The vaginal colonization of 495 healthy women with strains of Staphylococcus aureus positive for the toxic-shock marker protein (pyrogenic exotoxic C) was examined prospectively. Production of the marker protein was detected by analytic isoelectrofocusing of culture filtrates in polyacrylamide gels (isoelectric point, 7.2). Toxicogenic strains of S. aureus were isolated from 2.6% of the women and nontoxicogenic strains from 4.0%. One hundred patients undergoing postoperative cholangiography had blood cultures drawn prior to and 15 minutes and six hours after cholangiography. Bile cultures obtained prior to cholangiography grew organisms in 92 of 100 patients with E. coli, Klebsiella pneumoniae and enterococcus being the bacteria most frequently isolated. Anaerobes were isolated from the bile in 21% of the patients. Sixteen patients with necrotizing fasciitis were observed under clinical and laboratory conditions for collection, preservation, and culture that permitted optimal retrieval of anaerobes. The clinical observations of necrosis of fascia, subcutaneous fat and skin with thrombosis of the microvasculature, and absence of myonecrosis were clearly apparent in these patients.
