Telehealth-delivered cognitive rehabilitation for people with cognitive impairment as part of the post-COVID syndrome: protocol for a randomised controlled trial as part of the CICERO (Cognitive Impairment in Long COVID: Phenotyping and Rehabilitation) study.

BACKGROUND: Between 25 and 75% of people with persistent post-acute sequelae of SARS-CoV-2 infection (PASC) experience cognitive difficulties, compromising functional ability, quality of life, and activities of daily living, including work. Despite this significant morbidity, there is a paucity of interventions for this disorder that have undergone evaluation within a formal trial setting. Therefore, we have developed a cognitive rehabilitation programme, specifically designed to address the cognitive symptoms of PASC, notably impaired attention and processing speed, while also accounting for

A Combined Speech-Language and Counseling Intervention for Primary Progressive Aphasia: Acceptability, Feasibility, and Preliminary Outcomes.

PURPOSE: Individuals with primary progressive aphasia (PPA) face negative psychosocial repercussions, yet few studies have investigated counseling for this population. In a prior study, a novel intervention encompassing script training and aphasia-modified cognitive behavioral therapy was piloted with an individual with PPA, yielding promising results. The purpose of this study is to determine the acceptability and feasibility of this intervention for individuals with different PPA subtypes, with secondary aims of understanding preliminary treatment outcomes and evaluating the sensitivity of s…

Evidence-based stroke r-ehabilitation: an expanded guidance document from the european stroke organisation (ESO) guidelines for management of ischaemic stroke and transient ischaemic attack 2008.

OBJECTIVE: Despite recent high-profile advances in our understanding of rehabilitation post-stroke, the evidence base remains weaker than in other areas of stroke management. Under the aegis of the European Stroke Organisation a select committee was assembled to collate and appraise the evidence base for rehabilitation interventions. METHODS: Following systematic literature searching, relevant abstracts were screened for data quality and relevance. These data were summarized and presented to the members of the expert panel, who, both individually and across group discussions, modified the con

[Possible conservative treatment methods for patients with spinal claudication].

The success of multimodal therapy for patients with symptoms of spinal canal stenosis lasting >6 months was investigated in a prospective study. Clinical (age, weight, height) and morphological data were collected for 36 patients (21 women and 15 men) with an average age of 67 years. The extent of narrowing in the spinal space (diameter of the dural sac <100 mm(2)) was measured by magnetic resonance imaging at segments L1-S1. Impairments in daily activities were assessed by questionnaire before a 3-week multimodal treatment regimen and 3 weeks and 6 months thereafter. In addition, depression s

Future horizons: Toward an integrated neuroscience of sleep.

Sleep is now recognized as a vital biological process with roles beyond rest, including neurodevelopment, memory, emotional regulation, metabolism, immune function, and neuroprotection. Advances in neuroscience and technology reveal that sleep is an active, multifaceted process involving multiple brain and body systems. This chapter explores how neural networks, waste clearance, circadian rhythms, immune responses, and cognition interact to influence brain health and disease. We examine sleep's connections to neurodegeneration, mental illness, aging, and cognitive resilience. We also highlight

Mercury exposure leading to functional vitamin B12 deficiency and subacute combined degeneration: a case report and literature review.

INTRODUCTION: The association between neurological symptomatology and heavy metal exposure has been reported in the literature. A few cases of extrapyramidal symptomatology and subacute combined degeneration have been described as manifestations of mercury intoxication. We highlight a case of a patient presenting with Parkinsonian features (tremor, rigidity, and bradykinesia), pyramidal deficits, dysarthria, paresthesia, mild cognitive decline, and emotional lability, with proven elevated mercury levels in blood and hair and elevated arsenic in urine. CASE: A 60-year-old man, with history of

Treatment of Oropharyngeal Dysphagia.

Substantial ignorance and controversy surround the management of neuromyogenic pharyngeal dysphagia. The major obstacle to confident recommendations is the lack of high-level evidence supporting most available therapies. There is reasonable evidence supporting cricopharyngeal disruption (myotomy or dilatation) in primary restrictive disorders of this region, but low-level evidence supporting myotomy in pure neuromyogenic dysphagia. There is reasonable evidence supporting current dietary recommendations as aspiration-minimization strategies. There is only low-level evidence, albeit consistent,

Fetal alcohol spectrum disorders and neuroimmune changes.

The behavioral consequences of fetal alcohol spectrum disorders (FASD) are serious and persist throughout life. The causative mechanisms underlying FASD are poorly understood. However, much has been learned about FASD from human structural and functional studies as well as from animal models, which have provided a greater understanding of the mechanisms underlying FASD. Using animal models of FASD, it has been recently discovered that ethanol induces neuroimmune activation in the developing brain. The resulting microglial activation, production of proinflammatory molecules, and alteration in e

Variations in dietary iron alter behavior in developing rats.

Iron deficiency in children is associated with retardation in growth and cognitive development, and the effects on cognition may be irreversible, even with treatment. Excessive iron has also been associated with neurological disease, especially in reference to the increased iron content in the brains of Alzheimer's disease and Parkinson's disease patients. This study evaluated the effects of dietary iron deficiency and excess iron on physical activity in rats. The animal model used is developmentally sensitive and permits control of the timing as well as the duration of the nutritional insult.

Kaixinsan, a Well-Known Chinese Herbal Prescription, for Alzheimer's Disease and Depression: A Preclinical Systematic Review.

Alzheimer's disease (AD), the most common cause of dementia, is highly prevalent worldwide with no modifying therapy. Behavioral and psychological symptoms of dementia (BPSD) occur in most patients with AD, and depression is one of the most common AD-related BPSD. Kaixinsan (KXS) is an ancient Chinese herbal prescription widely used to treat dementia and forgetfulness. In this systematic review, we conducted a meta-analysis to assess preclinical evidence for the effects of KXS on cognitive impairment and depression. Thirty-eight articles involving 1,050 animals were included after searching fr

Study Protocol: The Coaching Alternative Parenting Strategies (CAPS) Study of Parent-Child Interaction Therapy in Child Welfare Families.

BACKGROUND: Child maltreatment (CM) constitutes a serious public health problem in the United States with parents implicated in a majority of physical abuse and neglect cases. Parent-Child Interaction Therapy (PCIT) is an intensive intervention for CM families that uses innovative "bug-in-ear" coaching to improve parenting and child outcomes, and reduce CM recidivism; however, the mechanisms underlying its effects are little understood. The Coaching Alternative Parenting Strategies (CAPS) study aims to clarify the behavioral, neural, and physiological mechanisms of action in PCIT that support

Intra-arterial Administration of Human Umbilical Cord Blood Derived Cells Inversed Learning Asymmetry Resulting From Focal Brain Injury in Rat.

Background: Focal brain injury is a leading cause of serious disability significantly worsening patients' quality of life. Such damage disrupts the existing circuits, leads to motor, and cognitive impairments as well as results in a functional asymmetry. To date, there is still no therapy to effectively restore the lost functions. We examined the effectiveness of human umbilical cord blood (HUCB)-derived cells after their intra-arterial infusion following focal stroke-like brain damage. Methods: The model of stroke was performed using ouabain stereotactic injection into the right dorsolateral

Efficacy of neck-specific exercises with and without internet-based support on psychological factors in chronic whiplash-associated disorders: secondary analyses of a randomized controlled trial.

OBJECTIVE: Chronic whiplash-associated disorders (WAD) can involve complex interactions between physical symptoms and psychological factors, and long-term effects of exercise programs are of interest. This study aimed to evaluate the effects of a predominantly internet-based neck-specific exercise program (NSEIT) compared with the same neck-specific exercises (NSE) performed at a physiotherapy clinic on psychological outcomes in chronic WAD, including cognitive-behavioral factors (pain catastrophizing, fear-avoidance beliefs, self-efficacy), emotional distress (symptoms of depression, anxiety)

Improving Social Functioning in Youth at Clinical High Risk for Psychotic Disorders With a Virtual Reality Based Cognitive Behavioural Therapy: The Feasibility and Acceptability of REVIVE.

INTRODUCTION: Adolescence is when mental disorders such as psychosis frequently emerge, with long-term impacts on social functioning if left untreated. Early intervention, including cognitive behavioural therapy (CBT) and innovative approaches like virtual reality (VR)-assisted therapy, can help young people in their social recovery. METHOD: During the development phase of REVIVE therapy, input was gathered through group consultations with 10 youth advisors aged 16-25 who had lived experience. Their insights informed the design of the virtual social environments. A feasibility and acceptabili

Impact of psychological treatment in freezing of gait: A pilot study.

UNLABELLED: One common gait issue associated with advanced Parkinson's disease (PD) is freezing of gait (FoG). FoG impacts approximately half of people with PD and negatively impacts quality of life. Studies have suggested that anxiety may contribute to FoG. Individuals with FoG exhibit higher rates of anxiety, but it is unclear if anxiety contributes to FoG development. No known studies have assessed the impact of treating anxiety to improve FoG symptoms. OBJECTIVE: This pilot study evaluated the efficacy of cognitive behavioral therapy (CBT) training in reducing anxiety and improving gait i…

Efficacy of a Mobile Multidisciplinary Digital Therapeutics App for Patellofemoral Pain: Randomized Controlled Trial.

BACKGROUND: Patellofemoral pain (PFP) is a common musculoskeletal disorder characterized by persistent knee pain, often without any structural abnormalities. Conservative treatments, particularly exercise therapy, are widely recommended; however, adherence remains generally low, and full recovery is often not achieved. Psychological interventions can aid in symptom management; however, studies integrating cognitive behavioral therapy (CBT), which is known to be effective for chronic pain, with exercise therapy for patients with PFP are limited. This study examined the impact of MORA Cure (PFP)…

Single versus combination treatment in tinnitus: an international, multicentre, parallel-arm, superiority, randomised controlled trial.

Tinnitus is defined as the conscious awareness of a tonal or composite noise in the absence of a corresponding external acoustic source. This international multicentre, parallel-arm, superiority, randomised controlled trial investigated whether combination therapies are superior to single interventions in the treatment of chronic subjective tinnitus. Tinnitus patients were recruited from five clinical sites across the EU and randomly assigned using a web-based system, stratified by their hearing and distress level, to single or combination treatment of 12 weeks. Cognitive-behavioural therapy,
