The Hidden Cause of Maculopapular Rash in Interleukin-12 Deficiency.

Interleukin-12 RB1 (IL12RB1) deficiency falls under the Mendelian susceptibility to mycobacterial disease. It is a rare genetic disease with autosomal recessive inheritance. It is characterized by recurrent infections with otherwise weak bacteria, such as mycobacteria and Salmonella. Often, when encountering a maculopapular eruption, a drug-related cause comes to mind. However, we report a case of IL12RB1 deficiency presenting with a maculopapular eruption, proven by a skin biopsy to be leukocytoclastic vasculitis. The patient was given antibiotics, which improved her skin lesions. Vasculitis …

[Refractory form of orbital granulomatosis with polyangiitis: A clinical and morphological study].

Orbital granulomatosis with polyangiitis (Wegener's granulomatosis, GPA), which is characterized by granulomatous inflammation with small-vessel vasculitis, can develop in local and generalized forms of the disease. The introduction of current immunosuppressive therapy regimens has improved the prognosis of the disease; however, there are immunosuppressive treatment-refractory forms of GPA, the morphology of which has been inadequately investigated. The paper describes a clinical case of refractory GPA involving the orbit, as evidenced by histological and immunohistochemical examinations. The …

Henoch Schönlein Purpura Nephritis Associated with Intravesical Bacillus Calmette-Guerin (BCG) Therapy.

Henoch Schönlein purpura (HSP), also known as IgA vasculitis (IgAV), is a systemic small-vessel vasculitis that predominantly affects adolescents and is rare in adults. In many cases, the onset of HSP has been causally linked to an infectious disease. We encountered a case of HSP with severe renal involvement diagnosed by renal biopsy following bacillus Calmette-Guerin (BCG) therapy for bladder cancer. This is of clinical relevance, as intravesical BCG administration is becoming an established therapy for superficial bladder cancer and is supposed to be safe. It is important for all clinicians…

Henoch-Schönlein disease localized in the appendix.

A 16-y-old boy presented with complaints of acute abdomen. Ultrasonography showed an enlarged and tender appendix suggestive of acute appendicitis. The patient underwent surgery and during surgery the appendix was found with a red vasculitis-like distal end, not typical for acute appendicitis. Further per operative examination of the intestines revealed a circular, vasculitis-like red band in terminal ileum. Four days after surgery, the patient developed skin purpura and complained of joint pain classical of Henoch-Schönlein disease. Pathological evaluation of the appendix and a skin biopsy co…

Cryoglobulinemia manifested by gangraene of almost all fingers and toes.

Cryoglobulinemia is a rather rare condition accompanying quite a broad spectrum of different states and diseases. Mixed or polyclonal cryoglobulins can be seen in patients with autoimmune disorders, chronic infections and lymphoproliferative disorders. Monoclonal cryoglobulins are often revealed in patients with multiple myeloma or Waldenström's macroglobulinemia. Cryoglobulinemia is in most cases asymptomatic. Cryoglobulinemic vasculitis is an immune complex-mediated systemic disorder involving mostly small, but sometimes also larger vessels. In this report, we describe a case of a patient pr…

[Pulmonary fibrosis as a presentation of Wegener's granulomatosis].

INTRODUCTION: Pulmonary fibrosis secondary to antineutrophil cytoplasmic antibodypositive vasculitis is rarely reported. Most cases have occurred in association with microscopic polyangiitis. CASE REPORT: We report the case of an 82-year-old man presenting with pulmonary fibrosis. The CT scan showed honeycombing and traction bronchiectasis with peripheral and basal predominance. The diagnosis of Wegener's granulomatosis was based on the association of positive antineutrophil cytoplasmic antibodies, urinary sediment abnormalities and renal biopsy findings. CONCLUSION: We emphasize, through th…

A case of limited systemic sclerosis with p-ANCA, complicated by multiple cerebral hemorrhages.

ANCA associated vasculitides are a group of disorders characterized by the presence of necrotizing inflammation of blood vessels that involve venules, capillaries, arterioles, arteries, and veins. They are characterized by the presence of antineutrophil cytoplasmic antibodies (ANCA). ANCA associated vasculitis commonly affects the kidney producing proteinuria, hematuria and variable degrees of renal failure. Other organs involved include the lung, skin and peripheral nervous system. We present a patient with limited systemic sclerosis, who later developed pauci-immune glomerulonephritis with p…

Acute hemorrhagic edema of infancy: a case report.

A 12-month-old male presented with edematous purpuric plaques on the extremities (including the palms and soles), and violaceous targetoid macules on the hard palate and earlobes. The patient had an upper respiratory infection and conjunctivitis 10 days prior to the development of the skin lesions, and was vaccinated 3 weeks before this presentation. He was found to have increased IgA levels and increased ESR. Skin biopsies demonstrated leukocytoclastic vasculitis and IgA granular deposition on superficial dermal vessels. These findings are characteristic of acute hemorrhagic edema of infancy,…

Churg-strauss syndrome in an 8-year-old girl.

Churg-Strauss Syndrome (CSS) is a small vessel granulomatous vasculitis that characteristically affects the middle age group and is extremely rare in children. We describe an 8-year-old girl with a prolonged history of cough and eosinophilia that initially was labeled as pulmonary tuberculosis, but after complementary assays, associated with skin biopsy, eosinophilic fibrinoid vasculitis, CSS was eventually diagnosed. In this case, due to her young age and more prevalence of infectious lung diseases in our area, there was a few months delay for correct diagnosis and treatment. After meeting th…

The hamster as an animal model for eastern equine encephalitis--and its use in studies of virus entrance into the brain.

Eastern equine encephalitis virus (EEEV) produces the most severe human arboviral diseases in the United States, with mortality rates of 30%-70%. Vasculitis associated with microhemorrhages in the brain dominates the pathological picture in fatal human eastern equine encephalitis, and neuronal cell death is detectable during the late stage of the disease. We describe use of the golden hamster to study EEEV-induced acute vasculitis and encephalitis. In hamsters, EEEV replicates in visceral organs, produces viremia, and penetrates the brain. The pathological manifestations and antigen distributi…

A patient with wegener granulomatosis and intraventricular hemorrhage.

We report a case of intraventricular hemorrhage (IVH) in a 27-year-old man with Wegener granulomatosis (WG), successfully treated with corticosteroids and cyclophosphamide. Neurologic manifestations occur in 22%-54% of patients with WG, with cerebral vasculitis in less than 5%. Only 2 cases of IVH associated with WG have been reported at autopsy. This is the first reported case of in situ IVH associated with WG in a patient who survived. In patients with WG who present with neurologic manifestations, cerebral vasculitis with secondary intracranial hemorrhage should be considered, as these pati…

[Wegener's granulomatosis with meningeal involvement: clinic-radiological correlations].

INTRODUCTION: Wegener's granulomatosis is a systemic vasculitis which, in its classical form, is characterized by involvement of the superior and inferior respiratory tract and the kidneys. The vasculitis may be multisystemic. Ophthalmic and neurological involvement are common (22% and 54% of those affected respectively). When considering involvement of the nervous system, the commonest finding is peripheral neuropathy, particularly in the form of multiple mononeuritis. Meningeal involvement is exceptional. CLINICAL CASE AND CONCLUSIONS: We present a case of Wegener's granulomatosis with meni…

Gastrointestinal manifestations of Henoch-Schönlein purpura.

We report a case of the intestinal lesion in Henoch-Schönlein purpura, presented with an acute abdomen in a 4 year old boy. Five days after sudden colicky abdominal pain, skin purpura and painful joint swelling developed. These manifestations were associated with abdominal distension, hematemesis, hematochezia and hematuria. Exploratory laparotomy revealed a marked bowel distension with edema and patchy dark reddish discoloration of the jejunum and ileum. These patchy areas showed transmural hemorrhage and necrosis associated with characteristic leukocytoclastic vasculitis in and around the he…

Erythema elevatum diutinum: an ultrastructural case study.

An electron microscopic study was made of a patient with erythema elevatum diutinum, a rare variant of cutaneous leukocytoclastic vasculitis. Biopsies were obtained from early evolving and late fibroproliferative lesions. Early lesions showed vasculitis and a massive dermal infiltrate composed mainly of neutrophils, histiocytes/macrophages and apparently increased Langerhans cells. Later lesions were characterized by a dense fibrosis with a proliferation of fibroblasts and myofibroblasts and a dermal infiltrate in which lymphocytes and histiocytes/macrophages predominated. Even in later lesion…

[A case of Wegener's granulomatosis with involvement of the aortic valve].

A clinico-morphological observation of Wegener granulomatosis in a 29-year-old man is described. The disease started with the gastrointestinal disorders followed by lung and renal disturbances which dominated. Heart enlargement and systolic-diastolic noise in the Botkin point were found. Infectious endocarditis with a complex aortal defect were diagnosed. The patient died 6 months later. The autopsy revealed a necrotizing lung granulomatosis, disseminated vasculitis, renal and spleen infarcts. Vasculitis with granulomatosis, destructive changes in the intima were found in the supravalvular par…

Kawasaki syndrome.

Kawasaki's syndrome is an acute, largely self-limited multisystem vasculitis of childhood with prominent rheumatic complaints, involving both the heart and the joints. Although the etiologic agent has not been discovered, the evidence appears overwhelming that a microbial agent is the responsible trigger for this multisystem disease. Immunologically mediated phenomena appear to be important in the development of the significant complications of KS carditis, coronary vasculitis, and arthritis. Although truly effective therapy is not yet available, there is an exciting possibility that immunolog…

Granulomatosis with polyangiitis: clinical course and outcome of 60 patients from a single center in South India.

Granulomatosis with polyangiitis (GPA) previously known as Wegener's granulomatosis is one of the forms of idiopathic systemic vasculitis. There is very scanty data available on GPA in Asian and Indian population. We studied data of 60 patients from southern India, diagnosed with GPA to describe the physical characteristics, the treatment, and outcome. Patients who fulfilled any two of the four criteria proposed by the American College of Rheumatology, and those with clinical features of GPA with ANCA positivity and histopathological confirmation, were included in the study. Disease activity a…
