Platelet-like cells differentiated from adipose-derived mesenchymal stem cells inhibit acute inflammation of tendinopathy in rats.

INTRODUCTION: Tendinopathy, a disease that causes inflammation and pain and limits patients' activities of daily living, is considered particularly important to treat during the acute inflammatory phase to prevent the transition to chronic degeneration. Recently, platelet-rich plasma (PRP) has been used to treat tendinopathy; however, it is not clear whether platelets themselves, which are the active component of PRP, could be effective in treating tendinopathy. MATERIALS AND METHODS: We made rat Achilles tendinopathy models by incision of the calcaneal attachment and administrated platelet-l

Corticosteroid infiltration in partial distal biceps ruptures.

BACKGROUND: Little is known about the potential negative or positive effects of peritendinous infiltration with corticosteroids in the non-surgical treatment of partial distal biceps tendon tears. Peritendinous fluid, synovitis, and bursitis often accompany partial tears and can be a source of persistent pain. We hypothesize that peritendinous corticosteroid infiltration is a safe non-surgical treatment option for complaints related to a partial distal biceps tendon rupture. METHODS: A single-center retrospective analysis was performed on a cohort of 52 patients with partial distal biceps tea

Imaging Anatomy of the Ankle in Normal and Pathological States: A Clinically Focused Pictorial Review.

This narrative pictorial review presents the imaging anatomy of the ankle in normal and pathologic states and emphasizes practical, clinically oriented correlations. The ankle is a compound synovial joint complex in which osseous architecture, ligamentous systems (lateral collateral, deltoid, and syndesmotic), and tendon compartments interact within a compact space to provide stability and motion during gait and sport. This anatomical complexity underlies a broad spectrum of traumatic and degenerative conditions, including lateral ankle sprains, syndesmotic injury, osteochondral lesions of the

Repetitive Overuse Injury Causes Entheseal Damage and Palmar Muscle Fibrosis in Older Rats.

Overuse injury is a frequent diagnosis in occupational medicine and athletics. Using an established model of upper extremity overuse, we sought to characterize changes occurring in the forepaws and forelimbs of mature female rats (14-18 months of age). Thirty-three rats underwent a 4-week shaping period, before performing a high-repetition low-force (HRLF) task for 12 weeks, with the results being compared to 32 mature controls. HRLF animals showed a reduced grip strength versus controls. ELISAs carried out in the HRLF rats, versus controls, showed elevated levels of IL1-α in tendons, IL1-α an

Perifascial Areolar Tissue Transplantation for Covering Exposed Proximal Interphalangeal Joint After Electric Burn.

Soft tissue damage in electric burn injuries is often deep and may expose tendons, bones, or joints in the hand. Here, we present the case of a 76-year-old man treated using perifascial areolar tissue (PAT) transplantation for covering the proximal interphalangeal joint of the middle finger, which had been exposed because of an electric burn. Following ointment therapy, surgery was performed on day 34 postinjury when a deep ulcer with opening of the proximal interphalangeal joint was observed on the dorsum of the right middle finger. After resecting the cartilage of the proximal interphalangea

Biomechanics of the Acute Boutonniere Deformity.

PURPOSE: To demonstrate which structures of the extensor mechanism create a boutonniere deformity, when damaged, in a cadaver model. An analysis of how damage to these anatomical structures affects the biomechanical performance of the extensor mechanism was also performed. METHODS: We secured 18 fresh cadaveric hands onto an apparatus consisting of a computer-controlled motor and tensiometer attached in series to the extensor communis tendon of the ring and middle digits. The central slip, transverse, and oblique fibers of the interosseous hood and the triangular ligament were sequentially di

Subscapularis slide correction of the shoulder internal rotation contracture after brachial plexus birth injury: technique and outcomes.

Internal rotation contracture is the most common shoulder deformity in patients with brachial plexus birth injury. The purpose of this investigation is to describe the indications, technique, and results of the subscapularis slide procedure. The technique involves the release of the subscapularis muscle origin off the scapula, with preservation of anterior shoulder structures. A standard postoperative protocol is used in all patients and includes a modified shoulder spica with the shoulder held in 60 degrees of external rotation and 30 degrees of abduction, aggressive occupational and physical

Anterior knee pain.

Anterior knee pain is a common complain in all ages athletes. It may be caused by a large variety of injuries. There is a continuum of diagnoses and most of the disorders are closely related. Repeated minor trauma and overuse play an important role for the development of lesions in Hoffa's pad, extensor mechanism, lateral and medial restrain structures or cartilage surface, however usually an increase or change of activity is referred. Although the direct relation of cartilage lesions, especially chondral, and pain is a subject of debate these lesions may be responsible of early osteoarthrosis

The presence of smooth muscle actin in fibroblasts in the torn human rotator cuff.

The rotator cuff frequently sustains athletic and occupational injury, often resulting in chronic pain and disability. However, despite the high incidence of such shoulder problems, the pathophysiology of rotator cuff injury and healing has not yet been fully elucidated. The notable finding of this study was the presence of a contractile actin isoform, alpha-smooth muscle actin (SMA), in nonvascular cells in all of the seven torn human rotator cuff specimens evaluated immunohistochemically. Up to 95% of cells in any one region, and over 95% of elongated cells found in association with crimped

A novel arthroscopic suturing technique for rotator cuff tears: A retrospective clinical study.

To overcome the drawbacks of these conventional suturing methods, we have developed a novel suture technique. This new technique combines the advantages of the traditional Mason-Allen technique and the suture bridge technique, which enhances the tendon-to-bone contact area and prevents suture cutout, among other benefits. This study is to investigate the method, technique, and clinical efficacy of this novel suture technique in the treatment of rotator cuff injuries. Between January and December 2023, 21 patients (9 male, 12 female; age range: 46-71 years, mean: 51) underwent rotator cuff repai

A comparative analysis of autograft choices of anterior cruciate ligament reconstruction and their effects on muscle strength and joint biomechanics.

INTRODUCTION: Anterior cruciate ligament reconstruction (ACLR) is crucial to restore knee stability and function after ACL injuries, especially in physically active individuals. Despite advances in surgical techniques and rehabilitation protocols, the choice of autograft has a significant impact on postoperative recovery, particularly on muscle strength and joint biomechanics. In this study, the effects of four autografts are investigated: Iliotibial band (ITB), combined ITB and hamstring tendon (ITB + HT), hamstring tendon (HT) and bone-tendon-bone (BTB) on quadriceps and hamstring peak torqu

Arthrozheal®, a Bioactive Fibrin Scaffold for Joint Cartilage, Tendon and Soft Tissue Lesions. Latest Results and Application Perspectives.

Treatment of articular cartilage, tendon and soft tissue damage remains a challenge for the practicing orthopaedic surgeon. Due to the multifactorial aetiology of these lesions, there is a narrow therapeutic window within which they can be treated successfully, thus preventing progression to other musculoskeletal tissues. Recently, a new material that combines platelet-rich fibrin with collagen and is applied as a gel scaffold (ArthroZheal®, Vivostat A/S, Allerød, Denmark) has been shown to provide unique results in these patients. We arthroscopically treated 210 patients (114 knees, 32 hips,

Tendon regeneration and muscle hypotrophy after isolated Gracilis tendon harvesting - a pilot study.

PURPOSE: The gracilis tendon (GT) is a commonly used autologous graft in Orthopaedic surgery. The majority of information on knee function and outcomes after hamstring harvest is related to both semitendinosus and GT harvest. Little is known regarding isolated harvest of a GT. It was hypothesized that isolated GT harvest would lead to altered gait patterns (e.g. augmented anterior-posterior translation or rotation in the tibiofemoral joint) and consequently a higher prevalence of cartilage lesions and meniscal tears in knees. METHODS: GT harvesting was performed on patients with chronic acrom

Three-dimensional-construct bioreactor conditioning in human tendon tissue engineering.

Human tendon tissue engineering attempts to address the shortage of autologous tendon material arising from mutilating injuries and diseases of the hand and forearm. It is important to maximize the tissue-engineered construct's (TEC's) biomechanical properties to ensure that the construct is in its strongest possible state before reimplantation. In this study, we sought to determine the bioreactor treatment parameters that affect these properties. Using small- and large-chamber three-dimensional-construct bioreactors (SCB and LCB, respectively), we applied cyclic axial load to TECs comprising

Closed Intratendinous Rupture of the Flexor Digitorum Profundus: A Rare Cause of Pediatric Trigger Finger.

Paediatric trigger finger (PTF) is a rare condition, far less common than paediatric trigger thumb, and often associated with anatomical anomalies or systemic disease. Unlike trigger thumb, which may resolve spontaneously, PTF usually requires surgical treatment. Trauma is an uncommon cause, and closed intratendinous rupture of the flexor digitorum profundus (FDP) has not previously been reported in a child following direct injury. We present the case of a healthy 12-year-old boy who developed triggering of the right middle finger eight weeks after blunt trauma during rugby. Examination reveal

The course of the distal saphenous nerve: a cadaveric investigation and clinical implications.

INTRODUCTION: Injury to the saphenous nerve at the ankle has been described as a complication resulting from incision and dissection over the distal tibia and medial malleolus. However, the exact course and location of the distal saphenous nerve is not well described in the literature. The purpose of this study was to determine the distal limit of the saphenous nerve and its anatomic relationship to commonly identified orthopaedic landmarks and surgical incisions. METHODS: Sixteen cadaveric ankles were examined at the level of the distal tibia medial malleolus. An incision was made along the

Traumatic anterosuperior rotator cuff tears: the outcome of open surgical repair.

BACKGROUND: Anterosuperior rotator cuff tears involving the subscapularis and supraspinatus tendons are less common than posterosuperior tears and are rarely discussed in the literature. The purpose of this study was to identify the unique features of this injury and to assess the outcome of operative treatment. METHODS: Thirty consecutive patients, with a mean age of fifty-seven years (range, forty-three to seventy-three years), had an open repair of a traumatic anterosuperior rotator cuff tear. Twenty-four patients (80%) were male. Sixteen patients (53%) had involvement of the dominant shou

Wrist Arthroscopy Using the 2R Portal: Is It Safer Than the 1,2 Portal?

BACKGROUD: The purpose of this study was to compare the 1,2 with a novel 2R portal in terms of proximity to critical structures. METHODS: Wrist arthroscopy was performed on 8 fresh frozen cadavers via the 1,2 and 2R portals. External anatomy was then dissected under loupe magnification. The closest distance between the portals and surrounding anatomical structures was measured in millimeters using digital calipers. RESULTS: The 1,2 portal was significantly closer to radial artery and first extensor compartment tendons than the 2R portal. The radial artery was on average 1.32 mm from the 1-2

[Characterisation of microbial growth and role of the foot tendinous-synovial formation in pathogenesis of diabetic gangrene of the lower extremity].

The paper is based on the analysis of results of treatment of 74 patients with diabetes mellitus complicated by pyo-necrotic injuries of the foot. In all patients, microbiological procedures were done besides general clinical studies. The material for microbiological assays was the purulent exudation taken from the deeper portions of the wound immediately after lancing the pathological focus. The microbial content was studied separately in fragments of the tendon from its distal (adjacent to the wound) and proximal (every other 6 to 7 cm within the confines of the intact common integument) end
