[An unusual case of gunshot wounds caused with an air gun].

A 28-year-old woman was shot in the face with an air gun while driving a car. The patient was examined in the Department of Forensic Medicine in Katowice. An inconspicuous scar was found near the medial angle of the right eye. Further ophthalmological and radiological examinations revealed the presence of small foreign bodies in the vitreous body of the right eye, the sphenoid sinus and the ethmoid cells. The authors issued an expert opinion, in which they stated that the sustained injuries had not caused any significant organ dysfunction.

Presumed metastasis of breast cancer to the abducens nucleus presenting as gaze palsy.

A 51-year-old woman with breast cancer presented with progressive diplopia. Neuro-ophthalmologic examination revealed right gaze palsy and peripheral facial nerve palsy. Brain magnetic resonance imaging (MRI) was normal. However, two months later a repeat brain MRI revealed an enhancing round nodular mass at the right facial colliculus of the lower pons, at the location of the abducens nucleus. Localized metastasis to the abducens nucleus can cause gaze palsy in a patient with breast cancer.

[Inflammation of the conjunctiva].

Inflammations of the conjunctiva represent some of the most frequent eye diseases in general and ophthalmologic practice. Etiologically multiple diseases can be identified. A group of infectious conjunctivites contrasts with a very hereogeneous non-infectious group. Neoplastic processes such as low grade epithelial dysplasias and lymphomas may mascarade conjunctivitis. The differentiation of the various clinical pictures may be challenging for the practitioner. This article reviews the methodology of patient history and clinical work up as well as the symptomatology and treatment options for t

[Miller Fisher syndrome, internal and external ophthalmoplegia after flu vaccination].

CASE REPORT: Miller Fisher Syndrome (MFS) is the most frequent variant of the Guillain-Barré Syndrome. It is characterised by the classic triad of ophthalmoplegia, ataxia and areflexia. We present a case of a patient who developed these clinical findings 5 days after flu vaccination. DISCUSSION: Miller Fisher Syndrome is an unusual condition seen in ophthalmologic clinical practice. Although respiratory and digestive infections have been reported as antecedent infectious agents in MFS, it has not previously been described in relation to the flu vaccine.

Oxidative stress in diseases of the human cornea.

Intense exposure to light, robust metabolic activity, and high oxygen tension render the human eye particularly vulnerable to oxidative damage and the list of ophthalmological disorders implicating reactive oxygen and nitrogen species is rapidly expanding. Here, we review the roles of oxidative stress in the etiopathogeneses and pathophysiology of diseases of the human cornea including pterygium, keratoconus, trauma and chemical injury, and a host of inflammatory, metabolic, degenerative, and iatrogenic conditions. Data from animal and tissue culture experimentation germane to these conditions

Orbitozygomatic fracture management.

Orbitozygomatic fractures are frequently encountered in plastic surgery. Management depends on a thorough preoperative physical examination, with attention to the ophthalmologic assessment. Coronal and axial computed tomography is essential for identifying fracture extent and orbital involvement. Adequate exposure and mobilization of fracture segments is essential for successful anatomical reduction. Failure to perform effective fixation may lead to subsequent complications, such as enophthalmos and diplopia. The authors illustrate the appropriate management of orbitozygomatic fractures in an

[Red eye and visual impairment two months after esophageal resection with complicated postoperative course].

A patient with a red, painful eye and gradual visual impairment is examined ophthalmologically. Culture of the vitreal aspirate yields Candida tropicalis. Subsequent medical evaluation reveals that during an acute hospitalization eight weeks prior positive blood cultures for Candida had been reported but not acted on, despite a high risk constellation of operative therapy, intensive care unit treatment, central catheters, parenteral nutrition and broad spectrum antibiotics. Diagnosis and treatment of candidemia with a focus on candida endophthalmitis is discussed.

Bilateral lens subluxation in a case of lipoid proteinosis.

We report a patient who presented with pain, redness, and diminution of vision in the right eye and low vision in the left eye. Ophthalmologic examination showed characteristic eyelid lesions, anterior dislocation of the crystalline lens in the right eye, and nasal subluxation in the left eye. The patient's voice was hoarse, and he had skin lesions. After histopathologic examination of the skin biopsy, lipoid proteinosis (LP) was diagnosed. Bilateral pars plana lensectomy was performed. The ophthalmic manifestations of LP are discussed.

Transient sixth nerve palsies and torticollis secondary to parietal arteriovenous malformation.

Arteriovenous malformations (AVMs) can compress on any intracranial structures, and the presence and nature of ophthalmologic symptoms and signs depend on the size and location of the AVMs. We report an 11-month-old girl with parietal fistulous AVM, transient bilateral sixth nerve palsies and torticollis. The postoperative period was uneventful and she was discharged in a good condition. The patient recovered from transient bilateral abducens nerve palsies and torticollis; she was asymptomatic during the 18-month follow-up period.

Choroidal and optic nerve infarction in hepatitis C-associated polyarteritis nodosa.

A 39-year-old man presented with headache, weight loss, bilateral subdural hematomas, pansinusitis, and visual loss. The neuro-ophthalmologic examination disclosed deep choroidal lesions and bilateral optic disc edema. Orchiectomy for testicular torsion showed acute vasculitis consistent with polyarteritis nodosa (PAN). Polymerase chain reaction (PCR) testing revealed hepatitis C. This is the first reported case of PAN due to hepatitis C with early findings of choroidal and optic nerve infarction.

[Acute pancreatitis presenting as sudden blindness].

CASE REPORT: A 36-year-old man with a history of alcohol abuse presented with sudden blindness. The ophthalmologic examination showed Purtscher-like retinopathy. The presumed diagnosis was acute pancreatitis, which was confirmed by complementary laboratory studies. DISCUSSION: Sudden acute visual loss with Purstcher-like retinopathy may be present in acute pancreatitis, although it is a very rare as a presenting symptom. Early diagnosis based on ophthalmic symptoms may help in the recognition and treatment of the disease and prevent later complications.

Occipital hypoperfusion in a patient with psychogenic visual disturbance.

A patient with bilateral diminished visual acuity and blurred vision was given the diagnosis of psychogenic visual disturbance after extensive psychiatric and ophthalmological examinations. Single photon emission computed tomography with Tc-99m-ethylcysteinate dimer demonstrated remarkably reduced regional cerebral blood flow (rCBF) in the bilateral occipital lobes. More specifically, rCBF was reduced in the visual association areas, but not in the primary visual areas. These findings suggest that functional suppression of the visual association area is associated with the development of psych

[Severe thyroid ophthalmopathy related to interferon alpha therapy].

The authors report a case of a 49-year-old patient presenting with dysthyroid ophthalmopathy, who was treated with interferon alpha for chronic hepatitis C. Proptosis appeared 3 months after treatment onset and many complications occurred over more than 1 year. The extreme severity seen in this case is infrequent. After a summary of other ocular damage linked to interferon, the authors suggest that every interferon treatment include a check-up before beginning therapy and regular biological and ophthalmological monitoring.

[Clinical and epidemiological aspects in ocular tuberculosis].

150 new cases of ocular tuberculosis, representing 1% of total tuberculose cases, was diagnosticated during the last 10 years, in both Ophthalmological and Pneumophtisiological Clinics of Craiova. Ocular tuberculosis is the local manifestation of a general latent tuberculosis; the uveal diseases represent 46% of cases of ocular tuberculosis. 51 of 150 cases of ocular tuberculosis was corioretinal forms, which evaluated uni or bilateral, with one and multiple focares; in only one case we constated a coroidian tuberculom form.

Bilateral optic neuritis in a child diagnosed with Gd-enhanced MR imaging using fat-suppression technique.

A 4-year-old boy developed bilateral optic neuritis. Although precise neuro-ophthalmological evaluation was difficult, the diagnosis was made with gadolinium-enhanced MR imaging using fat-suppression technique in the initial stage of the disease. Enhancement and enlargement of the intraorbital and intracanalicular optic nerve were demonstrated bilaterally as well as protrusion of the optic nerve head. The disease responded dramatically to intravenous steroid therapy. The etiologies in children usually differ from those in adolescent and adult patients.

[Traumatic bilateral paralysis of the abducens nerve without cervical fracture].

We report the case of a 75-year-old woman with an uneventful medical history who presented with sudden-onset diplopia immediately after neck trauma in a traffic accident. Clinical and ophthalmological examinations were consistent with bilateral abducens nerve palsy. The causal relationship, including the medicolegal implications and the pathophysiological mechanism are discussed. The case demonstrates that exceptionally benign trauma can cause severe diplopia. The delay to recovery may be very long, suggesting caution concerning the decision of surgical care.

[Changes in the visual system of patients with carotid artery occlusion].

The authors presented results of visual system examination in 28 persons in whom disturbances of blood flow in carotid arteries were diagnosed with colour doppler ultrasonography. The patients underwent routine ophthalmological examination, including kinetic and static perimetry. Ocular signs only, with no neurological changes, were found in 46% of patients. In 21% of cases there were episodes of transient blindness; homonymous hemianopsia occurred in 10%, acute ischaemic optic neuropathy in 11%, and secondary neovascular glaucoma in 14% of patients.

Melanoma-associated retinopathy.

PURPOSE/METHODS: A 64-year-old man with a history of maxillary antrum melanoma had abrupt-onset night blindness and photopsia. RESULTS/CONCLUSIONS: Ophthalmologic examination and electroretinogram suggested melanoma-associated retinopathy. Serum produced a weak but specific labeling of human bipolar cells by indirect immunofluorescence. Complete excision of melanoma may have contributed to low levels of circulating autoantibodies. The identity of the retinal bipolar antigen recognized by melanoma-associated retinopathy autoantibodies is needed for more accurate diagnosis.

Microdeletion of chromosome 7P syndrome ocular manifestations.

Several syndromes have been associated with microdeletions of the autosomes. These syndromes are diverse in their morphology and frequently manifest abnormalities of the ocular adnexa. A child with an uncommon microdeletion of the short arm of chromosome 7P presented initially with congenital myogenic ptosis. After multiple systemic abnormalities were found during a routine examination, the child was referred for genetic evaluation where the defects were incidentally found. The child responded well with a fascia lata frontalis sling. The genetic disorder is discussed with an emphasis on the op

[The retinal nerve fiber layer in retrobulbar neuritis].

In the course of 1 year we investigated 32 patients suffering from retrobulbar neuritis, using photographs of the retinal nerve fiber layer (RNFL). All photographs revealed defects of the RNFL in the area of the papillomacular bundle. These observations were compared with photographs of eyes not affected by any ophthalmological diseases. No loss of retinal nerve fiber bundles was found in any of these cases. One remarkable finding of this study was that 90% of the patients with retrobulbar neuritis also had defects in the RNFL of the contralateral eye.
