of active immunization are smallpox, polio, and measles. Vaccines for immunizatio…

Automated cytopathic effect (CPE) assays.

An automated CPE procedure has been developed that increases the precision and ease of performing titrations of measles, mumps and rubella viruses in vaccine materials. By this procedure, additions of cell suspensions and reagents and the dilution of samples are performed automatically by a modified Dynatiter instrument, using 96-well microtitre plates. Cell monolayers are stained with carbolfuchsin dye to eliminate the need for microscopic examination. Finally, the trays are read in an optical scanner and the end points calculated automatically by a programmable calculator. The increased accu…

[Evolution of viral diseases (author's transl)].

Biological characteristics of viruses determine their distribution in men and animals as well as their means of surviving or persisting. Taking measles as an example, we show that we are dealing with a recent phenomenon in evolution--as opposed, for instance, to those infections due to arboviruses (eg yellow fever) common in tropical regions. Survival rates are lower for highly virulent viruses (myxomatosis). Antigenmodification (influenza) or latency (herpes virus infections) allow the virus repeatedly to escape the body's immune system. The distribution and clinical appearance of the infecti…

Increased CSF IgM in multiple sclerosis.

CSF IgM levels have been measured by radioimmunoassay in 56 patients with MS, 62 patients with other neurologic diseases, and 31 normal controls. Forty-eight percent of the patients with MS had a raised CSF IgM level compared with 5 percent of the patients with other diseases. The IgM level did not correlate with the IgG level. Forty percent of the MS patients with normal IgG levels had high IgM levels. No relationship was found between the CSF IgM level and length or severity of the MS, relapses, or steroid therapy. Attempts to identify the IgM as being anti-measles were unsuccessful.

Rapid scale-up of long-lasting insecticide-treated bed nets through integration into the national immunization program during child health week in Togo, 2004.

In December 2004, Togo was the first country to conduct a nationwide free insecticide-treated net (ITN) distribution as part of its National Integrated Child Health Campaign. Community-based cross-sectional surveys were conducted one and nine months post-campaign as part of a multidisciplinary evaluation of the nationwide distribution of ITNs to children 9-59 months of age to evaluate ITN ownership, equity, and use. Our results demonstrated that at one month post-campaign, 93.1% of all eligible children received an ITN. Household ITN ownership and equity increased significantly post-campaign.

Application of short monolithic columns for improved detection of viruses.

Monolithic chromatography media represent a novel generation of stationary phases introduced in the last 10-15 years providing a chromatography matrix with enhanced mass transfer and hydrodynamic properties. These features allow for an efficient and fast separation of especially large biomolecules like e.g., DNA and viruses. In this study, the enrichment of virus RNA on short monolithic columns prior to molecular detection of viruses is described. Measles and mumps viruses were chosen as model viruses. The results show that it is possible to bind viral RNA on monoliths and concentrate viral nu…
