Prevalence and clinical significance of QRS fragmentation and low QRS voltages in elite female football players.

AIMS: QRS fragmentation (fQRS) and low QRS voltage are electrocardiogram (ECG) findings associated with cardiomyopathy and adverse cardiac events. However, their prevalence and significance in elite female athletes are unknown. The aim of this study was to determine the prevalence and clinical significance of fQRS and low QRS voltage in elite female footballers undergoing cardiovascular screening. METHODS AND RESULTS: Between April 2017 and October 2024, a cohort of 2566 consecutive elite female football players underwent cardiovascular screening, including a 12-lead ECG and echocardiogram. E

Seeing and sensing the heart: integrating non-coding RNA biomarkers with imaging in cardiovascular medicine.

Non-coding RNA (ncRNA), including microRNA, long non-coding RNA, and circular RNA, are epigenetic signals acting as upstream regulators in several cardiovascular disease processes. This review explores how ncRNA profiling complements non-invasive cardiac imaging modalities by providing biological insights into structural and functional phenotypes. In conditions such as hypertensive heart disease and aortic stenosis, ncRNAs like microRNA (miR)-29 and miR-155 correlate with left ventricular hypertrophy and fibrosis. In dilated cardiomyopathy and heart failure, circulating miR-150-5p, miR-21, and

Sympatho-Excitation in Pulmonary Hypertension: The Potential Role of Pulmonary Arterial Baroreceptors: An Acute Physiologic Intervention Study.

BACKGROUND AND OBJECTIVE: Sympathetic nerve activity is heightened in pulmonary arterial hypertension (PAH), promotes deleterious pulmonary vascular and right ventricular remodelling, and is associated with worsened exercise capacity and mortality. Pulmonary arterial baroreceptors, activated by increased pulmonary arterial pressure and distension, could drive muscle sympathetic nerve activity (MSNA) in pulmonary hypertension (PH). We hypothesised that pulmonary arterial baroreceptor unloading using nebulised iloprost as a pulmonary arterial vasodilator would lower MSNA in clinically stable tre

Connexin43, A Promising Target to Reduce Cardiac Arrhythmia Burden in Pulmonary Arterial Hypertension.

While essential hypertension (HTN) is very prevalent, pulmonary arterial hypertension (PAH) is very rare in the general population. However, due to progressive heart failure, prognoses and survival rates are much worse in PAH. Patients with PAH are at a higher risk of developing supraventricular arrhythmias and malignant ventricular arrhythmias. The latter underlie sudden cardiac death regardless of the mechanical cardiac dysfunction. Systemic chronic inflammation and oxidative stress are causal factors that increase the risk of the occurrence of cardiac arrhythmias in hypertension. These stre

Improvement in left atrial strain following ferric carboxymaltose in heart failure: an analysis of the Myocardial-IRON trial.

AIMS: Iron deficiency (ID) is associated with an impaired cardiac function and remodelling in heart failure (HF). Treatment with ferric carboxymaltose (FCM) has been showed recently to improve biventricular systolic function and ventricular strain parameters in patients with HF with reduced ejection fraction and ID, but there is no evidence on the benefit of FCM on the left atrium (LA). In this study, we aimed to evaluate the effect of FCM on LA longitudinal strain (LA-LS). METHODS AND RESULTS: This is a post hoc subanalysis of a double-blind, placebo-controlled, randomized clinical trial tha

Mitochondrial complex I dysfunction and altered NAD(P)H kinetics in rat myocardium in cardiac right ventricular hypertrophy and failure.

AIMS: In cardiac hypertrophy (CH) and heart failure (HF), alterations occur in mitochondrial enzyme content and activities but the origin and implications of these changes for mitochondrial function need to be resolved. METHODS AND RESULTS: Right ventricular CH or HF was induced by monocrotaline injection, which causes pulmonary artery hypertension, in rats. Results were compared with saline injection (CON). NAD(P)H and FAD autofluorescence were recorded in thin intact cardiac trabeculae during transitions in stimulation frequency, to assess mitochondrial complex I and complex II function, re

Clinical implications of nonsarcomeric gene polymorphisms in hypertrophic cardiomyopathy.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is characterized by cardiomyocyte hypertrophy and fibrosis. Although is an autosomal dominant trait, a group of nonsarcomeric genes have been postulated as modifiers of the phenotypic heterogeneity. MATERIAL AND METHODS: We prospectively recruited 168 HCM patients and 136 healthy controls from three referral centres. Patients and controls were clinically stable at entry in the study. Nine polymorphisms previously associated with ventricular remodelling were determined: I/D ACE, AGTR1(A1666C), CYP11B2(C344T), PGC1-α(G482S), COLIA1(G2046T), ADRB1(R3

A randomized double-blind control study of early intra-coronary autologous bone marrow cell infusion in acute myocardial infarction: the REGENERATE-AMI clinical trial†.

AIMS: Clinical trials suggest that intracoronary delivery of autologous bone marrow-derived cells (BMCs) 1-7 days post-acute myocardial infarction (AMI) may improve left ventricular (LV) function. Earlier time points have not been evaluated. We sought to determine the effect of intracoronary autologous BMC on LV function when delivered within 24 h of successful reperfusion therapy. METHODS AND RESULTS: A multi-centre phase II randomized, double-blind, and placebo-controlled trial. One hundred patients with anterior AMI and significant regional wall motion abnormality were randomized to receiv

Comparison of the rhythm control treatment strategy versus the rate control strategy in patients with permanent or long-standing persistent atrial fibrillation and heart failure treated with cardiac resynchronization therapy - a pilot study of Cardiac Resynchronization in Atrial Fibrillation Trial (Pilot-CRAfT): study protocol for a randomized controlled trial.

BACKGROUND: The only subgroups of patients with heart failure and atrial fibrillation in which the efficacy of cardiac resynchronization therapy has been scientifically proven are patients with indications for right ventricular pacing and patients after atrioventricular junction ablation. However it is unlikely that atrioventricular junction ablation would be a standard procedure in the majority of the heart failure patients with cardiac resynchronization therapy and concomitant atrial fibrillation due to the irreversible character of the procedure and a spontaneous sinus rhythm resumption tha

Role of 5-HT 2B receptors in cardiomyocyte apoptosis in noradrenaline-induced cardiomyopathy in rats.

1. Serotonin (5-hydroxytryptamine; 5-HT) plays important roles in the development of cardiac hypertrophy via activation of 5-HT receptors. The aim of the present study was to investigate the role of 5-HT(2B) receptors in the development of cardiomyocyte apoptosis and hypertrophy associated with noradrenaline (NA) overload. 2. Cardiac hypertrophy was induced in rats by intraperitoneal injection of 1.5 mg/kg NA for 4 weeks. Starting from Day 15, 5-HT2B receptor antagonist SB 204741 (i.p., 0.5 or 2 mg/kg) or SDZ SER 082 (i.p., 1 mg/kg) was injected twice daily for another 14 days. Whole-cell patc

The cardiovascular system and high-altitude exposure: from adaptation to disease. Part I.

Exposure to high altitude constitutes a complex physiological challenge, driven primarily by hypobaric hypoxia and, in some settings, by extreme environmental conditions such as severe cold and increased ultraviolet radiation. At the cardiovascular level, this exposure elicits a range of acute and chronic adaptations, including activation of the sympathetic nervous system, pulmonary vasoconstriction, and vascular and ventricular remodelling. In the Andean region, millions of people live at elevations above 2,500 metres above sea level, rendering them particularly susceptible to these physiolog

Acute versus chronic mitral regurgitation: a case of perioperative cardiogenic shock in an athlete.

Mitral regurgitation (MR) poses challenges in distinguishing acute from chronic aetiologies in compensated patients. A 48-year-old male rugby player presented with 2 weeks of worsening dyspnoea on a background of chronic bilateral lower limb cellulitis. Found in atrial fibrillation, transoesophageal echocardiography revealed flail P2 with severe MR, ejection fraction 35%, and severe left ventricular dilation-indicating chronicity despite acute symptoms. He underwent urgent mitral valve repair. Post-induction instability necessitated emergency cardiopulmonary bypass. Repair consisted of P2 tria

Mechanism and use strategy of uric acid-lowering drugs on coronary heart disease.

Coronary heart disease (CHD) is a serious cardiovascular illness, for which an elevated uric acid (UA) level presents as a considerable risk factor. This can be treated with UA-lowering drugs such as allopurinol and benzbromarone, which can reduce UA levels by the inhibition of UA production or by promoting its excretion. Such drugs can also be beneficial to CHD in other ways, such as reducing the degree of coronary arteriosclerosis, improving myocardial blood supply and alleviating ventricular remodeling. Different UA-lowering drugs are used in different ways: allopurinol is preferred as a si…

The authors therefore discuss how multimodality imaging can help to evaluate the left ventricular remodeling response and improve risk stratification in patients with moderate AS. Finally, they summarize current evidence on the management of moderate AS and highlight ongoing trials on AVR in moderate AS.

Mineralocorticoid receptor antagonists for heart failure with reduced ejection fraction: integrating evidence into clinical practice.

Mineralocorticoid receptor antagonists (MRAs) improve survival and reduce morbidity in patients with heart failure, reduced ejection fraction (HF-REF), and mild-to-severe symptoms, and in patients with left ventricular systolic dysfunction and heart failure after acute myocardial infarction. These clinical benefits are observed in addition to those of angiotensin converting enzyme inhibitors or angiotensin receptor blockers and beta-blockers. The morbidity and mortality benefits of MRAs may be mediated by several proposed actions, including antifibrotic mechanisms that slow heart failure progr

Canine tissue-specific expression of multiple small leucine rich proteoglycans.

Small leucine rich proteoglycans (SLRPs) are important constituents of extracellular matrix (ECM) and contribute to the production, organization and remodelling of collagen and elastin through complex biological systems. The relative expression and distribution of SLRPs in a variety of different mammalian tissues is poorly characterized. The aim of this study was to map the expression of seven SLRPs (biglycan, versican, prolargin, fibromodulin, osteoglycin, decorin and lumican) in seven tissues (bone, cartilage, cruciate ligament, skin, ventricular myocardium, mitral valve and cornea) in young

BRCA1 is an essential regulator of heart function and survival following myocardial infarction.

The tumour suppressor BRCA1 is mutated in familial breast and ovarian cancer but its role in protecting other tissues from DNA damage has not been explored. Here we show a new role for BRCA1 as a gatekeeper of cardiac function and survival. In mice, loss of BRCA1 in cardiomyocytes results in adverse cardiac remodelling, poor ventricular function and higher mortality in response to ischaemic or genotoxic stress. Mechanistically, loss of cardiomyocyte BRCA1 results in impaired DNA double-strand break repair and activated p53-mediated pro-apoptotic signalling culminating in increased cardiomyocyt

Association of Immunoglobulin E With Right Ventricular Dysfunction in Pulmonary Arterial Hypertension.

Pulmonary arterial hypertension (PAH) is characterized by pulmonary vasoconstriction and vascular remodeling, leading to increased pulmonary vascular resistance, pathological right ventricular (RV) remodeling, and ultimately right heart failure and death. RV function and right ventricle-pulmonary artery (RV-PA) coupling are critical determinants of patient prognosis, and echocardiography remains the preferred tool for assessing RV function and predicting RV-PA coupling. Recent studies have revealed that immunoglobulin E(IgE) plays a regulatory role in cardiac and pulmonary vascular remodeling;

Effects of high-dose supplemental oxygen therapy on pulmonary haemodynamics at 3250 m in high-altitude residents at risk for pulmonary hypertension.

QUESTION ADDRESSED BY THE STUDY: Hypoxic pulmonary vasoconstriction increases systolic pulmonary artery pressure (sPAP) and may contribute to high-altitude pulmonary hypertension (HAPH). While supplemental oxygen therapy (SOT) lowers sPAP during acute hypoxic exposure, its haemodynamic effects in permanent high-altitude residents remain unclear. In particular, the relative contribution of fixed vascular remodelling to chronically elevated sPAP is uncertain. PARTICIPANTS AND METHODS: In this open-label, non-randomised, sequential interventional trial, Kyrgyz highlanders living >2500 m with tri
