Post-Covid Cerebral Mucormycosis, Varied Presentations and Surgical Strategies of a Newer Epidemic: An Institutional Experience.

CONTEXT: During the challenging second wave of the COVID-19 pandemic, we encountered a most dreadful fungal infection in the form of COVID-associated mucormycosis (CAM), with a varied pattern of presentation from previous experience. Patients presented with simple fungal sinusitis or more complicated brain abscesses, and newer manifestations such as skull osteomyelitis. We report our findings and innovative treatment strategies used to manage this morbid condition. OBJECTIVES: To study the various presentations and surgical strategies in treating post-COVID rhino-orbito-cerebral mucormycosis …

Clinical and Radiologic Manifestations of Bone Infection in Children with Cat Scratch Disease.

We identified 13 patients with cat scratch (Bartonella henselae) bone infection among those admitted to a large tertiary care children's hospital over a 12-year period. The median age was 7 years and the median time from onset of illness to diagnosis was 10 days. Multifocal osteomyelitis involving spine and pelvis was common; no patient had a lytic bone lesion. Median treatment duration was 28 days (IQR, 24.5 days). Despite significant variations in treatment duration and antimicrobial therapy choices, all patients showed improvement.

Linezolid induced black hairy tongue: a rare side effect.

Linezolid induced black hairy tongue is a rare benign reversible side effect of linezolid therapy. We report a case of a 61 year old diabetic lady who developed thrombocytopenia and black hairy discoloration of the tongue after being prescribed linezolid for foot osteomyelitis by the orthopaedic surgeon. Patient was encouraged to practice good oral dental hygiene, advised to use a soft tooth brush, regular mouth wash and baking soda containing tooth paste. The condition resolved four weeks after cessation of the antibiotic therapy.

MRI of aggressive bone lesions of childhood.

MRI can yield valuable information about involvement of bone and soft tissue by aggressive bone lesions of childhood. The 18 lesions evaluated with MRI included osteosarcoma, Ewing's sarcoma, histiocytosis, osteomyelitis, osteoblastoma, giant cell tumor, aneurysmal bone cyst, ossifying fibroma, and fibrous dysplasia. In 12 children, where CT correlation was available, MRI provided equal or more information than CT. Our study shows that MRI should be the modality of choice in the evaluation of aggressive bone lesions of childhood, after conventional radiography.

Fluoroquinolones. New miracle drugs?

Fluoroquinolones are the newest antibiotics for treatment of some infections caused by highly resistant bacteria (eg, Pseudomonas aeruginosa). Clinically sound indications for these drugs include resistant infections of the urinary, intestinal, and respiratory tracts; otitis externa; osteomyelitis; foot infections in diabetic patients; and late infections complicating burns. Fluoroquinolone therapy is not indicated in children younger than 12 years old and patients with foreign bodies, abscesses, pelvic inflammatory disease, or infections requiring a prophylactic antibiotic. Drug interactions …

MRI of postamputation neuromas.

Magnetic resonance imaging of 24 symptomatic lower limb amputations revealed that postamputation neuromas were easily diagnosed in above-the-knee amputees. Visualization of nerve trunks in below-the-knee amputees was difficult due to greater compactness of calf muscles. Neuromas on magnetic resonance imaging appeared ovoid or rounded and were usually heterogenous and isointense with muscle. Magnetic resonance imaging also showed additional causes of stump pain such as scar formation, fat in atrophied muscles, soft tissue abscesses, osteomyelitis, and hematomas.

Non-iatrogenic deep vein thrombosis of lower extremities in children.

Non-iatrogenic ileo-femoral vein thrombosis is rarely encountered in childhood. Six children with postphlebitic leg following venous thrombosis are presented. Venous thrombosis occurred following peritonitis, osteomyelitis and scarlet fever. None of the patients had history of trauma or intravenous treatment. Based on world literature and our experience, the main clinical and radiological manifestations of this rare entity are summarised. Early detection and aggressive anti-coagulant therapy of deep vein thrombosis may probably prevent post-thrombotic syndrome--a major disabling condition in c…

Squamous cell carcinoma arising in acne conglobata.

A squamous cell carcinoma was found on the back of a man who had had acne conglobata for more than forty years. Squamous cell carcinomas called Marjolin's ulcers may arise in the sites of chronic inflammation and scarring such as thermal burn scars, discoid lupus erythematosus, leg ulcerations, and foci of osteomyelitis. Squamous cell carcinoma has also been reported in all three diseases included in the so-called "follicular occlusion triad": acne conglobata, dissecting cellulitis of the scalp, and hidradenitis suppurativa.

Augmentin treatment of bacterial infections in hospitalised patients.

Twenty-five hospitalised patients were evaluated after treatment with oral augmentin (amoxycillin and clavulanic acid). Ten of 14 with respiratory tract, four of eight with urinary tract and five of five with miscellaneous infections (two with osteomyelitis, two typhoid carriers and one with typhoid fever) were cured. Three of four infections caused by amoxycillin resistant bacteria were cured. The drug was well tolerated even at relatively high clavulanic acid doses up to 1.0 g/day. Augmentin is a potentially useful antibiotic combination.

A case report of Transnasal Endoscopic Drainage for Upper Parapharyngeal Abscess.

We report a case of upper parapharyngeal abscess arising from progression of a central skull base osteomyelitis, in a patient with poorly controlled diabetes mellitus. Our case is unusual for its presentation in an upper parapharyngeal location, involvement of the skull base, and poor response to medical treatment, hence requiring transnasal endoscopic surgical drainage to achieve definitive treatment. Here, we share our experience with a safe and effective technique of transnasal endoscopic drainage of a parapharyngeal abscess. Laryngoscope, 134:4274-4277, 2024.

Skull Base Aspergillus Infection Mimicking Tumor Recurrence: A Nasopharyngeal Carcinoma Case Study.

We report a case of recurrent nasopharyngeal carcinoma postnasopharyngectomy, presenting with headaches. MRI revealed abnormal signals of the clivus with enhancement, and FDG PET/CT indicated intense uptake in the nasopharynx, clivus, and left neck lymph nodes. Bone SPECT/CT showed bony erosion and uptake in bilateral skull base areas. Biopsy confirmed aspergillosis. Despite the challenges in distinguishing tumor invasion from Aspergillus infection on MRI, bone SPECT/CT, and FDG PET/CT, the short postsurgery period and extensive uptake suggested skull base osteomyelitis.

Current Status of Molecular Imaging of Infection: A Primer.

OBJECTIVE. Molecular imaging of infection has evolved since its inception. This articles covers a number of infectious processes, including osteomyelitis, diabetic foot infections, periprosthetic infections, cardiovascular infections (including those due to implantable devices), abdominal infections, and fever of unknown origin. Planar, SPECT, SPECT/CT, 18F-FDG PET/CT, and PET/MRI are also discussed. CONCLUSION. Molecular imaging of infection is a complex topic. Knowledge of the available modalities allows the imager to select the highest-yield study to answer the clinical question.

Thoracopulmonary disease due to Aggregatibacter actinomycetemcomitans in a pediatric patient.

Aggregatibacter actinomycetemcomitans is a gram-negative coccobacillus that is part of the usual flora of the oral cavity and is mainly associated with oral and dental pathology; however, in some cases, it has been isolated as the primary causative agent in systemic conditions such as infectious endocarditis, pneumonia, skin and soft tissue infections, and osteomyelitis. We present the case of a previously healthy patient who developed an invasive disease caused by Aggregatibacter actinomycetemcomitans with pulmonary involvement and fistulization to the chest wall, and the difficulties at the …

Early Detection of Pyogenic Sacroiliitis by MRI: A Case Report.

Pyogenic sacroiliitis is rare and difficult to diagnose because of its deep location and the absence of definitive clinical signs. Delayed diagnosis can result in complications such as abscess or sequestration formation, prolonged sepsis, and chronic joint deformity. MRI is a well-known, noninvasive imaging technique with a high sensitivity and specificity for osteomyelitis diagnosis. Here, we present the MRI findings for pyogenic sacroiliitis, 3 days after symptom onset, in an 11-year-old girl and confirm its value in early diagnosis of pediatric pyogenic sacroiliitis.

Anaplastic large cell lymphoma ALK-positive variant of primary bone lymphoma associated with melanoma.

We describe in detail a case of the anaplastic large cell lymphoma (ALCL) type of primary bone lymphoma, which initially was diagnosed and treated as osteomyelitis. The diagnosis was delayed because of unspecific clinical symptoms and uncertain radiographs and histology. Only relapse of the lymphoma from the same area with involvement of the soft tissue and local lymph nodes allowed to establish a correct diagnosis and start treatment. Also, in this case, we observed the development of the second cancer (melanoma), which has the same cytogenetic abnormality as ALCL (t[2;5]).

[The complex pediatric middle ear hemangioma].

Hemangiomas in the pediatric middle ear are a rare occurrence. We present the case of a 3-year-old boy with unilateral hearing impairment and recurrent otitis media with effusion due to a hemangioma in the middle ear. This caused a venous sinus thrombosis, osteomyelitis, and mastoiditis. Interdisciplinary treatment was performed. This case shows that even benign lesions in the middle ear can cause severe, life-threatening complications due to the close proximity of important anatomic structures in the middle ear. Every case of asymmetric hearing loss should thus be examined in detail.
