The role of personalized medicine in identifying appropriate candidates for menopausal estrogen therapy.

Menopausal estrogen therapy has a complex balance of benefits and risks and is no longer routinely recommended for the majority of women during or after the transition to menopause. Recent findings from the Women's Health Initiative (WHI) and other studies suggest that a woman's clinical and biological characteristics may modify her health outcomes on hormone therapy (HT) and that some women may be more appropriate candidates for therapy than others. An emerging body of evidence suggests that it may be possible to identify women who are more likely to have favorable outcomes and less likely to

Preference elicitation tool for abnormal uterine bleeding treatment: a randomized controlled trial.

BACKGROUND: It is estimated that one-third of women will experience abnormal menstrual bleeding. The majority of these cases are not due to cancer or pregnancy complications and, as a result, women are faced with a variety of treatment alternatives, the selection of which is largely dependent on personal preferences for care rather than clinical outcomes. OBJECTIVE: This randomized trial was designed to evaluate a preference elicitation tool to promote physician-patient collaborative decision making for treatment of abnormal uterine bleeding (AUB). METHODS: Adaptive conjoint analysis (ACA) w

From hopeless to good prognosis: journey of a failing tooth.

Chronic periodontitis, along with associated clinical findings such as pathologic tooth migration, diastema, functional and aesthetic aberrations, poses an immense challenge to a dental professional. These findings convert clinical decision making into a daunting task and adversely affect the prognosis and the treatment plan for the presenting clinical problem. An interdisciplinary approach aimed at restoring functional and aesthetic needs of the affected individual within the limitations of such a compromised clinical scenario may be a viable alternative to any radical treatment causing loss

A treatment decision-making model for infraoccluded primary molars.

The objective of this paper is to clarify the literature's findings and provide guidelines for decision-making during the long-term treatment planning of infraoccluded primary molars. This paper presents a synopsis of findings concerning prevalence, aetiology, diagnostic methods, diagnostic criteria and treatment options. Treatment decisions are mainly guided by the clinical assessment of the presence or absence of succedaneous tooth, evaluation of onset, time of diagnosis, resorption rate, rate of progression of infraocclusion, risk of adverse effects over time, and predictive clinical patter

Effects of A Novel Decision Aid for Breast Reconstruction: A Randomized Prospective Trial.

INTRODUCTION: The choice to undergo mastectomy and breast reconstruction is a highly personal decision with profound psychosocial effects, and ultimately, the decision between implant- and autologous tissue-based reconstruction should be made based on a combination of factual information and the patient's personal values and preferences. Unfortunately, patients undergoing breast reconstruction surgery may experience decision regret. Decision aids promote patient involvement in decision making by not only providing standard information about options, but also emphasizing comparative risks, bene

Hereditary ovarian cancer risk reduction: a retrospective evaluation of patient perspectives and service provision at a regional hereditary gynaecologic cancer clinic 2006-2016.

BACKGROUND: Germline pathogenic variants in BRCA1/2 have been established in hereditary breast and ovarian cancer (HBOC) syndrome and result in significantly elevated lifetime risk of ovarian cancer. Risk reduction interventions are presently the only effective means of improving survival and specialized counselling clinics have been established as an effective means of aiding this population in navigating complex decisions surrounding these interventions. This study sought to evaluate patient perceptions of a specialized counselling clinic for patients with HBOC Syndrome and referral patterns

Epilepsy surgery in children with a normal MRI.

Determining candidacy for epilepsy surgery in children with intractable seizures where the MRI is negative is a common challenge facing epilepsy specialists. In recent years there has been an increasing array of investigative approaches that helps to formulate the hypothesis for the epileptogenic zone, with excellent seizure outcomes in some epilepsy surgery series. This narrative review will examine the approach to MRI negative epilepsy surgery in children, with a focus on the need for early recognition of potential surgical candidates, and the preoperative work-up including clinical features

In a nutshell: Testing DOAC levels-when, why and what it means?

Plasma direct oral anticoagulant (DOAC) levels are not routinely required for the majority of patients; however, they may provide valuable information in urgent clinical situations, including perioperative management, life- or limb-threatening bleeding and patients requiring thrombolysis, when measurement mostly aims to determine whether a clinically significant concentration of DOAC is present in plasma. In non-urgent indications, including investigation of DOAC failure and in patients with extremes of body weight or drug-drug interactions, levels may assist clinical decision-making by determ…

The value and limitations of the Oxford Community Stroke Project classification in the hyperacute phase of cerebral infarction.

Originating from a large-scale community-based screening program in the United Kingdom, the Oxford Community Stroke Project (OCSP) classification has become a pivotal tool for categorizing ischemic stroke and guiding clinical decision-making since its inception. In the context of reperfusion therapy for hyperacute cerebral infarction, the integration of the OCSP classification with clinical functional assessment and neuroimaging enables clinicians to rapidly identify stroke subtypes, tailor recanalization strategies, and prognosticate associated complications. This review summarizes the applic…

Point-of-Care Ultrasound as a Key Diagnostic Tool in the Emergency Detection of Severe Pericardial Effusion.

Point-of-care ultrasound (POCUS) has become an essential extension of the physical examination in the emergency department, enabling rapid bedside assessment and timely clinical decision-making. We report a case of a 58-year-old female presenting with progressive dyspnea initially suggestive of a pulmonary etiology. Chest radiography revealed a right pleural effusion and apparent cardiomegaly. Bedside POCUS promptly identified a large circumferential pericardial effusion with echocardiographic signs of early cardiac tamponade, leading to immediate cardiology referral, intensive care admission,…
