Evaluation of BCNU and/or radiotherapy in the treatment of anaplastic gliomas. A cooperative clinical trial.

A controlled, prospective, randomized study evaluated the use of 1,3-bis(2-chloroethyl)-1-nitrosourea (BCNU) and/or radiotherapy in the treatment of patients who were operated on and had histological confirmation of anaplastic glioma. A total of 303 patients were randomized into this study, of whom 222 (73%) were within the Valid Study Group (VSG), having met the protocol criteria of neuropathology, corticosteroid control, and therapeutic approach. Patients were divided into four random groups, and received BCNU (80 mg/sq m/day on 3 successive days every 6 to 8 weeks), and/or radiotherapy (500

Cromolyn effects on vernal keratoconjunctivitis in children.

A double-masked, coded trial was undertaken to evaluate the effect of the topical administration of 2% cromolyn sodium eye drops on vernal keratoconjunctivitis in 14 children. One eye of each patient was treated for one year with cromoly, the other eye with placebo. Cromolyn reduced the characteristic vernal keratitis, vernal corneal ulcers and plaques, and limbal edema and infiltrates but did not affect the number or size of the giant papillae. The drug's long-term topical use did not have any adverse side effects.

Cyclandelate in the treatment of senile mental changes: a double-blind evaluation.

The treatment of cerebrovascular insufficiency and its many symptoms in the ever-increasing numbers of the aged, is of major concern to physicians engaged in such care. Despite past skepticism as to the degree of efficacy of cerebral vasodilators, there is renewed interest in this form of therapy. Our investigation was designed to assess the effectiveness of cyclandelate, under strict double-blind conditions, in 58 geriatric patients. The cyclandelate and placebo groups (32 and 26 patients respectively) received either 1,600 mg/day of cyclandelate in fractional doses, or identical-appearing pl

Retraining gait in Parkinson's Disease via a personalised app: A study protocol.

Parkinson's disease (PD) is a progressive neurological disorder that often leads to gait impairments and an increased risk of falls, negatively impacting quality of life. Inertial-based wearables have extended gait evaluation beyond clinical settings, but their technical integration and general accessibility hinder widespread adoption. Smartphones, equipped with inertial sensors, present a scalable approach for gait assessment and retraining/rehabilitation. Auditory cueing, specifically music, has shown promise to retrain gait, yet its use and uptake is often limited by a lack of personalisati

Reviewing inclusivity of the UK bladder and head and neck oncology trial portfolio through eligibility criteria: a scoping review.

BACKGROUND: In clinical trials, eligibility criteria define a population who may benefit from the intervention without incurring undue safety risks. Despite their value, overly stringent eligibility criteria may pose barriers to inclusive recruitment. They may disproportionately exclude groups of people more likely to be diagnosed at later cancer stages, or have specific co-morbidities which can be associated with socio-demographic or ethnic background. The National Institute for Health and Care Research Innovations in Clinical Trial Design and Delivery for the Under-served (NIHR-INCLUDE) guid

Population Pharmacokinetics of Topical Timolol Maleate Gel in Healthy Volunteers and Infants With Superficial Infantile Hemangioma.

Although timolol ophthalmic solutions or hospital-compounded formulations have been used off-label for the treatment of superficial infantile hemangioma (IH), no topical timolol formulation has been developed specifically for this indication. This study was the first to develop a population pharmacokinetic (popPK) model for topical timolol maleate gel and to perform exploratory exposure-safety analyses. Data from Phase I-III clinical trials in healthy adults and infants with proliferative superficial IH were used to develop the popPK model. Potential covariate effects on the pharmacokinetics w

Hydroxyurea pharmacokinetics in children with sickle cell anemia across different global populations.

Hydroxyurea provides effective disease-modifying treatment for people with sickle cell anemia (SCA), especially when escalated to maximum tolerated dose (MTD), which has wide interpatient dosing variability due to pharmacokinetic (PK) differences. Whether hydroxyurea PK parameters differ among children with SCA in different global regions is unknown. We compared hydroxyurea PK parameters among children with SCA from 5 clinical trials: HUSTLE (United States), TREAT (United States), NOHARM (Uganda), REACH (Uganda and Kenya), and EXTEND (Jamaica). Key hydroxyurea PK parameters were determined usi

Core outcome domain sets for clinical trials in epidermolysis bullosa - a COSEB protocol to achieve consensus on "what" to measure.

BACKGROUND: Epidermolysis bullosa (EB) comprises a heterogeneous group of rare, genetic blistering diseases. The wide variety in EB trial outcomes limits the comparability of outcomes and, consequently, the implementation of the best available treatment options. A core outcome set (COS) is a minimum set of outcomes that should be measured in all clinical trials, comprising what should be measured (i.e., outcome domains) and how it should be measured (i.e., outcome measurement instruments). This enables standardization of outcome measurement aiming at improving the comparability and quality of

The role of stakeholder mapping and engagement in Mongolia during the implementation of the STREAM clinical trial for MDR-TB.

BACKGROUND: Clinical trials evaluating new regimens for multidrug-resistant tuberculosis (MDR-TB) are typically conducted in multiple countries because global registration of new TB drugs requires evaluation in diverse populations. The complexity of multi-site trials makes implementation challenging, especially in lower-resource settings, where the burden of MDR-TB is highest. Stakeholder engagement can improve trial implementation and outcomes. Here, we describe the Mongolia site's stakeholder engagement during STREAM, a phase III clinical trial evaluating novel treatment regimens for MDR-TB.

Global biological sample collections from tuberculosis studies: a scoping review.

Progress in tuberculosis vaccine development is hindered by the incomplete understanding of protective immunity and other disease mechanisms. An interconnected network of sample biorepositories from tuberculosis studies could help to address these gaps. To assess the feasibility of such a resource, we conducted a scoping review of tuberculosis observational studies and vaccine clinical trials. The included studies collected at least one biological sample from tuberculosis cases, contacts, or controls and had more than 100 participants. We contacted the corresponding authors of these studies to
