Role of preoperative Hounsfield units in predicting subsidence after anterior cervical discectomy and fusion in the United States: a retrospective analysis including osteopenia diagnosis.

STUDY DESIGN: A retrospective cohort study. PURPOSE: This study aimed to investigate the relationship among osteopenia, bone density, and subsidence following anterior cervical discectomy and fusion (ACDF). OVERVIEW OF LITERATURE: Subsidence following ACDF procedures can lead to worse clinical outcomes. Although studies have linked osteopenia to cage subsidence, no consensus has been established on the relationship between bone density and subsidence. METHODS: Patients undergoing ACDF between 2016 and 2021 were included and assigned to the osteopenia cohort based on chart review and dual-en

Machine learning models for predicting dysphonia following anterior cervical discectomy and fusion: a Swedish Registry Study.

BACKGROUND: Dysphonia is one of the more common complications following anterior cervical discectomy and fusion (ACDF). ACDF is the gold standard for treating degenerative cervical spine disorders, and identifying high-risk patients is therefore crucial. PURPOSE: This study aimed to evaluate different machine learning models to predict persistent dysphonia after ACDF. STUDY DESIGN: A retrospective review of the nationwide Swedish spine registry (Swespine). PATIENT SAMPLE: All adults in the Swespine registry who underwent elective ACDF between 2006 and 2020. OUTCOME MEASURES: The primary ou

Spinal epidural haematoma after blunt trauma to the neck and hyperflection of the cervical spine.

Spinal epidural haematomas (sEDH) can be regarded as rare events, in principle a spontaneous and a traumatic aetiology can be distinguished. Spontaneous spinal epidural haematomas can arise, e.g. from vascular malformations, coagulopathies, etc. On the other hand, traumatic sEDH are related to, e.g. spinal trauma or intraoperative vascular injuries. With regard to clinical significance, spinal epidural haematomas accompanied by transient mild neurological symptoms up to lethal outcomes have been observed. We report on a 53-year-old male alcoholic who was found in the kitchen of his asylum in a

Somatosympathetic reflexes from the low back in the anesthetized cat.

In the appendicular skeleton, substantial evidence demonstrates that somatosensory input from deep tissues including limb muscles and joints … 11-L5 had been cut (n = 4) or when the spinal cord had been transected between the second and third cervical vertebrae (n = 6). The results indicate that inflammatory stimulation of multifidus muscle in the low back evokes a somatosympathetic reflex integrated supraspinally in the upper cervical spinal cord or higher. The reflex's afferent arm travels in the medial …

Three-dimensional analysis of per-rotational nystagmus.

BACKGROUND: Head rotation produces a vestibulo-ocular reflex (VOR). In horizontal rotation, not only lateral semicircular canals but also posterior semicircular canals are stimulated, because posterior canals cupulae are not horizontal in the sitting position. Therefore, theoretical nystagmus is horizontal and torsional. Convection of endolymph does not occur, because the centre of head rotation is a dens of the second cervical vertebra, not the center of lateral canal. Although per-rotational nystagmus is a result of VOR, whether it could be explained by the movement of cupula remains undeter

Sublaminar Band Fixation Provides Excellent Anchors for MAGEC Rod Distraction Systems.

MAGEC rods (NuVasive) provide distraction growth in early-onset scoliosis. Pedicle screw use with MAGEC rods can lead to anchor failure. Sublaminar bands offer superior fixation points for the MAGEC system while preserving pedicles and facets, avoiding spinal cord injury, and eliminating the need for fluoroscopy. Sublaminar bands can be safely used up to cervical vertebra four (C4), substantially decreasing the risk of complications such as anchor pull-out, rod breakage, and proximal junctional kyphosis that typically occurs with pedicle screws and hooks. This case demonstrates the viable opti

Ponticulus posticus is a frequent radiographic finding on lateral cephalograms in nevoid basal cell carcinoma syndrome (Gorlin-Goltz syndrome).

UNLABELLED: Nevoid basal cell carcinoma syndrome (NBCCS) is a predisposition to a rare tumor type with a variable phenotype. Besides tumors, skeletal alterations, such as bifid ribs or frontal bossing constitute the phenotype. Recently, a variant of the first cervical vertebra, the ponticulus posticus, was reported to occur in 50% of patients with NBCCS as revealed by analysis of lateral cephalograms. MATERIALS AND METHODS: Lateral cephalograms of eight patients with NBCCS were studied for the presence of ponticulus posticus. RESULTS: The ponticulus posticus was present in all patients. In o

An unusual case of IgE-multiple myeloma presenting with systemic amyloidosis 2 years after cervical plasmacytoma resection.

A 69-year-old man suffered from a fractured cervical vertebra, and magnetic resonance imaging revealed a solitary mass occupying the injured lesion. Surgical resection of the mass was conducted, and the infiltration of plasma cells expressing IgE-lambda monoclonal protein was shown on pathological investigation. Concurrently, a monoclonal band of IgE-lambda was shown in the sera on immunoelectrophoresis, and the proliferation of plasma cells (more than 10%) was also detected in bone marrow aspirates. A rare IgE-multiple myeloma (MM) was diagnosed, and careful monthly follow-up was started. Dur
