Screw fixation of an OCD loose body: 21-year results.

Osteochondritis dissecans (OCD) lesions can progress to loose body formation for which treatment is controversial and may involve excision or fixation. There is a paucity of published data regarding long-term outcomes following OCD loose body fixation. This case report presents an interval follow-up of a patient from a previous small case series of individuals who underwent open reduction internal fixation of large, lateral OCD loose bodies. At 21-year follow-up the patient has full, pain-free range of motion, and has not required subsequent surgery. This case, to our knowledge, is the first t

[The study of pedicle imaging and reformation with the multi-slice spiral CT in the surgical treatment of lumbar spondylolisthesis].

OBJECTIVE: To evaluate the value of pedicle parameter obtained by the reformation images on multi-slice spiral CT (MSCT) in the surgical treatment of lumbar spondylolisthesis. METHODS: From January 2009 to March 2010, 60 patients with lumbar spondylolisthesis failing in conservative treatment were enrolled into the study and divided into experimental and control group randomly (each group with 30 patients). There were 26 males and 34 females ranging in age from 18 to 59 years with an average of (42.60 +/- 9.36) years. The experimental group was examined with volumetric scanning on MSCT before

Computer navigation-assisted spinal fusion with segmental pedicle screw instrumentation for scoliosis with Rett syndrome: a case report.

Scoliosis is a common clinical manifestation of Rett syndrome, a neurodevelopmental disorder that almost exclusively affects females. The spinal curve in patients with Rett syndrome is typically a long C curve of a neuromuscular type. As the onset of the scoliosis is very early and shows rapid progression, early surgical intervention has been recommended to prevent a life-threatening collapsing spine syndrome. However, there are high perioperative risks in Rett syndrome patients who undergo spinal surgery, such as neurological compromise and respiratory dysfunction due to rigid spinal curve. W

Open-wedge high tibial osteotomy: a technical trick to avoid loss of reduction of the opposite cortex.

High tibial osteotomies for the treatment of gonarthrosis in young and active patients are well-accepted and frequently performed procedures. Several techniques are described to achieve good axial alignment with enough stability for early motion, but they come along with a considerable number of complications, such as insufficient or excessive axial correction, loss of reduction, delayed union, and pseudoarthrosis. We present a case of early loss of reduction in a medial opening-wedge high tibial osteotomy and a simple technical trick to avoid and treat this potential complication.

Dynamic stabilization devices in the treatment of low back pain.

Soft stabilization has an important role in the treatment of the degenerative lumbar spine. Fusion of one or two motion segments may not make a big difference in the total range of motion of the lumbar spine, but preserving flexibility of a motion segment may prevent adjacent segment disease and may permit disc replacement, even when facet joints need to be excised. If a favorable environment is created in the motion segment by unloading the disc and permitting near normal motion, the disc may be able to repair itself or may supplement the reparative potential of gene therapy. Although soft st

Posterolateral rotatory instability of the elbow.

Posterolateral rotatory instability (PLRI) of the elbow is recurrent elbow instability caused by injury to the radial ulnohumeral ligament or lateral ulnar collateral ligament. Deficiencies of the radial ulnohumeral ligament and laxity of the lateral capsule allow the proximal radioulnar joint to rotate and the radial head to subluxate posteriorly when stressed. Whether isolated or in conjunction with other injuries, PLRI represents the initial stage in the pathology of the unstable elbow. The diagnosis of PLRI can be difficult because the provocative tests are challenging to perform. Diagnost

Atypical slipped capital femoral epiphysis after radiotherapy and chemotherapy.

Atypical slipped capital femoral epiphysis after radiotherapy and chemotherapy is uncommon. Only 32 cases have been reported in the literature. Because patients may have slippage at atypical ages, we report two cases of slipped capital femoral epiphysis in children and review the 32 cases previously reported to heighten clinicians' awareness of this condition in patients who have received radiation and chemotherapy for pelvic tumors. The controversy over prophylactic pinning of the uninvolved hip in radiotherapy-associated slipped capital femoral epiphysis is unresolved. It may be justifiable

Spontaneous femoral neck fracture complicating a healed subtrochanteric fracture--a case report.

A rare case of spontaneous femoral neck fracture complicating a healed subtrochanteric fracture was encountered in an 85-year-old female. The subtrochanteric fracture was caused by a minor trauma with no other combined injury. Patient received close reduction and internal fixation two days later and the fracture healed in three months. Six months after operation, spontaneous ispilateral femoral neck fracture occurred without trauma. Although there were some reports about subcapital fracture or femur neck fracture complication after operation, this is the first case of spontaneous femoral neck

Surgical correction of carpal valgus deformity in three alpacas.

Two alpaca crias and one 14-month-old alpaca were referred for treatment of bilateral carpal valgus. In one cria, hemi-circumferential periosteal transection and elevation combined with an ulna osteotomy was performed initially without success, however transphyseal bridging with screws and wires was used successfully in both crias to straighten the limbs. As the distal radial and ulna growth plates had closed in the 14-month-old alpaca, bilateral wedge ostectomies of the distal radius and transfixation casts were used to straighten and stabilise the limbs. The ostectomy sites healed and the al

[Quantitative 3D anatomy of the lumbar spine].

PURPOSE OF THE STUDY: There are many anatomic descriptions of the spine, but most concentrate on qualitative information. Quantitative data is however important to achieve a better clinical approach, to adapt implant size and to construct geometric models of spine mechanics. MATERIAL AND METHODS: We examined 32 dry spines (160 lumbar vertebrae) obtained from the Orfila Museum anatomy laboratory at the Saints-Pères School of Medicine in Paris. We drew 178 landmarks on the surface of each vertebra and recorded the 3D spatial coordinates of each point using a Fastrack electromagnetic system oper

Biomechanical analysis of hindfoot fixation using an intramedullary rod.

To measure the stiffness of the hindfoot when fixed with an intramedullary rod placed in a retrograde manner, two biomechanical experiments were carried out on five matched pairs of cadaveric below knee specimens. Experiment 1: In the right leg of each pair, an intramedullary rod was placed in a retrograde fashion through the calcaneus, talus, and into the tibial intramedullary canal. Biomechanical testing was done to determine hindfoot stiffness, with and without distal and proximal transverse interlocking screws. The uninstrumented left leg of each pair was tested as a control. Experiment 2:

A method for open reduction and internal fixation of the unstable posterior sternoclavicular joint dislocation.

Posterior sternoclavicular joint (SCJ) dislocations are most often stable after reduction but may be associated with significant complications related to the location of the medial head of the clavicle within the mediastinum. In rare instances, a posterior SCJ dislocation is irreducible or redislocates after a closed reduction. Because of the potential hazards related to compression of vital structures within the superior mediastinum, open reduction and internal fixation is usually required. Although open reduction is widely accepted as the method of choice, the best method for achieving stabl

Arthroscopic intraarticular interference screw technique of posterior cruciate ligament reconstruction: one-incision technique.

Posterior cruciate ligament (PCL) reconstruction using a bone-patellar tendon-bone graft with double incisions has been described in the literature. We present the single-incision technique that may aid the surgeon in performing difficult PCL reconstructions and reduce the possibility of injuring the knee extensor mechanism. By using three proper portals, we have shown that it is possible to make a femoral tunnel and engage a bone peg of the graft in the tunnel without any extensor damage and without disturbing the divergence angle of the bony tunnels. We have performed 22 such operations with

[Transjugal osteosynthesis by compression miniscrew in sagittal ramus osteotomy].

The transjugal osteosynthesis technique of the Obwegeser-Dalpont type is used for treatment by sagittal osteotomy of ascending rami. The material used is a direct compression mini-screw derived from that described by Michelet-Champy. Principles underlying all types of osteosynthesis (rigidity, solidity, miniaturization, according to the authors) are discussed and advantages and inconveniences of their material compared with those of other existing procedures. Two clinical cases are presented as an illustration.

A retrospective study of prosthetic capsule technique for the treatment of problem cases of dislocated hips.

This article describes a technique for stabilizing coxofemoral luxations in dogs that uses placement of nonabsorbable sutures from two screws in the dorsal acetabular rim to a screw in the trochanteric fossa of the femur to create a prosthetic capsule. Twenty-one hips with luxations that were either recurrent or greater than 7 days duration were treated. Owners of 17 dogs were contacted from 4 to 40 months after surgery. Eight dogs were reported as having excellent function, three had good function, three had fair function, and three had poor function. No complications directly related to the
