Benzylamine-related compounds stimulate rat vas deferens neurotransmission and potentiate memory in the mouse acting as potassium channel blockers.

In stimulated rat vas deferens, the new compound 2, 6-dibutylbenzylamine (B25) and some related benzylamines, first potentiated then completely inhibited electrically-induced twitch response, showing the biphasic effect previously observed in unstimulated preparations. To verify if this effect could be referred to as a modulation of potassium channels the activity of some benzylamines, KCl, tetraetylammonium (TEA), BaCl(2), 4-aminopyridine (4-AP), glibenclamide (GLI), charibdotoxin (ChTX) and apamin (APA) has been compared. While KCl and benzylamine-related derivatives induced biphasic effects

Uncontrolled hypertension in siblings: an unsuspected diagnosis.

Hypertension in the young is defined as the occurrence of elevated blood pressure in patients aged <40 years. It is unusual for hypertension to be present before the second decade of life and more so in a pair of siblings. This study aimed to report a case of a sibling pair (18- and 16-year-olds), reared as females, who presented with primary amenorrhea and resistant hypertension. The initial presentation was hypertension. The siblings were treated with escalating doses of antihypertensive agents, including calcium channel blockers, beta-blockers, and diuretics, with suboptimal control. Primar…

Hypertension in children is increasing over the years, prevalence being 4% worldwide with peak at adolescence. While secondary causes predom … ificant prognostic markers of cardiovascular morbidity. While first-line anti-hypertensives include calcium channel blockers, angiotensin-converting-enzyme inhibitors (ACE-I), angiotensin receptor blockers (ARB), and thiazide diuretics, specific management and targeted antihypertensives may be required for the underlying cause in secondary hypertension. For acute severe h …

From Peptides to Aliskiren: The Evolution of Direct Renin Inhibitors in Hypertension Therapy.

Hypertension remains a major global health concern, significantly contributing to the burden of cardiovascular, renal, and cerebrovascular diseases. Conventional pharmacological interventions include calcium channel blockers, beta-blockers, diuretics, Angiotensin-Converting Enzyme Inhibitors (ACEIs), and Angiotensin Receptor (AR) antagonists. However, these therapies often present limitations, particularly in patients with comorbid conditions such as diabetes, where end-organ damage persists despite Renin-Angiotensin-Aldosterone System (RAAS) blockade. A promising alternative lies in targeting…

Blood pressure in haemodialysis: measurement, volume control and drug selection.

Hypertension in haemodialysis is common, prognostically important, and difficult to manage because blood pressure measurement is often impre … ecomes necessary. Beta-blockers have the strongest outcome-linked support in haemodialysis, whereas calcium channel blockers (CCBs) are practical complementary agents, and conventional renin-angiotensin system (RAS) blockade is best reserved for selected indications. Mineralocorticoid receptor antagonists (MRAs) should be considered cautiously because recent large dialysi …

Frequently involved drugs encompass diuretics, β-blockers, calcium channel blockers, ACE inhibitors, antidepressants, and antiparkinsonian therapies. These medications, within the framework of age-associated physiological alterations, elevate the risk of orthostatic hypotension. The diagnosis relies on monitoring blood pressure dur …

Skin Phototoxicity With Aminolevulinic Acid Hydrochloride in Patients With Non-Muscle Invasive Bladder Cancer: Real-World Incidence and Concomitant Medication Effects.

OBJECTIVES: The study quantifies the real-world incidence of skin phototoxicity associated with oral 5-aminolevulinic acid hydrochloride during photodynamic diagnosis-assisted transurethral resection of bladder tumor. Furthermore, we assessed whether the Japanese regulatory revision in 2022 on the concomitant use with photosensitizing medications modified the risk. METHODS: We retrospectively analyzed 462 procedures performed at Toranomon Hospital between 2020 and 2024. The clinical data included demographics, laboratory results, medication history, and adverse reactions. Skin reactions were

Factors associated with medication adherence among young adults with hypertension.

BACKGROUND: Poor adherence to antihypertensive medication remains a significant barrier to blood pressure control in young patients. The objective of this study was to identify factors associated with antihypertensive medication adherence among young adults with hypertension. METHODS: From the Korean National Health Insurance Service database, we included 141,132 participants aged 20 to 39 years (80.4% male), without cardiovascular disease, who initiated antihypertensive medication between 2013 and 2018. Participants were categorized as exhibiting good adherence (proportion of days covered [P

Outcomes of a multidisciplinary approach to management of mavacamten in obstructive hypertrophic cardiomyopathy.

PURPOSE: Traditional treatments for obstructive hypertrophic cardiomyopathy (oHCM) include β-blockers, calcium channel blockers, and disopyramide. Mavacamten, a novel cardiac myosin inhibitor, is a promising oHCM therapy but has practical challenges limiting its use. This descriptive study aimed to describe a clinic workflow for mavacamten management in a real-world setting, addressing challenges such as cost, drug interactions, and monitoring requirements. The focus was on reducing patient-level costs while ensuring feasibility and efficiency. SUMMARY: A retrospective analysis was conducted …

Efficacy of pharmacological therapies in reducing outflow tract obstruction in patients with obstructive hypertrophic cardiomyopathy: a systematic review and meta-analysis.

AIMS: Significant advancements have been made in the management of obstructive hypertrophic cardiomyopathy (oHCM), yet the extent of left ventricular outflow tract (LVOT) gradient reduction achieved with commonly used pharmacological therapies [beta-blockers (BBs), calcium channel blockers (CCBs), disopyramide, and cardiac myosin inhibitors (CMIs)] relative to each other is still unclear. METHODS AND RESULTS: PubMed and Scopus were searched up to September 2024. Clinical trials or observational studies that assessed the changes associated with BBs, CCBs, disopyramide, or CMIs in LVOT gradient…

Management of hypertension in heart transplant recipients: an ongoing conundrum.

PURPOSE OF REVIEW: Hypertension remains one of the most common clinical problems leading to significant posttransplant complications. This study reviews the pathophysiology of hypertension in the postcardiac transplant phase and provides an update on currently available antihypertensive therapies for heart transplant patients. RECENT FINDINGS: The true prevalence of hypertension in the heart transplant population remains unknown. Effective blood pressure (BP) control is key to prevent left ventricular remodeling, diastolic dysfunction and stroke. Calcium channel blockers (CCBs) are the most c…
