Novel insights into the anatomy and histopathology of the sacroiliac joint and correlations with imaging signs of sacroiliitis in case of axial spondyloarthritis.

For a better understanding of the pathophysiology of spondyloarthropathy (SpA), a detailed anatomical description of the sacroiliac joint is required because sacroiliitis is the earliest and most common sign of SpA and an essential feature for the diagnosis of ankylosing spondylitis. Beyond the anatomy, the histopathology of sacroiliac entheses and immunological mechanisms involved in sacroiliitis are crucial for a better understanding of disease causation. In this narrative review, we discuss the core anatomical, histological, and immunohistological observations involved in the development of…

Simultaneous Total Knee Arthroplasty and Proximal Tibiofibular Joint Arthrodesis: A Case Report.

CASE: A 57-year-old man presented with tricompartmental left knee osteoarthritis, as well as proximal tibiofibular joint arthritis and a ganglion cyst. He underwent simultaneous total knee arthroplasty and proximal tibiofibular joint arthrodesis, with an excellent outcome. CONCLUSION: Proximal tibiofibular joint arthritis is uncommon and may be associated with tibiofemoral arthritis, proximal tibiofibular joint instability, and ankylosing spondylitis. We present a case of simultaneous total knee arthroplasty and proximal tibiofibular arthrodesis. This is an effective option for treating patie…

An overview of biologic disease-modifying antirheumatic drugs in axial spondyloarthritis and psoriatic arthritis.

Biologic disease-modifying antirheumatic drugs (bDMARDs) are engineered proteins with high affinity for various proinflammatory immune mediators to reduce inflammation and its sequelae in various rheumatic diseases. These medications, introduced at the advent of the 21st century, have revolutionized the treatment of axial spondyloarthritis (including ankylosing spondylitis) and psoriatic arthritis. Currently approved bDMARDs for axial spondyloarthritis are etanercept, infliximab, adalimumab, golimumab, certolizumab pegol, and secukinumab. For psoriatic arthritis, all of these drugs are approve…

Etanercept-induced leukemia: could increased mean corpuscular volume be a predictor of hematologic malignancy?

In rheumatology practice, anti-tumor necrosis factor (TNF) alpha agents are frequently used medications, more so in ankylosing spondylitis (AS). There are case reports, besides their adverse effects, such as infection and injection site reaction, suggesting these agents may cause solid or hematologic malignancies. Acute leukemia secondary to anti-TNF alpha agents has been rarely reported in patients with AS. In this case report, based on a patient who developed acute leukemia while on the treatment with etanercept, we will discuss whether it is possible to predict acute leukemia by monitoring …

Overview of the complications and sequelae in spinal infections.

Spondylitis or infection of the spine is a spectrum of diseases involving the bone, disks, and/or ligaments. Because of a significant increase in the immunocompromised patient population, spinal infections are a growing and changing group of conditions, making the diagnosis based on imaging more challenging. Most cases of spinal infections are pyogenic and occur after hematogeneous spread of an infection located elsewhere in the body. A prompt diagnosis remains crucial and MR imaging remains the cornerstone in the diagnosis. This article provides a pictorial overview of the complications and s…

Bone Loss Triggered by the Cytokine Network in Inflammatory Autoimmune Diseases.

Bone remodeling is a lifelong process in vertebrates that relies on the correct balance between bone resorption by osteoclasts and bone formation by osteoblasts. Bone loss and fracture risk are implicated in inflammatory autoimmune diseases such as rheumatoid arthritis, ankylosing spondylitis, inflammatory bowel disease, and systemic lupus erythematosus. The network of inflammatory cytokines produced during chronic inflammation induces an uncoupling of bone formation and resorption, resulting in significant bone loss in patients with inflammatory autoimmune diseases. Here, we review and discus…

IgA nephropathy and Reiter's syndrome. Report of two cases and review of the literature.

Immunoglobulin A (IgA) nephropathy is the commonest type of primary glomerulonephritis worldwide. It has previously been reported in association with the seronegative spondyloarthropathies (ankylosing spondylitis, Behcet's syndrome, psoriatic arthritis, Reiter's syndrome and the postenteritic arthritides). Since this condition was first described in 1968, 5 previous case reports of biopsy-proven IgA nephropathy associated with Reiter's syndrome have been published in the English-language literature. Here we report 2 more such cases, along with a review of the literature describing the associat…

Arthritis and HLA-B27 in native North American tribes.

Whereas it is clear that HLA-B27 is increased in many North American natives, the prevalence varies a great deal. There is still a paucity of data on a large number of tribes. There is, however, a suggestion that HLA-B27 positive prevalence follows cultural/linguistic groupings. Reactive arthritis, sacroiliitis, and ankylosing spondylitis also appear to be increased in these people, but their relationship to HLA-B27 and possibly additional genetic predisposing factors is far from clear. Given these data and the known association of infectious agents, more extensive studies are warranted, espec…

[Spondylodiscitis caused by Streptococcus agalactiae].

Two previously healthy adults, a man aged 69 and a woman aged 51 years, presented with spondylitis caused by Streptococcus agalactiae. One patient had fever and acute pain in the neck, the other progressive pain in the lower back. From cultures of blood and bone respectively. S. agalactiae was isolated. Both patients recovered after treatment with benzylpenicillin. S. agalactiae (group B streptococcus) is a wellknown cause of invasive infections in neonates and pregnant adults. Infections in nonpregnant adults are increasingly reported. Chronic conditions such as diabetes mellitus are strongly…

Enteropathic arthritis, Whipple's disease, juvenile spondyloarthropathy, uveitis, and SAPHO syndrome.

During the past year, concepts of spondyloarthropathy and associated gut abnormalities have been further confirmed. In addition, the evolution of spondyloarthropathy to ankylosing spondylitis seems to be determined by the underlying gut inflammation. Other factors present early in spondyloarthropathy that affect the long-term prognosis of the disease have been determined. HLA-B27 has an impact on the severity of spondyloarthropathy. Its presence is also highly associated with uveitis. Paradoxically, HLA-B27+ uveitis is usually benign, with a good prognosis. A subgroup of patients can, however,…

Spontaneous extrusion of an intraocular lens implant.

A 51-year-old man with ankylosing spondylitis had intracapsular cataract extraction and anterior chamber lens implantation in the left eye in 1985. He had localized sclerokeratitis around the corneal section and a protracted anterior uveitis in the immediate postoperative period. He developed a spontaneous extrusion of the implant in 1991 (six years later). The extrusion was through a site 1.5 mm posterior to the previous surgical section. There was no corneal decompensation or corneal melt and no history of trauma. Signs of localized scleritis were present. It is suggested that anterior chamb…

An overview of clinical spectrum and heterogeneity of spondyloarthropathies.

Ankylosing spondylitis and related spondyloarthropathies form a family of rheumatic diseases that are characterized by inflammatory peripheral and axial arthritis, with predilection for sacroiliitis, and a remarkably strong association with a genetic marker, HLA-B27. The association with B27 has provided a great impetus to the epidemiologic studies of spondyloarthropathies and also helped broaden the clinical spectrum of these diseases. There are at least six subtypes of B27, and the x-ray crystallographic structure of the common B27 subtype (B*2705) is now known. Endogenous peptides bound in …

[Utility of Bullard intubating laryngoscope with a special stylet in two cases of difficult tracheal intubation].

The Bullard intubating laryngoscope is useful for cases of difficult tracheal intubation, but a skilled hand is needed to manipulate it. In two cases of difficult tracheal intubation, we used a recently improved Bullard intubating laryngoscope to which a special stylet is attached to introduce an endotracheal tube easily into the larynx. The difficulties of tracheal intubation were caused by micrognathia and trismus in one case and by restriction of neck movement and trismus due to ankylosing spondylitis in the other case. Using the Bullard intubating laryngoscope with the special stylet, intu…

Medical and surgical treatment of seronegative spondyloarthropathies.

Advances in the treatment of the seronegative spondyloarthropathies are reviewed here. The search for effective disease-modifying agents in this group of disorders has continued. Recent attention has focused primarily on sulfasalazine and different immunosuppressive agents. Sulfasalazine has been evaluated by several groups of investigators in ankylosing spondylitis and psoriatic arthritis. Results have been variable, but data suggest that sulfasalazine is less effective for spinal disease than for peripheral joint involvement. Preliminary studies have shown that cyclosporine and other immunos…

Intraocular lesions in patients with systemic disease: findings on MR imaging.

Twenty-one intraocular lesions associated with various systemic diseases in 15 patients were studied by MR imaging. The disorders included diabetes mellitus, cardiovascular disease, Behçet disease, sarcoidosis, and ankylosing spondylitis. MR was performed on a 0.5-T system using a surface-coil technique. Ophthalmoscopic visualization of the fundus was precluded by the presence of opaque media in all cases. MR was found to be effective in demonstrating intraocular bleeding, vitreous opacity, detached lesions of the posterior pole, and eyeball deformity. Surface-coil MR is a useful adjunct in th…

[HLA system and collagen diseases (author's transl)].

Association of histocompatibility antigen (HLA antigens) with collagen diseases is not a casual event. Even known data, the practical conclusions have a limited usefullness because there is not a definitive correspondence between HLA antigens and rheumatic diseases. With HLA typing is possible to determine risk populations. The pauciarticular type II of juvenile reumatoid arthritis (J.R.A) is associated with HLA-B27 in the 75% of cases. Also, this antigen is associated with ankylosing spondylitis, Reiter's disease, Behçet disease, psoriasic arthritis, reactive arthritis (yersinia, shigella, sa…
