Why is the concept of spondyloarthropathies important?

The concept of spondyloarthropathy was recognized first by clinicians based on the aggregation of several diseases occurring either sequenti … mmatory-bowel-disease-related arthritis and/or other clinical manifestations such as acute anterior uveitis. Recognition of the concept of the spondyloarthropathy is of great importance not only for research purposes but also in daily practice because such recognition has at least a threefold effect: (a) it permits earlier diagnosis, (b) it facilitates pa …

[A case of juvenile rheumatoid arthritis with MPO-ANCA associated nephritis].

A 12-year-old girl with a main complaint of sever pain on the both knees was admitted to our hospital in October, 1995. She gave a three year history of recurrent arthralgia and purpuric rashes, and persistent microhematuria and proteinuria. She developed vesicles and purpuric rashes on the hands and auricles, morning stiffness, fever, uveitis and pericarditis. Laboratory findings showed an elevated level of erythrocyte sedimentation rate and iron-deficiency anemia. Serum perinuclear pattern ANCA with antimyeloperioxidase specificity (MPO-ANCA) was positive. A renal biopsy specimen disclosed a…

Systemic toxicity from ocular homatropine.

Toxidromes are well known to emergency physicians. An unclear or incomplete history and subtle findings on physical examination make the diagnosis of poisonings challenging. This article reports a patient who had an acute onset of visual hallucinations, pressured speech, and mania. Although she denied taking any medications, she was ultimately diagnosed as having anticholinergic toxicity. On further questioning of family members, it was discovered that she was being treated for anterior uveitis with 5% homatropine. This case illustrates the potential role of ocular medications in systemic toxi…

Adenoma of the nonpigmented epithelium of the ciliary body.

A 56-year-old man presented with a left ciliary body mass, anterior vitreous hemorrhage and a subluxed, cataractous lens. The clinical course included rapid enlargement of the mass, anterior uveitis, cataract progression and secondary glaucoma. Investigation included fluorescein angiography, ultrasonography and computed tomography. The eye was enucleated because of progressive enlargement with poor vision and uncontrolled secondary glaucoma. Histopathological examination showed an adenoma of the nonpigmented ciliary epithelium with cystic areas of hyaluronidase-sensitive acid mucopolysaccharid…

[Neurological involvement as manifestation of Behçet's disease. 4 clinical case reports].

We report four patients with Behçet's disease characterized by initial and predominant neurological signs and symptoms. In three cases, a clinical picture of relapsing meningoencephalitis preceded the appearance of the classical signs of the disease for several months or years; in the fourth case, an acute febrile aseptic meningitis coincided with the development of bipolar aphthosis and uveitis. Disease activity was linked to a blood inflammatory syndrome and neutrophilic leucocytosis. Acute phases were associated with CSF mixed pleocytosis and high protein content. Brain CT scans and MRI wer…

[Behçet's disease: a study of 20 cases].

We observed 20 patients with ocular Behçet's disease. During the episodes of exacerbation the main features were: deep retinal infiltrates and vascular occlusions which were responsible for a sudden decrease in visual acuity. Also, there is posterior uveitis and retinal edema which persists during the whole course of the disease. In the final stages we observed an avascular, atrophic retina. Most patients also presented with a striking maculopathy and very often an atrophic optic disc was associated. On time of diagnosis, most patients were in their third decade, ranging from 19 to 61 years of…

Clinical manifestations in HLA-B27-positive patients.

The clinical manifestations of rheumatic diseases have been studied in a population of 440 HLA-B27-positive patients. Fifty-eight per cent had peripheral joint involvement. After statistical analysis and clustering of characteristics, three patterns of peripheral joint involvement were found. They were referred to as the X, Y and Z type for mnemotechnical reasons. The X type includes male patients with sacroiliitis, spondylitis, uveitis and uni- or bilateral arthritis of hip and/or shoulder. Y type patients present sacroiliitis, pauci-articular asymmetrical joint involvement and tendinitis. Z …

Clinical manifestations of secondary syphilis.

The results of a prospective study, aimed at having a fresh look at the clinical features of secondary syphilis in 89 patients, are presented. Eighty-one (91.0%) had syphilides, and of these, 24 (29.6%) had atypical morphology. Two or more groups of lymph nodes were enlarged in 60, and hepatosplenomegaly was seen in 20 (22.5%) patients. Condylomata data in atypical sites occurred in six patients. A total of 10 patients had alopecia on the scalp, and anterior uveitis was seen in 7 (7.9%). The clear CSF showed minimal elevation of lymphocytes in one of the 21 patients on whom lumbar puncture was…

Immunologically mediated ocular disease in the horse.

The continued study of immunology and its relationship to diseases of the eye will hopefully give some insight into the pathogenic mechanisms of certain ocular diseases of many species, including the horse. It may lead to a better understanding of equine recurrent uveitis, a disease that has remained an enigma for years and that now appears to be an immunologic hypersensitivity response to a number of varied antigens. The precise mechanism of the inflammation is still unclear, and the immunologic response may be variable or mixed depending upon the inciting antigen. Other ophthalmic diseases i…

As a chronic, focal, necrotizing granulomatous angiitis Wegener's granulomatosis mainly affects the upper respiratory tract, the lungs and kidneys. Various other organs may be affected including te eyes. Ocular manifestation of Wegener's granulomatosis may result from granulomatous sinusitis that has proceeded into the orbit causing nasolacrimal duct obstruction and proptosis, and affecting eye muscles as well as the optic nerve. Another possible picture is presented by focal vasculitis in the form of conjunctivitis, episcleritis, scleritis, uveitis and granulomatous vasculitis of the retina a…

Glaucoma in Port Moresby Papua New Guinea.

A retrospective study was carried out, of 48 patients with glaucoma, who presented over a 2 year period at Port Moresby General Hospital. No cases of chronic open angle glaucoma or acute angle closure glaucoma with shallow anterior chamber were found in the 37 Papua New Guinean patients. Trauma was the most common cause of glaucoma in this group particularly in males. Glaucoma also occurred in association with the exfoliative syndrome and secondary to anterior uveitis. All non-Papuan New Guineans retained some vision in both eyes while 23 of the Papua New Guineans had no perception of light in…

Pars plana vitrectomy. Microsurgical pars plana lensectomy.

Pars plana lensectomy, wherein the entire lens within the pupillary area (the anterior capsule, anterior cortex, nucleus, posterior cortex, and posterior capsule) is removed, is now a well-established technique. It is to be considered as a method of choice for the removal of presenile cataracts in patients under the age of 40, developmental cataracts, cataracta complicata secondary to uveitis, subluxated and luxated cataracts. Pars plana lensectomy of the senile cataract is limited to a grade 2 nuclear sclerosis on a scale of 4. The use of ultrasonic instrumentation via the pars plana in conju…

Systematic Review of Postvaccination Ocular Adverse Events: A Comprehensive Analysis of Published Reports.

Ocular adverse events following COVID-19 vaccination are well described; however, systematic analyses of non-COVID antiviral vaccines remain limited. This review aimed to evaluate ocular complications associated with non-COVID antiviral immunizations, including influenza, varicella-zoster (VZV), human papillomavirus (HPV), and hepatitis B (HBV) vaccines. A systematic search (PROSPERO CRD4202450873) identified 122 patients (184 eyes) from 8,487 publications, including case reports, case series, and observational studies. Uveitis was the most common (92/184 eyes; 50.0%, 95% CI 42.8%-57.2%), freq…

Ankylosing spondylitis: acute/subacute vs. chronic iridocyclitis - a bidirectional two-sample Mendelian randomization study.

BACKGROUND: Observational studies found associations between ankylosing spondylitis (AS) and iridocyclitis (IC), but the causality remained unconfirmed. METHODS: We employed two-sample Mendelian randomization (MR) to investigate the bidirectional causal relationships between AS and IC. Single-nucleotide polymorphisms (SNPs) were chosen from the FinnGen database's genome-wide association studies (GWAS) following a rigorous evaluation of the studies' quality. Sensitivity analysis was performed to assess the potential influence of pleiotropy and heterogeneity on the MR findings. RESULTS: Elevat
