Could Expanding and Investing in First-Episode Psychosis Services Prevent Aggressive Behaviour and Violent Crime?

OBJECTIVE: Some persons developing, or presenting, schizophrenia engage in aggressive behaviour (AB) and/or criminal offending. Most of thes … at FEP. Studies are also necessary to determine whether providing such programs in hospitals and/or prisons, with long-term community after-care, and in some cases with court orders to participate in treatment, would enhance effectiveness. Whether investing at FEP would be cost-effective requires investigation.

Prevalence of Novel Psychoactive Substance (NPS) Use in Patients Admitted to Drug Detoxification Treatment.

BACKGROUND: About 15 years ago, a diverse group of new recreational psychotropic substances began to emerge, which were marketed for example … ly a few times. An important reason for their use was that NPS were not detected by drug testing in prisons or drug treatment facilities. Cathinones, herbal drugs or other NPS had rarely been used during lifetime. NPS use during the last 30 days before admission was nearly zero. This was confirmed by urine analysis results. In contrast, lifetime and curre …

A Cohort Study of Emergency Department Trauma Evaluation and Outcomes by Incarceration Status.

People experiencing incarceration (PEI) have high rates of trauma and emergency department (ED) utilization, yet limited data describe their ED evaluation and outcomes. We sought to compare trauma patient characteristics, ED care, and disposition by incarceration status. We hypothesized PEI would have shorter ED lengths-of-stay (LOSs), undergo more cross-sectional imaging, and have lower rates of admission. We conducted a single-center retrospective cohort study of adult incarcerated and nonincarcerated trauma patients presenting to a Level 1 trauma center from 1/2010 to 12/2020. The primary o

Neurodevelopmental and environmental pathways to psychopathy: Insights from minor physical anomalies in forensic psychiatric population.

Psychopathy is a multidimensional disorder influenced by behavioral, social, and biological factors. Minor physical anomalies (MPAs) are subtle morphological variations that arise from developmental disruptions during the prenatal period and are considered potential phenotypic indicators of such neurodevelopmental irregularities. Previous research has examined several neurodevelopmental aspects of psychopathy. However, the specific relationship between MPAs and psychopathy has not yet been systematically investigated. This cross-sectional study investigated the association between MPAs and psy

Assessing the Relationship Among Childhood Maltreatment, Personality Dysfunction, and Externalizing Behavior.

Research has sought to understand the relationship among childhood maltreatment, maladaptive personality, and antisocial behaviors. However, most of the literature overlooks possible gender differences by using primarily male samples. The current study examined these relationships, focusing on the mediating role of personality dysfunction, in samples of women who are incarcerated (N = 200) and undergraduate students (N = 187). Correlations revealed moderate to strong associations among childhood trauma, personality psychopathology, and antisocial behavior (rs = .31-.66). The findings demonstra

Risk of Incident Mild Cognitive Impairment and Dementia Soon After Leaving Incarceration Among a US Veteran Population.

OBJECTIVES: Increasing numbers of older adults are reentering community following incarceration (i.e., reentry), yet risk of incident neurodegenerative disorders associated with reentry is unknown. Our objective was to determine association between reentry status (reentry vs never-incarcerated) and mild cognitive impairment (MCI) and/or dementia. METHODS: This nationwide, longitudinal cohort study used linked Centers for Medicare & Medicaid Services and Veterans Health Administration data. Participants were aged 65 years or older who experienced reentry between October 1, 2012, and December 3

A descriptive analysis of rural-urban overdose experiences among incarcerated women with opioid use disorder.

BACKGROUND AND OBJECTIVES: Despite elevated overdose risk in rural communities, rural/urban differences in overdose risk factors are understudied among women with criminal legal system involvement. This study examines substance use and overdose among incarcerated women. METHODS: Women (N = 900) were randomly selected from nine Kentucky jails, screened, and interviewed as part of a larger study. Bivariate analyses were used to identify group differences. RESULTS: Rural women were less likely to report pre-incarceration overdose and to receive emergency services post-overdose. Correlates of Staff Acceptability of a Novel Telemedicine-Delivered Medications for Opioid Use Disorder Program in a Rural Detention Center.

Opioid overdose death is significantly increased immediately following incarceration. Evidence-based medications are underutilized in rural jails and detention centers. We have reported our efforts to address this gap through telemedicine-based medications for opioid use disorder treatment (tele-MOUD) for incarcerated patients. Staff acceptance and perceptions are critically important factors in the assurance of program validation. We assessed tele-MOUD acceptability and perceptions of effectiveness and stigma in one detention center. Overall, we found that jail staff's general acceptability o

Prevalent Adverse Childhood Experiences Among Young Adults Returning Home From Jail: The Need for Trauma-Informed Reentry Services.

Prevalence of adverse childhood experiences (ACEs) and the extent to which they relate to health among young adults (ages 18-25) returning home from jail is underexamined. To build on the growing literature examining associations between ACE exposure among young people involved with carceral systems and health, we (1) measured ACE prevalence and (2) explored associations between ACEs and health/well-being indicators among young adults experiencing reentry. Using a telephone survey on reentry experiences, participants completed an ACE screening, single-item responses on health and social indica

Impact of Changes in Criminal-Legal Practices During the COVID-19 Pandemic on the HIV Risk Behaviors of Women Who Use Drugs: Protocol for a Multimethods Qualitative Study.

BACKGROUND: HIV risk behavior in women who use drugs is related to myriad psychosocial issues, including incarceration. The experience of incarceration elevates women's HIV risk by disrupting social networks, housing, employment, and access to health care. During the COVID-19 pandemic, changes in criminal-legal practices resulted in decreased incarceration, especially among women. These changes may have largely altered HIV risk among women who use drugs, depending on their access to care in the community. OBJECTIVE: This study seeks to build knowledge about the impact of shifts in criminal-le

Does stress from incarceration of family and friends contribute to signs of early vascular ageing in African American women?

BACKGROUND: Early vascular ageing (EVA) contributes to elevated risk of cardiovascular disease (CVD), which disproportionately affects African American women. Incarceration, an event disproportionately impacting African Americans, may be a stressor contributing to EVA in African American women. Further, the subjective perspective, commonly referred to as appraisal, of incarceration may also be important for health. We hypothesised that having family and/or friends incarcerated and appraising the incarceration as upsetting would be associated with indices of EVA. METHODS: In a community-based

"Because I Was a Criminal and Drug Addict.": Experiences of Anti-Black Gendered Racism and Reproductive Injustice Among Black Pregnant and Postpartum Women with a Substance Use Disorder and Incarceration and Family Policing Histories.

Racism pervades the US criminal legal and family policing systems, particularly impacting cases involving women with a history of a substance use disorder (SUD). Laws criminalizing SUD during pregnancy disproportionately harm Black women, as do family policing policies around family separation. Discrimination within intersecting systems may deter Black pregnant women with a SUD from seeking evidence-based pregnancy and substance use care. This convergent parallel mixed-methods study aimed to illuminate how systemic oppression influenced the lived experiences of Black mothers with a SUD, facing

Lifetime and Jail-Specific Suicidal Ideation: Prevalence and Correlates in a Sample of People in Jail in the United States.

Despite high suicide mortality in U.S. jails, there is limited research into precursors for suicide in this population, such as suicidal ideation. The current study examined the prevalence and correlates of lifetime and jail-specific suicidal ideation among a sample of 196 individuals (137 men) in custody in a U.S. jail. Nearly half the sample had reported lifetime suicidal ideation (45%), whereas 30% had reported jail-specific suicidal ideation. Adjusted correlates of lifetime suicidal ideation included a history of mental illness (OR = 2.79) and drug use (OR = 2.70). Adjusted correlates of j

Rapid Transmission of Severe Acute Respiratory Syndrome Coronavirus 2 in Detention Facility, Louisiana, USA, May-June, 2020.

To assess transmission of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in a detention facility experiencing a coronavirus disease outbreak and evaluate testing strategies, we conducted a prospective cohort investigation in a facility in Louisiana, USA. We conducted SARS-CoV-2 testing for detained persons in 6 quarantined dormitories at various time points. Of 143 persons, 53 were positive at the initial test, and an additional 58 persons were positive at later time points (cumulative incidence 78%). In 1 dormitory, all 45 detained persons initially were negative; 18 days later,

Optimisation in the multidisciplinary approach to patients with hepatitis C in the era of direct-acting antivirals.

The advents of current direct-acting antiviral treatments has revolutionised the therapeutic approach to hepatitis C, increasing cure rates to above 90% and substantially simplifying treatment, which translates into benefits for patients, clinicians and the health system. These new drugs allow cure to be achieved, irrespective of the patient's characteristics, with tolerability similar to that of placebo and few drug reactions with concomitant medication. This in turn improves patients' quality of life and wellbeing. Moreover, these drugs allow multidisciplinary optimisation of the approach to
