Post-Transplant Lymphoproliferative Disorder Presenting as Biventricular Failure After Heart Transplantation.

BACKGROUND: The diagnosis of post-transplant lymphoproliferative disorder (PTLD) is challenging because of its diverse clinical presentations. CASE SUMMARY: A 65-year-old man, previously stable 61 months after heart transplantation for dilated cardiomyopathy, presented with biventricular failure and worsening dyspnea. Given the acute decline in graft function, acute rejection was suspected, and rejection therapy was initiated. However, the patient showed no clinical improvement, and he required venoarterial extracorporeal membrane oxygenation support as a bridge to redo heart transplantation.

A Novel PHEX Mutation in X-Linked Hypophosphataemic Rickets With Reduced Expression of NaPi-IIa and NaPi-IIc in the Proximal Tubules: A Case Report.

X-linked hypophosphataemia (XLH, MIM#307800) is an inherited form of rickets resulting from mutations in the phosphate-regulating neutral endopeptidase (PHEX) gene on the X chromosome. These mutations lead to elevated circulating fibroblast growth factor 23 (FGF23), which disrupts phosphate homeostasis and contributes to XLH pathogenesis. We present a sporadic case of a 41-year-old woman diagnosed with rickets in childhood who later developed persistent proteinuria. Kidney biopsy revealed segmental sclerosis with a perihilar lesion in one of 19 glomeruli, along with dilated proximal tubules, r

Atypical location of a rare tumor: orbital myopericytoma.

A 46-year-old man presented with a 2-year history of progressive visual acuity loss in his right eye. A computed tomography (CT) scan revealed a well-circumscribed, isodense intraconal lesion at the apex of the right orbit. Magnetic resonance imaging (MRI) showed a solid, hyperintense lesion on T2-weighted images with heterogeneous enhancement after gadolinium contrast administration. The resection of the mass was performed via a lateral orbitotomy. Complete excision was not possible due to dense adhesions to deep structures, and intraoperative manipulations to dissect the tumor from its attac

Heterotopic pancreatic tissue presenting as an unusual cause of gastric outlet obstruction in infancy: a case report.

BACKGROUND: Gastric outlet obstruction beyond the neonatal period is an extremely rare condition when other causes such as infantile hypertrophic pyloric stenosis, pyloric atresia, and antral diaphragm are ruled out. Herein, we present the case of a 2-month-old girl who presented with repeated nonbilious vomiting and showed ectopic pancreatic tissue compressing the pylorus of the stomach. CASE PRESENTATION: A 2-month-old Caucasian girl who suffered from frequent attacks of projectile vomiting that was not resolved by medication presented to our institute. On ultrasonography, pyloric stenosis

Severe Cardiomegaly in Congestive Heart Failure: Postmortem Evidence of Cardiac Remodeling.

BACKGROUND: Cardiomegaly is the gross manifestation of advanced cardiac remodeling, a compensatory response to prolonged hemodynamic stress. Direct postmortem examination of cardiac remodeling is rare, because cardiomegaly is clinically identified through radiologic imaging. CASE SUMMARY: An 81-year-old male cadaveric donor with congestive heart failure as the known cause of death was dissected under standard procedure. Gross examination revealed severe enlargement of the heart and marked dilation of the aorta. Microscopic examination demonstrated significant left ventricular myocyte hypertro

An Incidental Finding of Abdominal Aortic Aneurysm and Its Histology in a Dissected Human Cadaver.

Abdominal aortic aneurysm (AAA) is a vascular condition often associated with atherosclerotic degeneration and typically remains asymptomatic until rupture. It predominantly affects older males and carries a significant mortality risk if undiagnosed or untreated. This report describes a rare case of an infrarenal AAA observed during the routine dissection of a male cadaver aged 65-75 years at All India Institute of Medical Sciences (AIIMS), Rishikesh. The aneurysm exhibited fusiform dilatation with a maximum transverse diameter of 46.79 mm, accompanied by atherosclerotic changes, mural thrombu

Analysis on forensic pathology of water intoxication: A review.

Post-mortem diagnosis of water intoxication should be confirmed through analysis during an autopsy. Demonstrating typical tissue changes in accordance with the clinical history is useful for assessing the reliability of a hyperhydrated state. A review of current literature on autopsy cases involving excessive liquid intake was conducted, and a definition of pathological death due to water intoxication is proposed. Analyzed cases revealed general cerebral and lung abnormalities along with significant right ventricular heart dilation. Histological findings included neuronal degeneration characte

A rare case of pancreatic mucinous cystic neoplasm in a pediatric patient.

Pancreatic mucinous cystic neoplasms (MCNs) are rare tumors predominantly identified in middle-aged females. Occurrences are extremely rare in children. We report a rare case of pancreatic MCN in an adolescent. A 14-year-old female with Smith-Magenis syndrome and steatotic liver disease (SLD) presented with abdominal discomfort. Magnetic resonance elastography obtained for SLD revealed cholelithiasis and a 0.5-cm cyst in the distal pancreas. Observation was recommended by her pediatric surgery provider. At 18 years old, contrast-enhanced abdominal magnetic resonance imaging showed a 2.4-cm mil

Challenges in Echocardiography for the Diagnosis and Prognosis of Non-Ischemic Hypertensive Heart Disease.

It has been well established that arterial hypertension is considered as a predominant risk factor for the development of cardiovascular diseases. Despite the link between arterial hypertension and cardiovascular diseases, arterial hypertension may directly affect cardiac function, leading to heart failure, mostly with preserved ejection fraction (HFpEF). There are echocardiographic findings indicating hypertensive heart disease (HHD), defined as altered cardiac morphology (left ventricular concentric hypertrophy, left atrium dilatation) and function (systolic or diastolic dysfunction) in pati

Retrospective analysis on the occurrence of kidney cysts in mice in a central animal facility in the years 2009-2019.

Kidney cysts in humans are mainly caused by inheritable polycystic kidney disease. Although they are a regular finding in laboratory mice, their occurrence upon dissection has not been systematically investigated, yet. Therefore, the aim of this report was to investigate on prevalence, phenotype and aetiology of spontaneously occurring kidney cysts in mice by retrospectively analysing the laboratory-receipt tables of the in-house laboratory of a central animal facility in North Rhine-Westphalia, Germany, years 2009-2019. A percentage of 0.4% of dissected mice displayed kidney cysts, with more

Proteomic and metabolomic analyses of the human adult myocardium reveal ventricle-specific regulation in end-stage cardiomyopathies.

The left and right ventricles of the human heart are functionally and developmentally distinct such that genetic or acquired insults can cause dysfunction in one or both ventricles resulting in heart failure. To better understand ventricle-specific molecular changes influencing heart failure development, we first performed unbiased quantitative mass spectrometry on pre-mortem non-diseased human myocardium to compare the metabolome and proteome between the normal left and right ventricles. Constituents of gluconeogenesis, glycolysis, lipogenesis, lipolysis, fatty acid catabolism, the citrate cy

State-of-the-Art Imaging of Infiltrative Cardiomyopathies: A Scientific Statement From the American Heart Association.

Infiltrative cardiomyopathies comprise a broad spectrum of inherited or acquired conditions caused by deposition of abnormal substances within the myocardium. Increased wall thickness, inflammation, microvascular dysfunction, and fibrosis are the common pathological processes that lead to abnormal myocardial filling, chamber dilation, and disruption of conduction system. Advanced disease presents as heart failure and cardiac arrhythmias conferring poor prognosis. Infiltrative cardiomyopathies are often diagnosed late or misclassified as other more common conditions, such as hypertrophic cardio

A Rare Case of Metastatic Ampullary Adenocarcinoma Following the Whipple Procedure.

Ampullary carcinoma is an extremely rare type of gastrointestinal cancer that originates at the ampulla of Vater, distal to the junction between the pancreatic duct and the common bile duct (CBD). There are three subtypes depending on the histological findings: pancreatobiliary, intestinal, and mixed subtype. Symptoms can mimic other pathologies related to biliary obstruction, such as jaundice, diarrhea, steatorrhea, and weight loss. In this report, we present a case of a 40-year-old male who presented with painless jaundice and dizziness. Magnetic resonance cholangiopancreatography (MRCP) sho

[Heart failure in young children; a serious cause of feeding problems].

Feeding problems in children are usually harmless and common, but can rarely exist as a result of pathology. Heart failure is one of them and has to be recognized early because of its many consequences. We present a 15-week-old female infant who was seen at the outpatient clinic. She had already been evaluated several times by a youth doctor and general practitioner because of feeding problems and transpiration, for which several nutritional interventions had already been carried out. With no effect of nutritional interventions she was referred to a pediatrician. Physical examination showed cl

Signet-ring cell carcinoma of the appendix with ganglioneuromatosis: a case report.

BACKGROUND: Primary cancer of the appendix, especially signet-ring cell carcinoma, is an uncommon disease, and it is rarely suspected before surgery. Diffuse intestinal ganglioneuromatosis that is not associated with neurofibromatosis-1 or multiple endocrine neoplasia 2b is also rare. The most frequent symptoms caused by it are changes in bowel habits, abdominal pain, and occlusive episodes. CASE PRESENTATION: The patient was a 48-year-old woman who had a month-long history of chronic abdominal pain, fullness, constipation, and diarrhoea. Enhanced computed tomography showed a 100-mm irregular

Cardiovascular disease associated with methamphetamine use: a review.

Methamphetamine abuse is a global epidemic associated with a wide-ranging array of adverse effects on the cardiovascular system including dilated cardiomyopathy, malignant and benign arrhythmias, coronary vasospasm, and atherosclerotic coronary artery disease. While the acute behavioral manifestations of amphetamine abuse are the most easily clinically identified, cardiovascular toxicity is common in this patient population and should be considered in this setting due to its high morbidity and mortality. The specific mechanisms for amphetamine cardiotoxicity have not been fully established, bu

Duplication of the gallbladder: Case report of a rare biliary malformation.

Duplication of the gall bladder is a rare anatomic variation. The incidence is approximately 1 in 4000 in literature. Preoperative identification of such anomaly and its various types is very important since it can avoid damage to possible vascular and biliary aberrant anatomy during surgery. My case is a 29-year-old male patient with a complaint of epigastric pain which was on and off type. An abdominal ultrasonogram showed multiple calculi in the gallbladder lumen with normal wall thickness and no evidence of intra or extra-hepatic biliary tree dilatation. Another cystic structure was noted
