Does surgical technique influence post-tonsillectomy haemorrhage? Our experience.

INTRODUCTION AND OBJECTIVES: Tonsillectomy represents one of the main surgical procedures for the otolaryngologist, with haemorrhage being the most common postoperative. The objective of this study was to determine the post-tonsillectomy haemorrhage rate, and relate the surgical technique, diagnosis and patient age. METHODS: This was a retrospective study, from April 2012 to January 2014, covering 429 patients. We used the following surgical-dissection techniques: cold, Colorado needle and monopolar forceps. Haemostasis was carried out in every case with monopolar forceps and gauze compressio

Explaining the poor bacteriologic eradication rate of single-dose ceftriaxone in group a streptococcal tonsillopharyngitis: a reverse engineering solution using pharmacodynamic modeling.

OBJECTIVE: To explore pharmacokinetic factors underlying the poor bacteriologic eradication rate with a single 500-mg dose of ceftriaxone for streptococcal tonsillopharyngitis and to identify the minimum ceftriaxone dose required for effective treatment. METHODS: Population modeling techniques were applied to pharmacokinetic data derived from paired plasma and tonsil samples from 153 children to assess the contribution of pharmacokinetic variability to patients' responses to ceftriaxone. In addition, a Monte Carlo simulation was performed to determine (1) the amount of time that free ceftriax

Improved somatic growth following adenoidectomy and tonsillectomy in young children. Possible pathogenetic mechanisms.

The effect of Adenotonsillectomy on somatic growth was studied prospectively in 57, randomly selected children (31 boys, 26 girls), aged 5.03+/-1.32 (mean+/-1SD) years. The indication for surgery was adenotonsillar hypertrophy with or without recurrent infections. Weight, height, triceps skinfold thickness, and Body Mass Index were measured prior to the operation and 6-13 months afterwards. Weight was significantly improved following T&A in all children. The improvement in height was significantly only for children under 5 years. In an attempt to uncover the pathogenetic mechanisms, lactic aci

Prolonged morbidity due to delays in the diagnosis and treatment of obstructive sleep apnea in children.

The inclusion of a query concerning the presence of snoring in a questionnaires used by the Allergy Service of Childrens Hospital Los Angeles (CHLA) uncovered a significant number of patients who were experiencing prolonged and discomforting symptoms owing to previously undiagnosed obstructive sleep apnea (OSA) caused by adenotonsillar hypertrophy. Of 352 patients who were discharged with a diagnosis of OSA and tonsillectomy and/or adenoidectomy at CHLA in 1996-1997, a retrospective study of the first 45 randomly selected patients who agreed to participate in a telephone interview was performe

Eosinophilic Granulomatosis With Polyangiitis in a Pediatric Patient With Bilateral Lacrimal Gland Swelling.

A 2-year-old male presented to ophthalmology following a short admission for dacryoadenitis, with a background of 6 months of bilateral fluctuating upper lid swelling, obstructive airway disease, and enlarged tonsils. He had been treated for an allergy-related disease with little effect and, post-hospital discharge, continued in a similar fashion as prior. Given the poor response to escalating allergy medication, a biopsy was taken from the right lacrimal gland. This demonstrated an eosinophil-rich small vessel vasculitis with extravascular eosinophilic granulomas, consistent with a diagnosis

Hairy Polyp Originating From the Lateral Nasal Wall of the Nasal Cavity.

Hairy polyps are benign lesions with limited growth potential. They usually occur in newborns, infants and young children, although occasional cases have been reported in adults, with a higher prevalence in females. Clinical symptoms include stridor, respiratory difficulty, snoring, and difficulty in oral intake. Histopathologically, hairy polyps consist of tissues derived from both mesodermal and ectodermal components. The nasopharynx and oropharynx are known to be the most common sites of origin, followed by less common sites such as the hard and soft palate, tonsils, tongue, auditory tube,

A case of sleep-disordered breathing presenting with increased intracranial pressure in a child with type 1 Chiari malformation.

UNLABELLED: Sleep-disordered breathing, with both central and obstructive sleep apneas, has been reported in association with Chiari malformation type 1. Chiari malformation type 1 is a congenital or acquired herniation of the cerebellar tonsils through the foramen magnum. In this case, a 5-year-old girl with a history of Chiari malformation type 1 and syringomyelia experienced worsening intracranial pressure secondary to sleep-disordered breathing. This case highlights the importance of early recognition of sleep-related respiratory disorders in patients with Chiari malformation and its assoc

IgG4-Related Disease With Larynx Involvement: A Case Report and Literature Review.

IgG4-related disease (IgG4-RD) is a systemic fibroinflammatory disorder with diverse organ involvement. Laryngeal involvement is exceptionally rare and easily misdiagnosed due to its non-specific symptoms and tumor-like appearance. We report a unique case of a 60-year-old woman who presented with progressive dyspnea and was ultimately diagnosed with laryngeal IgG4-RD after multiple biopsies. Imaging revealed additional asymptomatic lesions in the palatine tonsils, subcutaneous tissue, ribs, and lymph nodes. Histology confirmed dense IgG4-positive plasma cell infiltration in the laryngeal lesio

Risk of internal carotid injury during Peritonsillar abscess drainage in the emergency department.

Peritonsillar abscess (PTA) is a common neck infection that presents in the emergency department (ED). The treatment of PTA typically involves either incision and drainage or needle aspiration of the abscess, along with the administration of antibiotics. Patients with obvious PTA get needle aspiration in the ED. When the diagnosis is unclear, or there is a concern for a deeper extension of the abscess, accompanied by symptoms such as drooling and hoarseness, or to rule out other pathologies that mimic PTA, a Computed Tomography (CT) scan of the neck may be performed. Point-of-care ultrasound (

Tonsillar Kaposi's Sarcoma in HIV Positive Patient with Syphilis Infection.

INTRODUCTION: Since the introduction of Highly Active Anti-Retroviral Therapy (HAART), there has been a significant increase in the survival of HIV-infected patients. Consequently, there has been increased attention on the potential neoplastic pathologies, such as Kaposi's sarcoma, associated with AIDS in these individuals. CASE REPORT: In this case report we present, for the first time, a patient affected by Kaposi's sarcoma of the palatine tonsil with a concomitant syphilis infection. The patient underwent enlarged tonsillectomy and continued antiretroviral therapy. There were no signs of d

The clinical assessment of foreign body ingestions: Experience in a developing country.

Foreign body (FB) ingestion is an important emergency in low- to middle-income countries. Our study sought to identify which clinical and radiological findings best predict upper gastrointestinal FB impaction. Amongst 149 adolescents and adults presenting to two Jamaican hospitals between 2018 and 2020, FB's were found at 31% of telescopic laryngoscopies and 60% of rigid oesophagoscopies. Commonest sites of impaction were the tonsils at laryngoscopy (15/46) and proximal oesophagus at oesophagoscopy (17/22). Odds of pharyngeal impaction were higher with fish bone ingestion, persistent discomfor
