Predictors and prognosis of paroxysmal atrial fibrillation in general practice in the UK.

BACKGROUND: Natural history of paroxysmal atrial fibrillation (AF) is not very well documented. Clinical experience suggests that paroxysmal AF could progress to chronic AF with estimates ranging between 15 and 30% over a period of 1-3 years. We performed an epidemiologic study to elucidate the natural history of paroxysmal AF, this study estimated its incidence in a general practice setting, identified associated factors and analyzed the progression into chronic AF as well as the mortality rate. METHODS: Using the UK General Practice Research Database (GPRD), we identified patients aged 40-8

Complications following bilateral prophylactic mastectomy.

BACKGROUND: Bilateral prophylactic mastectomy significantly decreases breast cancer risk, but complications of the procedure have only been described in single-site studies. We describe the frequency and type of complications in women who underwent bilateral prophylactic mastectomy in a multisite community-based cohort. METHODS: Women aged 18-80 years undergoing bilateral prophylactic mastectomy without a personal history of breast cancer at one of six health plans were eligible. We identified women from automated data sources, then reviewed hospital data, ambulatory notes, and other chart el

Physical activity and risk factors for hip fractures in Thai women.

Hip fractures are among the most important causes of ill health and death among elderly people. Several potentially modifiable risk factors have been reported. Most claimed physical activity as a promising, inexpensive preventive measure for hip fracture. However, knowledge about risk factors for hip fracture in Asian populations is very limited. We therefore conducted a case-control study to assess the relationships between physical activity and risk of hip fractures in Thai women. From 14 hospitals in Thailand, 229 cases with a radiologically confirmed first hip fracture were enrolled. Two h

Social isolation in community-dwelling seniors: an evidence-based analysis.

UNLABELLED: In early August 2007, the Medical Advisory Secretariat began work on the Aging in the Community project, an evidence-based review of the literature surrounding healthy aging in the community. The Health System Strategy Division at the Ministry of Health and Long-Term Care subsequently asked the secretariat to provide an evidentiary platform for the ministry's newly released Aging at Home Strategy. After a broad literature review and consultation with experts, the secretariat identified 4 key areas that strongly predict an elderly person's transition from independent community living

Past wounds, present bonds, future mothers: item-level analysis of perinatal risk profiles.

BACKGROUND: Perinatal depression is shaped by interacting psycho-relational risk factors-childhood adversity, current relational functioning, perceived social support, and prenatal attachment. Variable-centered approaches focused on isolated predictors may obscure how these dimensions combine within individuals to generate distinct vulnerability configurations. OBJECTIVE: To identify person-centered profiles of psychosocial functioning during pregnancy and to examine their association with depressive symptoms across the perinatal period. PARTICIPANTS AND SETTING: A sample of 774 pregnant wom

A 5-Metabolite Plasma Panel Predicts Progression-Free Survival and Adds Prognostic Value in Imatinib-Treated Gastrointestinal Stromal Tumors.

Among patients with gastrointestinal stromal tumor receiving first-line imatinib, outcomes vary substantially, and traditional stratification based on KIT/PDGFRA genotype and tumor burden does not fully explain individual prognostic differences. Baseline plasma metabolomics may noninvasively reflect the tumor-host systemic state and provide additional prognostic information. This single-center prospective cohort study aimed to develop and internally validate a baseline plasma-based metabolomic risk score and to assess its incremental discrimination, calibration, and clinical net benefit over a

"I Wouldn't Be Here Without Her": Chosen Sisters as a Social Determinant of Mental Health.

ObjectiveIn recent years, more attention has been given to the unique challenges Black women in the U.S. face as it pertains to mental health and psychological distress. Research has consistently shown that close relationships among this demographic can be emotionally supportive, stigma-reducing, serve as isolation buffers, and are positively correlated with reduced rates of depression. Despite a growing body of literature on the benefits of social ties, chosen sister relationships, a subsystem within adaptive family structures, have received limited research in the public health sector, parti

Deep learning to predict future cognitive decline: a multimodal approach using brain MRI and clinical data.

Predicting the trajectory of clinical decline in aging individuals is a pressing challenge, especially for people with mild cognitive impairment, Alzheimer's disease, Parkinson's disease, or vascular dementia. Accurate predictions can guide treatment decisions, identify risk factors, and optimize clinical trials. In this study, we compared two deep learning approaches for forecasting changes, over a 2-year interval, in the Clinical Dementia Rating scale 'sum of boxes' score (sobCDR), as a continuous outcome (regression). This is a key metric in dementia research and clinical trials, and scores

Understanding the psychosocial impact and healthcare experiences of people living with epilepsy in the West Bank, Palestine.

BACKGROUND: Epilepsy has several effects on patients and their families, including experiences of fear, stigma, and discrimination. Insights into their well-being are lacking in several under resourced and politically constrained contexts, such as Palestine. This study aimed to explore the lived experiences of people with epilepsy, focusing on their personal, social, and healthcare-related challenges and coping mechanisms. METHODS: A qualitative study employing a Husserlian phenomenological approach was conducted through semi structured in-depth interviews from February to April 2024 from prim

Qualitative insights into nurses' emotional and functional experiences in the 2023 military humanitarian mission to Turkey.

BACKGROUND: Humanitarian aid delegations play a crucial role in assisting victims of disasters worldwide. Nursing and medical teams are involved in treating injuries and illnesses, promoting health, and providing emotional support to patients. Such teams often have long-term emotional, social, and functional impacts. Those impacts can include post-traumatic stress disorder, disrupted social relationships, and decreased professional efficacy. This paper examines the emotional, social, and professional experiences of nurses participating in the Israeli Defence Forces’ (IDF) 2023 humanitarian ope

Inside Prognostication Meetings in the Neuro-Intensive Care Unit: How Clinicians Convey Evidence to Families - A Mixed-Methods Study.

ObjectivesEffective communication with surrogate decision-makers is critical in neuro-intensive care settings, where complex prognostication shapes high-stakes decision-making. To better understand how clinicians convey prognostic evidence in clinician-family meetings for patients with severe acute brain injuries (SABI), we characterized and quantified types of evidence-sharing used by clinicians in these meetings and explored their independent predictors and associations with quality of communication (QOC) and patient-centeredness.MethodsIn this mixed-methods study, we first qualitatively ana

Impact of the COVID-19 Pandemic on Knowledge, Attitudes, and Practices Related to Avian Influenza Among Poultry Workers in Cambodia.

BACKGROUND: Live bird markets (LBMs) are high-risk environments for the amplification and transmission of avian influenza viruses (AIVs). In Cambodia, limited data exist on knowledge, attitudes, and practices (KAP) among poultry workers, who represent a key occupational risk group. This study assessed KAP related to avian influenza (AI) among poultry workers in three major Cambodian LBMs, with a focus on changes before and during the COVID-19 pandemic. METHODS: A cross-sectional survey of 100 poultry workers was conducted using a structured questionnaire covering socio-demographics, AI awaren

Colorectal Cancer in Bhutan: Identifying Health System Gaps and Priorities.

BACKGROUND: Colorectal cancer (CRC) is an emerging public health concern in Bhutan, a lower-middle-income country with a geographically challenging terrain and limited specialised oncology services. As the country undergoes an epidemiological and economic transition, CRC numbers are steadily increasing. While Bhutan has made progress in cancer control through national programmes, CRC has not been a priority cancer, and health system readiness for its screening and management remains unclear. This study aimed to assess CRC care delivery in Bhutan and identify systems gaps using the World Health

Patient-centered communication and emotional well-being among cancer survivors: The role of online peer interaction and the COVID-19 context.

OBJECTIVE: Patient-centered communication (PCC) predicts better emotional well-being among cancer survivors, but whether this benefit holds across changing care contexts is unclear. The COVID-19 pandemic, which disrupted in-person care and shifted survivors toward digital support, offers a natural test of whether PCC's value depends on context. We examined how the PCC-well-being association varies across pandemic periods and online participation, and whether self-efficacy consistently mediates it. METHODS: Data came from cancer survivors (N = 2765) across four Health Information National Tren

Qualitative evaluation of the implementation and national roll-out of the NHS App in England.

BACKGROUND: The NHS App launched in 2019 as the 'digital front door' to the National Health Service in England with core features including General Practitioner (GP) appointment booking, repeat prescriptions, patient access to records and, later on, COVID-19 vaccination certification. Similar patient portals have been adopted in different formats and with variable levels of success. In this longitudinal study (2021-2023) we examined how the NHS App became implemented in the pandemic context and beyond. METHODS: We recruited 88 participants in 62 qualitative interviews and four focus groups. P

Multimodal large language models for women's reproductive mental health.

BACKGROUND: Women's risk of mental health conditions fluctuates across the lifespan with hormone-mediated reproductive transitions. Reproductive psychiatry, a relatively new subspecialty, focuses on preventing and treating these conditions throughout various reproductive stages. Multimodal large language models (MLLMs) are advanced artificial intelligence (AI) systems that can process and integrate information across multiple modalities, including text, images, audio, and video. Although MLLMs have shown broad utility in healthcare, their potential in reproductive psychiatry remains largely un
