Seeing is Believing - A Qualitative Study Exploring What Motivates Medical Students to Pursue a Career In General Surgery.

OBJECTIVE: The personal statement (PS) is a rich text in which medical students introduce themselves to the programs to which they are applying. There is no prompt or agreed upon structure for the personal statement. Therefore it represents a window in to medical students' beliefs and perceptions. The goal of this study was to identify what events or experiences motivate medial students to pursue a career in general surgery. Previous work in this area has been largely survey and interview-based. This study is unique in that it looks at what medical students say when not prompted. DESIGN: This

What older people and their relatives say is important during acute hospitalisation: a qualitative study.

BACKGROUND: Due to the growing population of older people across the world, providing safe and effective care is an increasing concern. Older persons in need for hospitalisation often have, or are susceptible to develop, cognitive impairment. Hospitals need to adapt to ensure high-quality care for this vulnerable patient group. Several age-friendly frameworks and models aiming at reducing risks and complications have been promoted. However, care for older people must be based on the persons' reported needs, and relatives are often an important part of older persons' social support. The primary

Sexual and Gender Minority Persons' Perception of the Female Sexual Function Index.

BACKGROUND: Patient-reported outcome instruments to assess sexual functioning typically assume that patients are heterosexual and have a single sexual partner, thus they may have limited applicability for sexual and gender minority (SGM) populations as well as for nonpartnered individuals or those with multiple partners. AIM: To explore the perceptions of SGM persons regarding the Female Sexual Function Index (FSFI), a commonly used sexual functioning questionnaire. METHODS: We conducted 2 rounds of cognitive interviews with 27 SGM persons with and without a cancer diagnosis. Interviews were

Feasibility and effectiveness of a novel dynamic arm support in persons with spinal muscular atrophy and duchenne muscular dystrophy.

BACKGROUND: Neuromuscular disorders (NMD) commonly affect the upper extremity. Due to muscle weakness, performance of daily activities becomes increasingly difficult, which leads to reduced independence and quality of life. In order to support the performance of upper extremity tasks, dynamic arm supports may be used. The Yumen Arm is a novel dynamic arm support specially developed for people with NMD. The aim of this study is to evaluate the feasibility and effectiveness of the Yumen Arm in persons with Duchenne Muscular Dystrophy (DMD) and persons with Spinal Muscular Atrophy (SMA). METHODS

Validation of a Biomarker Panel and Longitudinal Biomarker Performance for Early Detection of Ovarian Cancer.

OBJECTIVES: Longitudinal multimarker combinations have the potential to improve sensitivity while maintaining the high specificity required for the early detection of ovarian cancer. The use of multiple markers to improve sensitivity over cancer antigen 125 (CA125) in longitudinal algorithms for early ovarian cancer detection requires the selection of markers with optimal discriminatory power and low longitudinal variance relative to disease-initiated changes. Our objective was to identify a multimarker panel suitable for ovarian cancer, where each individual marker has its own baseline, permi

Risk factors for keratinocyte skin cancer in patients diagnosed with melanoma, a large retrospective study.

BACKGROUND: Melanoma survivors are at an increased risk of developing other malignancies, including keratinocyte skin cancer (KSC). While it is known that many risk factors for melanoma also impact risk of KSC in the general population, no previous study has investigated risk factors for KSC development in melanoma patients. OBJECTIVE: We assessed associations of personal and clinical characteristics, including skin phenotype and variations in the melanocortin 1 receptor (MC1R) gene, with KSC risk in melanoma patients. PATIENTS AND METHODS: We used prospective follow-up information on 1200 p

Perspectives of service users and carers with lived experience of a diagnosis of personality disorder: A qualitative study.

UNLABELLED: WHAT THE PAPER ADDS TO EXISTING KNOWLEDGE?: Personality disorder is a serious mental health condition affecting up to 52% of psychiatric outpatients and 70% of inpatients and forensic patients. People with a diagnosis of personality disorder have higher morbidity and mortality than those without. Service users and carers reported a lack of training for staff in the management of individuals with a diagnosis of personality disorder, particularly with regard to self-harm and suicidal behaviours. Staff burnout creates barriers to compassionate person-centred care for individuals with

A practice research study concerning homeless service user involvement with a programme of social support work delivered in a specialized psychological trauma service.

ACCESSIBLE SUMMARY: Homeless persons are known to be highly vulnerable to psychological trauma, in events triggering periods of homelessness and the considerable social isolation and adversity suffered when homeless. This study provides an account of how mental health support work is experienced by homeless service users when it is informed by a person-centred, non-directive approach and implemented by trainee health and social care professionals under the auspices of a specialized psychological trauma service. The study draws upon material gathered from interviews with service users domiciled

21st century (clinical) decision support in nursing and allied healthcare. Developing a learning health system: a reasoned design of a theoretical framework.

In this paper, we present a framework for developing a Learning Health System (LHS) to provide means to a computerized clinical decision support system for allied healthcare and/or nursing professionals. LHSs are well suited to transform healthcare systems in a mission-oriented approach, and is being adopted by an increasing number of countries. Our theoretical framework provides a blueprint for organizing such a transformation with help of evidence based state of the art methodologies and techniques to eventually optimize personalized health and healthcare. Solution-Processed Organic UV-Sensitive Neuromorphic Sensors for Healthy Protection Application.

Ultraviolet (UV) radiation is a key environmental trigger for the flare-ups of systemic lupus erythematosus (SLE), making personalized UV monitoring and early warning an urgent unmet clinical need for SLE management. Conventional UV detectors, however, cannot adapt to the cumulative and delayed characteristics of UV-induced damage in SLE patients or the interindividual differences in UV sensitivity and thus fail to provide personalized protection strategies. To enable precise UV damage assessment and proactive early warning for SLE patients, we developed an organic optoelectronic synaptic devi

Personality traits and the managerial capacity of community-based facilities providing HIV services to key populations in Kenya and Malawi.

Community-based facilities delivering HIV services to key populations often face significant operational and financial challenges, making the quality of management in these organizations particularly important. A growing body of evidence links management quality to health facility performance. However, research on management in health facilities has focused largely on structural characteristics and formal qualifications, with less attention to the non-cognitive characteristics of managers themselves, particularly personality traits. This is a cross-sectional, quantitative analysis of 45 facili

Effectiveness of the capability approach in rehabilitation for persons with neuromuscular diseases: A controlled before-after study.

BACKGROUND: Rehabilitation of persons with neuromuscular diseases (NMD) requires a personalised approach that focuses on achieving and maintaining a level of functioning that enables them to be in a state of well-being. The capability approach states that well-being should be understood in terms of capabilities, which are the opportunities that people have to be and do things they have reason to value. The aim of this study is to investigate whether providing care inspired by the capability approach (capability care) has an effect on the well-being of persons with NMD. METHODS: In the Rehabil

Brain-based predictive modeling of mind-wandering and involuntary thought: the idiographic approach.

A substantial portion of waking life consists of mental experiences that arise involuntarily, including mind-wandering, spontaneous thought and rumination. Given that these experiences are fundamental to cognitive function and mental health, there has been growing scientific and clinical interest in using measures of brain activity to model and predict the momentary occurrence of these various forms of involuntary thought. In the most common machine learning approach in neuroimaging and electrophysiology research, model training data include neural features derived from a population of individ

Mapping the Trajectories of Social Relations for White, Black, and Hispanic/Latino Individuals Approaching Death With Dementia.

OBJECTIVES: Larger social networks are associated with a lower risk of dementia, but little is known about how social networks shift as someone with dementia approaches death. We investigate these shifts while giving special attention to race and ethnicity, which are related to different dementia patterns, social network sizes, and social network makeup. METHODS: This study included 2,301 deceased people with dementia from the Health and Retirement Study (2004-2018; waves = 8). Multilevel models estimated associations between dementia, race/ethnicity, time, and close family and friend network
