Neuroinvasive West Nile Virus Presenting as Subacute Progressive Quadriparesis and Intractable Pain: A Case Report.

West Nile virus (WNV) is the most common mosquito-borne infection in North America; while most cases are asymptomatic, fewer than 1% develop neuroinvasive disease with significant morbidity and mortality. We report a 57-year-old man from rural Wisconsin who presented with a 10-week history of progressive asymmetric quadriparesis and severe intractable pain, preceded by fatigue, shoulder pain, and paresthesias. Neurologic examination demonstrated mild encephalopathy, bulbar involvement, and mixed upper and lower motor neuron signs. MRI showed patchy thoracic cord T2 hyperintensities and diffuse

Therapeutic strategies to enhance proliferation in diabetic wound contraction: a comprehensive review from 1991-2025.

The global rise in diabetes mellitus has been accompanied by a marked increase in associated complications, most notably impaired wound healing. Diabetic non-healing wounds arise from multifactorial pathophysiological mechanisms, including excessive and prolonged inflammation, immune dysregulation, impaired angiogenesis, defective fibroblast-to-myofibroblast transition and aberrant growth factor signaling. These abnormalities delay wound contraction and re-epithelialization, increasing susceptibility to infection, chronic ulceration and limb amputation. Traditional and complementary approaches

The regulatory landscape in tuberculosis integrated research platforms.

BACKGROUND: Tuberculosis (TB) is the infectious disease with the highest mortality burden globally. Despite a recently enriched drug pipeline, there has been limited advance in improving its treatment. Integrated research platforms (IRPs) provide a novel research framework which, by hosting adaptive platform trials, can streamline the evaluation of several new TB regimens, whilst reducing costs. However, there is no clear, evidence-based guidance on the use of IRPs for regulatory drug development in TB. METHODS: This consensus statement summarizes the discussion and recommendations produced d

Understanding neuro-pulmonary dysfunction in children with neurodisability.

Children with neurodisability experience a high burden of respiratory morbidity, which remains a leading cause of hospitalisation, long-term lung disease and reduced quality of life. Respiratory problems in this population often reflect shared underlying neuro-pulmonary pathophysiology that evolves across a child's development. In this narrative review, we describe how respiratory vulnerability arises from three interacting domains: dysregulated central control of breathing; impaired neuromuscular activation and coordination; and altered respiratory mechanics related to abnormal tone, weakness

Evaluating Tinnitus in Children: A Comprehensive Review of Diagnostic Methods and Treatment Strategies.

Tinnitus in children represents a clinically significant condition with potential influence on auditory maturation, emotional stability, concentration, sleep quality, and academic performance. Limited symptom expression, developmental variability, and diverse underlying etiologies often delay recognition, creating the need for clearer diagnostic pathways. The objective of this narrative review is to consolidate contemporary understanding of diagnostic methods and treatment strategies applicable to pediatric tinnitus, with emphasis on developmental and neurocognitive factors that shape symptom

Silent Invasion of the Rectum: Aggressive Anorectal Melanoma With Pelvic Mass Effect, Venous Compression, and Hepatic Metastases.

Anorectal melanoma is a rare and aggressive malignancy arising from melanocytes within the mucosal epithelium of the anorectal region. Because presenting symptoms such as rectal bleeding frequently mimic benign anorectal conditions, diagnosis is often delayed until advanced stages of the disease. Mucosal melanoma is characterized by aggressive local invasion and early metastatic spread. We present the case of a 54-year-old female with metastatic rectal melanoma whose disease course was complicated by extensive pelvic tumor burden, malignant venous compression leading to deep venous thrombosis,

Small Bowel Ischemia Secondary to Splanchnic Venous Thrombosis in a Postpartum Patient-A Case Report.

BACKGROUND: Splanchnic vein thrombosis (SVT) is a rare but life-threatening condition. Pregnancy and the puerperium are hypercoagulable states that increase the risk of thrombosis, yet SVT remains an underrecognized cause of postpartum abdominal pain. CASE PRESENTATION: A 40-year-old Somali woman, four weeks postpartum after an uncomplicated vaginal delivery, presented with two weeks of progressive abdominal pain, distension, and anorexia. Physical examination revealed diffuse abdominal tenderness with voluntary guarding. Contrast-enhanced CT demonstrated extensive thrombosis of the superior

Ectopic ACTH syndrome in an adult with metastatic Ewing sarcoma.

Ectopic adrenocorticotropic hormone (ACTH) syndrome (EAS) accounts for 8-18% of Cushing syndrome cases and is typically due to neuroendocrine tumors such as small cell lung carcinoma and bronchopulmonary neuroendocrine tumors. EAS arising from Ewing sarcoma (EwS) is rare, with only 9 pediatric and 5 adult cases reported to date. We present the case of a 40-year-old woman with refractory, metastatic EwS who developed progressive swelling, fatigue, hypokalemia, profound proximal muscle weakness, and intermittent confusion over 2 months. Physical examination revealed facial plethora, supraclavicu

Economic analysis of using home mechanical ventilation for children with neuromuscular weakness: the case study from Thailand.

Neuromuscular Weakness (NMW) is a chronic condition often leading to respiratory failure and the need for long-term ventilatory support. While Home Mechanical Ventilation (HMV) is an alternative to hospital-based mechanical ventilation (MV), its economic impact remains unclear. This study evaluated the economic and clinical impact of switching from hospital-based MV to HMV for Thai children requiring long-term ventilation, based on SMA Type II. A Markov model was developed to estimate lifetime costs and quality-adjusted life years (QALYs) from a societal perspective. Survival transition probab

Midlife Vascular and Lifestyle Determinants of Late-Life Cognitive Decline and Dementia: A Life-Course Prevention Framework with a Gulf (GCC) Perspective.

Dementia is a growing global health challenge, yet many determinants of late-life cognitive decline emerge decades before symptoms appear. Midlife is a practical window for prevention because hypertension, diabetes, obesity, dyslipidemia, smoking, physical inactivity, unhealthy diet, sleep disturbance, and social isolation can be identified and modified before substantial brain injury becomes apparent. This narrative review synthesizes evidence linking midlife vascular and lifestyle exposures to late-life cognitive impairment and dementia. The most consistent data support a life-course model i

AI-assisted multimodal data integration for precision oncology.

The complexity of data pertaining to patients diagnosed with cancer requires a shift from fragmented, unimodal diagnostics towards multimodal artificial intelligence (MAI) in order to achieve true precision oncology. In this literature review we examined the landscape of unimodal data modalities used in oncological practice including clinical records, multi-scale imaging (radiology and histopathology), and multi-omics signatures, alongside a critical comparison of the deep-learning architectures and integrative frameworks used to combine these data sources into advanced predictive models. Aging beauty matters: Normative and resistant narratives on female aging bodies.

This article examines how older women navigate beauty work and anti-aging norms within contemporary Western aesthetic regimes. Rather than centering menopause as a biological transition, the study foregrounds beauty work as a key site where ageism and sexism intersect in shaping women's embodied experiences. Drawing on feminist sociology, cultural gerontology, embodiment theory, and scholarship on decline narratives, visibility and temporal governance, anti-aging norms are conceptualized as a form of gendered biopolitical regulation that frames visible aging as aesthetic and moral failure.Base

Postpartum as a site of sexual script renegotiation and intensification among women socialised within purity culture.

This interview study examined how postpartum women socialised within the Purity Culture Movement (PCM) in the USA navigated inherited sexual scripts during the transition to motherhood. Purity Culture, a religious system emphasising premarital abstinence, modesty, and sexual obligation within marriage, is widely associated with sexual shame and relational strain. Less is known about how women rework these beliefs after childbirth. Fifteen postpartum women were recruited from faith-informed maternal communities across the USA and screened to include those reporting either high or low sexual sat

Global prevalence of vasomotor symptoms in the menopause transition: systematic review and meta-analysis.

OBJECTIVE: Vasomotor symptoms (VMS) are common symptoms experienced during the menopausal transition. Previous reviews have highlighted regional heterogeneity but have not quantitatively pooled prevalence estimates. This study estimated pooled VMS prevalence across five global regions using a systematic review and meta-analysis. METHOD: MEDLINE, PsycINFO, CINAHL and Web of Science were searched from database inception to January 2025. Population-based studies reporting VMS prevalence among perimenopausal or postmenopausal women aged 40-65 years published from 2014 onwards (minimum sample size

The interembodiment of healing: Holistic transformations in neurological rehabilitation and care.

While disability literature has largely moved away from rehabilitation settings, critiquing its philosophy as reifying normative ableism, many communities still request accessible treatment options. Those with paralysis (on account of spinal cord injury and traumatic brain injury) are especially surrounded by constant messaging centered on hopes of a cure, with activity-based therapy, a relatively new therapy modality, being situated as providing maximum hope for patients and clients. This paper draws on 10 months of ethnographic research with paralysis patients, their caregivers, and rehabili

Severe luminal stricture in immune-related adverse event-associated enteritis: a case report.

Immune checkpoint inhibitors, such as ipilimumab and nivolumab, target cytotoxic T-lymphocyte-associated antigen 4 and programmed cell death protein 1, respectively, and are increasingly used in cancer treatment. These therapies can induce immune-related adverse events, including colitis and ileitis. However, the reports on enteritis caused by these immune-related adverse events are limited, particularly in Japan, with only a few case reports available. The incidence, typical location, and timing of onset of such inflammation remain undefined. We present the case of a 56-year-old male patient

The Impact of the Shift in Clinical Management From Reactive to Preventative on the Rate of Bowel Resection in Inflammatory Bowel Disease Within a Large Health Care Organization.

BackgroundThe advent of biologic therapy has been used as a surrogate marker for the shift from reactive to preventative management and surveillance for inflammatory bowel disease (IBD). This shift has resulted in earlier detection, earlier medical therapy, and increased bowel preservation in many published series.MethodsA retrospective population-based study was conducted in adult patients (age ≥18) with an IBD diagnosis using SCPMG Clinical & Administrative Database containing diagnostic, prescribing, and billing codes for IBD and IBD-related gastrointestinal surgery. Assessment of two different criteria for post-pancreatectomy acute pancreatitis: a single-center retrospective analysis and literature review.

BACKGROUND: The significance of postoperative acute pancreatitis (postoperative pancreatitis [POP] as defined by Connor and post-pancreatectomy acute pancreatitis [PPAP] as defined by the International Study Group for Pancreatic Surgery [ISGPS]) has not been determined because of the transition of definitions, a lack of prospective studies and ignorance regarding appropriate management. METHODS: The incidence of POP/PPAP were retrospectively analyzed. Additionally, the clinicopathological parameters and outcomes were compared between patients with and without POP/PPAP. RESULTS: Using Connor'

Healthcare utilization among Japanese older adults during later stage of prolonged pandemic.

This study examines healthcare utilization patterns among Japan's older population (aged 75 and above) during a prolonged public health emergency, focusing on the later phase of the COVID-19 pandemic (November 2021-September 2022). This period was characterized by the Omicron variant, widespread vaccination coverage, and adapted public health measures. Using a comprehensive dataset of 189,841,257 medical claims linked with income tax records, we analyze how healthcare utilization correlates with public health measures, pandemic severity, and income levels. Our findings reveal distinct utilizat
