An ultrasound dissector especially developed for laparoscopic surgery was used during laparoscopic cholecystectomy on 34 patients. The ultrasound power, the volume of suction and irrigation could be determined individually at the generator and activated during the operation with a foot pedal. With the dissector it was possible to fragmentate, emulgate and aspirate simultaneously fat tissue as well as infected edematous structures. A new potential indication for cardiac pacing is chronotropic incompetence, that is, an inadequate cardiac rate response to exercise and other metabolic demands. Many patients who have been paced for indications such as complete heart block or sick sinus syndrome also have chronotropic incompetence. Such patients are not adequately treated when fitted with a constant rate pacemaker. Lower extremity equipment-related injuries are the most significant injury group in alpine skiing. The lower extremity equipment-related injuries occurring at four Norwegian ski resorts were studied during the winter of 1985 to 1986. A total of 132 skiers with injuries were included (40% of all injured skiers) and compared with a randomly selected control population of 316 uninjured skiers. Fourteen young adult patients between 14 and 29 years of age underwent 15 procedures of transcatheter occlusion of patent ductus arteriosus. In all patients but one, the attempt to implant the device was successful (92%). In one patient the ductus was judged to be too large for occlusion and the patient was sent for surgery. The principle of ligamentotaxis obtained by longitudinal traction is useful in restoring skeletal length to distal radial fractures. Using external skeletal fixation to translate the hand in radial-ulnar and dorsal-palmar directions, ligamentotaxis in two additional planes aligns and tilts the distal radial fragment and its articular surface. Following restoration of palmar tilt by palmar translation, wrist position can be adjusted into neutral or extension to help avoid finger stiffness and carpal tunnel syndrome without compromising fracture reduction.

There are many surgical approaches for closure of nasal septum, most of which present technical difficulties. Moreover, the clinical results are, as yet, unsatisfactory. Prefabricated prostheses have been effective for treatment of smaller perforations, and are an excellent alternative in a great number of cases. We present an optional surgical technique to deal with this increasingly common problem.

The 2nd generation lithotripter LITHOSTAR plus was used for extracorporeal shock wave lithotripsy of 9 kidney stones, 2 ureteral calculi and 2 bladder stones in 10 children 15 months to 17 years old. Ultrasonic stone localization was used in 2 children, x-ray guided stone localization in 8 children. General anesthesia was necessary in 6 children. A stone free rate of 60% was achieved after 1 treatment. Except for one 14-year old boy no adjuvant procedures like percutaneous nephrostomy or ureteral stents were applied. No major complications were encountered.

Prostatic obstruction of urinary flow is a common age-related event resulting in both troublesome symptoms and retention of urine. There have been recent developments in the management of such patients by inserting metallic stents or by balloon dilatation of the prostatic urethra. The role of these treatments as alternatives to surgery is evaluated and some of the difficulties encountered in their usage are described in this article.

An original technique of direct isthmic reconstruction described by E. Morscher was used in 25 patients with painful spondylolysis after failure of conservative treatment. After a follow-up of 25 months, the functional and radiological results were evaluated and compared with other techniques. A miniaturized Lashley cup for collecting rat parotid and submandibular/sublingual saliva is described. The small dimensions of the cup enabled its proper positioning on the orifices of both salivary ducts. The authors describe an external fixation rod apparatus and the results of its application in 28 patients with various injuries. The apparatus was used in the patients with fractures of phalanges of the fingers and metacarpal bones for extra-articular fixation after the elimination of contractures and dislocations in the joints, in cases of combined epidermatoplasty, arthrodesis, etc. The method of treatment had to be changed only in 2 patients. In all other cases the effect of the rod apparatus was positive.

Upper tract tumors may be diagnostic problems, and the importance of obtaining tissue in a closed fashion has been illustrated. This is probably the most important advantage of the ureteroscopic approach to these tumors. Ureteroscopy is safe and reliable and allows examination of about 95 per cent of all patients evaluated.
