Cytoimmunofluorescence Diagnosis in Surgically Induced Pemphigus Foliaceus.

Pemphigus foliaceus is an autoimmune bullous disease with autoantibodies against desmoglein 1. Case reports of pemphigus after surgery have also been described, which may simulate an infection of the surgical wound, a contact dermatitis, or even a tumor recurrence. Cytoimmunofluorescence can help to establish a rapid diagnosis.

Topical nitrofurazone, a potent sensitizer of the skin and mucosae.

Although freely available, topical nitrofurazone (Furacin) frequently causes severe contact dermatitis, necessitating hospitalization. Six patients with sensitivity reactions to topical nitrofurazone are described. Our experience suggests that the free availability of this drug may increase the incidence of reactions due to its use, and should be condemned.

The Langerhans cells in healed patch tests reactions, before and after oral administration of nickel.

Langerhans cells (LC) were studied at the light and electron microscopic levels in normal skin and in positive patch test areas after long-term healing before and after the flare-up reaction induced by oral administration of nickel. A number of reactive changes indicating increased cell activity, similar to those found in positive patch tests 72 hours after allergen application, were registered in the test areas. Thus, strong reactive changes persist in the LC system after a fulminant contact allergic dermatitis, even beyond clinical healing. Some cellular reactions, especially pronounced afte

A Practical Approach to Chronic Hand Eczema.

Chronic hand eczema (CHE) is a multifactorial condition with significant physical, psychological, and socioeconomic burdens. Its complex pathogenesis often does not align with clinical presentation, leading to overlapping etiologic subtypes, including allergic contact dermatitis (ACD), irritant contact dermatitis (ICD), and atopic hand eczema (AHE). While patch testing remains the gold standard for confirming ACD, its clinical utility can be limited owing to frequent subtype overlap, impracticality of allergen avoidance, and practical considerations relating to the process and availability of

Tacrolimus 0.1% vs mometasone furoate topical treatment in allergic contact hand eczema: a prospective randomized clinical study.

Tacrolimus is a macrolide immunosuppressant that has been demonstrated to inhibit T-lymphocyte activation without the side-effects of corticosteroids. The safety profile of tacrolimus makes it a promising therapeutic option for dermatitis. To evaluate and compare the therapeutic ability of tacrolimus 0.1% ointment and mometasone furoate 0.1% ointment in patients with chronic hand eczema and positive patch tests. Thirty adults with chronic hand eczema and positive patch test reaction to relevant contact allergens were treated with tacrolimus 0.1% ointment or mometasone furoate 0.1% ointment in

Hand eczema: an update.

Eczema, the commonest disorders afflicting the hands, is also the commonest occupational skin disease (OSD). In the dermatology outpatient departments, only the severe cases are diagnosed since patients rarely report with early hand dermatitis. Mild forms are picked up only during occupational screening. Hand eczema (HE) can evolve into a chronic condition with persistent disease even after avoiding contact with the incriminated allergen / irritant. The important risk factors for hand eczema are atopy (especially the presence of dermatitis), wet work, and contact allergy. The higher prevalence

Contact allergy caused by fragrance mix and Myroxylon pereirae (balsam of Peru)--a retrospective study.

Because of their widespread use, fragrances are among the most common causes of contact allergic dermatitis, second only to nickel. During a five-year period 3,065 patients with contact dermatitis were patch tested using a specific mix of fragrances. 509 (16.6%) patients were allergic to the fragrance mix, while 258 (8.4%) patients exhibited an allergic reaction to Myroxylon pereirae (balsam of Peru). Between those 509 patients, 157 were patch tested with eight individual substances contained in the fragrance mix: cinnamal, cinnamyl alcohol, eugenol, isoeugenol, geraniol, hydroxycitronellal, a

Routine diagnostic patch-testing with formaldehyde 2.0% (0.6 mg/cm2) may be an advantage compared to 1.0%.

Our clinical experience has suggested that the presently recommended patch-test concentration (1.0%) for formaldehyde in the baseline series might be too low. Therefore, consecutively patch-tested dermatitis patients were tested simultaneously with formaldehyde 1.0% and 2.0% (w/v) in aqua. Formaldehyde 1.0% and 2.0% were applied with a micro-pipette (15 microl) to filter paper discs in Finn Chambers (0.30 mg/cm(2) and 0.60 mg/cm(2), respectively). A total of 1397 patients with dermatitis were patch-tested. In all, 68 (4.9%) patients reacted positively to formaldehyde; 37 reacted only to 2.0%,

Spice allergy evaluated by results of patch tests.

Allergic contact dermatitis caused by spices is well documented; however, commercial patch tests are unavailable. Between October, 1991, and August, 1992, a series of fifty-five patients with suspected contact dermatitis were tested at Ochsner Clinic for sensitivity to a group of spices at concentrations of 10 percent and 25 percent in petrolatum. Concordant patch test results (positive at concentrations of 10 percent and 25 percent) were most common with ginger (seven), nutmeg(five), and oregano (four); the remaining spices produced zero or one positive responses. Patients exhibiting positive

An attempt to reduce the photosensitizing potential of chlorpromazine with the simultaneous use of beta- and dimethyl-beta-cyclodextrins in guinea pigs.

The effects of topically applied beta-cyclodextrin (beta-CyD) and heptakis(2,6-di-O-methyl)-beta-cyclodextrin (DM-beta-CyD) on photoallergic contact dermatitis due to chlorpromazine hydrochloride (CPZ) were investigated using the photomaximization technique in guinea pigs. From the gross and histological observations, the photosensitizing potential of CPZ with the simultaneous topical use of beta-CyDs was significantly lower than that of CPZ alone. The alleviating efficacy of DM-beta-CyD was greater than that of beta-CyD. beta-CyDs suppressed the penetration of CPZ into the skin through the fo

The 'MOAHLFA(P) Index': An Attempt to Standardise a Widely Used Array of Descriptors of Patch-Tested Patients.

BACKGROUND: Since its inception in 1980, the MOHL index (% patients who are male, have occupational, hand, or leg dermatitis, respectively) and its later evolutions until the presently used MOAHLFA(P) index (adding % patients with atopic dermatitis, face dermatitis, age 40+ years and positive reaction(s) to ≥ 1 baseline series allergen) have been intended to convey important demographic and clinical information on the patients patch tested in a certain area and time, aiding the interpretation of the observed spectrum of sensitisation. OBJECTIVES: To examine the current usage of the MOAHLFA(P)

Mangiferin antagonizes TNF-α-mediated inflammatory reaction and protects against dermatitis in a mice model.

This study aimed to investigate whether mangiferin played a protective role in a well-established dermatitis mouse model and tumor necrosis factor alpha (TNF-α)-induced RAW264.7 macrophages. Contact dermatitis is an inflammatory skin disease in the clinic, while its underlying mechanism still remains to be elucidated. Mangiferin, 1,3,6,7-tetrahydroxyxanthone-C2-β-d-glucoside (C-glucosyl xanthone), a natural antioxidant that was reported to inhibit inflammatory reactions, has been recently proved to be a potential therapy for inflammation. As a result, the oxazolone-induced dermatitis mice mode

Crosstalk of regulatory T cells and tolerogenic dendritic cells prevents contact allergy in subjects with low zone tolerance.

BACKGROUND: Allergic contact dermatitis is one of the most common occupational diseases. A main protective mechanism in those who do not develop allergic contact dermatitis is tolerance induction by repeated exposure to low doses of contact allergen, which is termed low zone tolerance (LZT). The mechanisms that determine the tolerance induction in subjects with LZT are still elusive. OBJECTIVE: We performed analysis of the role of CD4(+)CD25(+) forkhead box protein 3 (FOXP3)-positive regulatory T (Treg) cells and dendritic cells (DCs) in mice with LZT. METHODS: Mechanisms of tolerance induct

Differential phosphorylation of MAPK isoforms in keratinocyte cell line by contact allergens and irritant.

ABSTRACT Mitogen-activated protein kinase (MAPK) is an important mediator of cellular signaling pathways. In an attempt to investigate the role of these kinases in contact allergen/irritant-induced signaling cascades, we have utilized keratinocyte (A431 cell line) as antigen presenting cells (APCs). This group has previously reported the applicability of A431 as an APC. The role of these kinases in the elicitation pathways of allergic contact dermatitis was investigated using interleukin-12p40 (IL-12p40) as a marker. Inhibitors of the isoforms of MAPK reduced the allergen-induced IL-12 release

[Allergies to dental materials and dental pharmacologic agents].

Allergic reactions to dental materials and remedies cause stomatitis in the patient and a contact dermatitis in the dental personnel. Topical contact (acrylic resin denture materials, heat accumulation, plaques on the denture) and endogenic (symptom of internal and psychiatric illnesses) factors are the cause for denture sore mouth. In metallic alloys corrosions and electric currents can effect an irritation to the oral mucosa. The application of amalgam fillings can possibly result in a mercurialism. Dental remedies used for treatment of wounds and dental roots are potent sensitizers. Surface

Hypersensitivity reactions to corticosteroids.

Hypersensitivity reactions to corticosteroids (CS) are rare in the general population, but they are not uncommon in high-risk groups such as patients who receive repeated doses of CS. Hypersensitivity reactions to steroids are broadly divided into two categories: immediate reactions, typically occurring within 1 h of drug administration, and non-immediate reactions, which manifest more than an hour after drug administration. The latter group is more common. We reviewed the literature using the search terms "hypersensitivity to steroids, adverse effects of steroids, steroid allergy, allergic co
