Surgical strategy for improving forearm and hand function in late obstetric brachial plexus palsy.

For the purpose of treatment, obstetric brachial plexus palsy can be subdivided into two distinct phases: initial obstetric brachial plexus palsy, and late obstetric brachial plexus palsy. In the latter, nerve surgery is no longer practical, and treatment often requires palliative surgery to improve function of the shoulder, elbow, forearm, and hand. Late obstetric brachial plexus palsy in the forearm and hand includes weakness or absence of wrist or metacarpophalangeal or interphalangeal joint extension; weakness or absence of finger flexion; forearm supination, or less commonly pronation con

Dynamic Three-Dimensional Ultrasound to Evaluate Scapular Movement Among Manual Wheelchair Users and Healthy Controls.

BACKGROUND: A large percentage of individuals with spinal cord injury (SCI) report shoulder pain that can limit independence and quality of life. The pain is likely related to the demands placed on the shoulder by transfers and propulsion. Shoulder pathology has been linked to altered scapular mechanics; however, current methods to evaluate scapular movement are invasive, require ionizing radiation, are subject to skin-based motion artifacts, or require static postures. OBJECTIVE: To investigate the feasibility of applying 3-dimensional ultrasound methods, previously used to look at scapular

Latarjet procedure using subscapularis split approach offers better rotational endurance than partial tenotomy for anterior shoulder instability.

PURPOSE: Latarjet, which is a coracoid bone block procedure, is an effective treatment for anterior shoulder instability with glenoid bone loss. During this reconstructive procedure the subscapularis may be tenotomized or be split to expose the glenoid neck. The aim of this study was to assess the effect of subscapularis management on functional outcomes and internal and external rotation durability and strength. Hypothesis is that the subscapularis split approach will result in better functional results and superior internal rotation strength and endurance. METHODS: The study included 48 pat

Outcomes of arthroscopic Hill-Sachs remplissage and anterior Bankart repair: a retrospective controlled study including ultrasound evaluation of posterior capsulotenodesis and infraspinatus strength assessment.

BACKGROUND: Hill-Sachs lesions are compression fractures that result from shoulder dislocation. They involve "engaging" the humeral head on the anterior glenoid rim when the arm is abducted and externally rotated. The defect grows as the number of dislocations increases. HYPOTHESIS: Arthroscopic remplissage and anterior Bankart repair do not significantly affect infraspinatus strength while ensuring healing of the capsulotenodesis. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: Sixty-one patients with traumatic anterior shoulder instability treated by arthroscopic Bankart repair

Imaging pediatric sports injuries: upper extremity.

With increasing youth participation in organized sports, more injuries in this age group are being treated by primary care and sports medicine physicians. Overuse injuries are much more common now than in past decades, with Little League shoulder, Little League elbow, and gymnast wrist being particular concerns. Rotator cuff tears and glenoid labral injuries, once thought to be rare in this age group, are also more common now. Osteochondritis dissecans of the elbow is relatively common and typically fares poorly without surgery. Wrist abnormalities that occur nowadays include triangular fibroc

Is the Anterior Injection Approach Without Ultrasound Guidance Superior to the Posterior Approach for Adhesive Capsulitis of the Shoulder? A Sequential, Prospective Trial.

BACKGROUND: Shoulder injections for conditions such as adhesive capsulitis are commonly performed and can be administered through image-based or landmark-based injection approaches. Ultrasound-guided injections are widely used and accurate because ultrasound allows real-time visualization of the needle and injected contrast. Landmark-based injections would be advantageous, if they were accurate, because they would save the time and expense associated with ultrasound. However, few prospective studies have compared well-described landmark-based shoulder injection techniques without ultrasound.

PyroTITAN Pyrocarbon Shoulder Hemiarthroplasty: Clinical and Radiographic Outcomes with Medium-Term Follow-up.

BACKGROUND: Pyrocarbon hemiarthroplasty (HA) is a recent option for younger patients with end-stage glenohumeral joint (GHJ) arthritis. Early results are promising but limited by study bias. The aim of this study was to evaluate medium-term clinical and radiographic outcomes following PyroTITAN pyrocarbon HA. METHODS: One hundred and nineteen shoulders with GHJ arthritis in 115 patients (mean age, 56.5 years; 92 shoulders were in male patients) underwent PyroTITAN pyrocarbon HA. Primary patient-reported outcome measures (PROMs) included the Western Ontario Osteoarthritis of the Shoulder Index

A retrospective study on the short-term efficacy of injection and manipulation in primary frozen shoulder.

There is no consensus on the optimal treatment approach for frozen shoulder. This study aimed to evaluate the short-term clinical efficacy of intra-articular injections combined with manipulative therapy, intra-articular injections alone, and local steroid pain point injection for the treatment of primary frozen shoulder. A retrospective study was conducted on 202 patients with frozen shoulder who visited the Department of Orthopedics from January 2020 to April 2022. This study followed the Consolidated Standards of Reporting Trials guidelines for reporting retrospective studies. The patients

Shoulder MRI features with clinical correlations in subacromial pain syndrome: a cross-sectional and prognostic study.

BACKGROUND: Previous studies on shoulder patients have suggested that the prevalence of rotator cuff or bursa abnormalities are weakly related to symptoms and that similar findings are often found in asymptomatic persons. In addition, it is largely unknown whether structural changes identified by magnetic resonance imaging (MRI) affect outcome after treatment for shoulder pain. The purpose of this study was therefore to evaluate the presence of structural changes on MRI in patients with subacromial pain syndrome and to determine to what extent these changes are associated with symptoms and pre

Long-term effects of bupivacaine on cartilage in a rabbit shoulder model.

BACKGROUND: Previous investigations have reported on the chondrotoxicity of bupivacaine in short-term in vivo and in vitro models. This study was designed to provide additional information on the long-term effects of bupivacaine infusion on articular cartilage in an established rabbit shoulder model. HYPOTHESIS: Infusion of bupivacaine into the rabbit shoulder will have long-term deleterious effects on articular cartilage. STUDY DESIGN: Controlled laboratory study. METHODS: Thirty-six rabbits were randomized into 3 groups and were infused over 48 hours with saline (S), bupivacaine alone (B)

Three-dimensional scapulothoracic motion following treatment for breast cancer.

Varying levels of shoulder morbidity following treatment for breast cancer have been reported. Patients report pain, weakness, tightness and reduced functional capacity. Normal painfree motion of the arm and shoulder requires mobility in the scapulothoracic, glenohumeral, acromioclavicular and sternoclavicular joints. Under healthy conditions elevation of the arm is accompanied by scapula retraction, lateral rotation and posterior tilt. However, when scapulothoracic motion is disproportionate to glenohumeral motion, the potential exists for microtrauma and long term pain. A number of studies o

The Addition of Remplissage to Arthroscopic Bankart Repair and Effect on Recurrent Instability in Shoulders With Critical Humeral Bone Loss.

BACKGROUND: Recent literature has shown that inferior Hill-Sachs extension for on-track shoulders is predictive of recurrent instability after arthroscopic Bankart repair alone. Specifically, there is a high risk for recurrent instability when the lesion extends below the humeral equator on sagittal magnetic resonance imaging. This worrisome inferior extension has been termed "critical humeral bone loss (CHBL)." Remplissage has yet to be explored as a potential useful augmentation in patients with CHBL. HYPOTHESIS: The addition of remplissage would decrease recurrence rates for Hill-Sachs les

Capsular release following total shoulder arthroplasty: an analysis of early outcomes.

BACKGROUND: The purpose of this study is to analyze the outcomes of open and arthroscopic capsular release following total shoulder arthroplasty. METHODS: Over 15 years, 19 patients experienced persistent shoulder stiffness after anatomic total shoulder arthroplasty refractory to nonoperative treatment, requiring either open (n = 5) or arthroscopic (n = 14) capsular release. There were seven (39%) patients who had a prior diagnosis of stiffness before the primary arthroplasty. RESULTS: At a follow-up of 2.3 years (1-5.5), there were changes in range of motion, including forward flexion (77°-

Determining On-/Off-track Lesions in Glenohumeral Dislocation Using Multiplanar Reconstruction Computed Tomography Is Easier and More Reproducible Than Using 3-dimensional Computed Tomography.

BACKGROUND: The glenoid track is a useful tool to predict engagement and therefore the risk of recurrence of dislocation in the presence of Hill-Sachs and/or bony Bankart lesions. To assess the glenoid track preoperatively, only methods using 3-dimensional reconstruction (3DR) have been described, but these lack a standardized, reliable, and easy description. PURPOSE/HYPOTHESIS: The purpose was to evaluate a new method for determining the glenoid track using computed tomography (CT) scan with multiplanar reconstruction (MPR) in comparison with using 3DR images. Our hypothesis was that the MPR

Effect of Shoulder Stabilization on Career Length and Performance in National Basketball Association Athletes.

PURPOSE: Untreated episodes of shoulder instability can have major consequences on athletic careers. Operative shoulder stabilization reduces the rate of recurrent instability and allows for high rates of return to sport (RTS). Basketball players who experience an episode of instability have high rates of recurrence, though little is known about postoperative player performance. The purpose of this study was to identify the impact of shoulder instability and surgical stabilization on player performance and career lengths in the National Basketball Association (NBA). METHODS: NBA players who h

Strategies for an anterior capsular shift of the shoulder. A biomechanical comparison.

We evaluated the effect of two different anterior capsular shift strategies on load-induced multidirectional glenohumeral motion. Nine cadaveric shoulders were tested on a special test apparatus in which three motions were constrained but which allowed simultaneous measurement of three translations: anteroposterior, mediolateral, and superoinferior. A simulated Bankart lesion was created by detaching the inferior glenohumeral ligament and labrum from the anterior glenoid from 5 mm superior to the anterior band of the inferior glenohumeral ligament to a point just posterior to the infraglenoid
