[Effects of lumbar epidural anesthesia on brainstem auditory response].

Auditory brainstem responses (ABR) are used increasingly to monitor the integrity of neural pathways in anesthetized patients. To evaluate the effect of epidural anesthesia on the central nervous system, we studied the effects of lumbar epidural anesthesia with lidocaine on the ABR in 10 patients. The seven peak latencies (waves I-VII) and amplitudes (waves I-VII) of the ABR were recorded before and 15 min after epidural anesthesia. Peak latencies of waves III, IV, V, VI, VII increased after epidural anesthesia compared with control values. Amplitude of waves II only decreased significantly fo

[Polyesthesia--report of two cases].

Polyesthesia is an abnormal sensation in which a single sensory stimulation seems to be felt as several ones. We report two cases showing th … l distribution. Rubbing the foot could not elicit any sensation on the hand or trunk. Somatosensory evoked potentials (SEPs) following tibial and peroneal nerve stimulation could not be recorded on the scalp. Dermatomal SEPs below the tenth thoracic level were also silent over the scalp. SEPs obtained by median nerve stimulation were normal bilaterally. Various neu …

Neurophysiological abnormalities in adolescents with type I Crigler-Najjar syndrome.

We report neurophysiological abnormalities in two adolescents with type I Crigler-Najjar syndrome, an autosomal recessive disorder characterized by severe unconjugated hyperbilirubinemia. Electroencephalograms (EEGs) demonstrated frequent generalized single and polyspikes, and background slowing. Normal pattern reversal evoked responses (PRVERs) and normal central brain-stem auditory evoked responses (BAERs) were recorded in both patients. These findings differ from the EEG triphasic wave pattern seen in the EEG in hepatic encephalopathy and the central BAER abnormalities seen in many infants

Myoclonus and adult coeliac disease.

A 56-year-old woman with coeliac disease developed myoclonus of cortical origin and palatal myoclonus with lesions of subcortical white matter on magnetic resonance imaging. Myoclonus can thus be a prominent feature of coeliac disease encephalopathy. A slight vitamin E deficiency was found but does not satisfactorily explain this myoclonic encephalopathy, which remains of unknown origin.

Abnormalities of visual evoked responses in hyperprolactinaemia.

The relationship between the visual evoked response (VER) and chiasmal lesions remains unclear, although a few published studies have reported waveform distortions as the major abnormality. In this study of 10 female and 2 male patients we carried out a retrospective analysis of the latencies and waveforms of their full field and half field VERs to pattern reversal stimulus. CT scanning showed minimal suprasellar involvement in 3 patients. The VER in 2 of these 3 patients showed slight prolongation in latency and waveform distortions. In 4 of the remaining 9 patients with no suprasellar involv

Evoked accelerometry: a sensitive and accurate method for evaluating facial nerve function using a portable device.

A new electrodiagnostic technique has been developed which can be used to monitor facial nerve function. The technique has been termed "evoked accelerometry" (EAC) and is administered by a portable DC powered device. In performing a test, the device stimulates the facial nerve and then measures the evoked muscular response by a small accelerometer sensor affixed to the face with suction. The magnitude of the response is displayed as a digital readout on the device meter. The degree to which a reading is subnormal indicates the extent of nerve degeneration. In a study of normal participants the

Auditory event-related potentials in well-characterized groups of children.

Sixty-eight subjects ranging in age from 6 to 23 years were studied in an 'auditory oddball' event-related potential (ERP) paradigm. Our results replicate other studies, finding P3 as the most consistent component of ERPs since childhood, although great variability of this component was found in the 6-year-old group. Separate age/ERP component latency and amplitude linear regressions were computed for subjects 6-14 and 6-23 years old. Our data show in both groups a significant negative and positive correlation between age and P3 latency and N1-P2 amplitude respectively. The age/P3 latency slop

Pyridoxine for neonatal seizures: an unexpected danger.

A case of pyridoxine-dependent seizures is reported. Administration of pyridoxine to an infant after a long period of convulsions was followed by acute hypotonia. Other cases have been reported in the literature, in one of which assisted ventilation was required. A possible mechanism for this alarming outcome is discussed, and it is suggested that resuscitation facilities should be quickly available during such trials.

Postnatal development of auditory function in the chicken revealed by auditory brain-stem responses (ABRs).

Auditory evoked brain-stem responses (ABRs) were recorded from the surfaces of the brain of lightly anesthetized newborn (1-7 days old) and adult (7-9 weeks old) chickens as a measure of the development of auditory processing. One-day-old and older chickens showed a series of waves within 5 msec after the stimulus onset. This precocity of the ABR in chickens contrasts with the first appearance of the ABR in cats at 4 days of age. The ABR onset latency was shorter in adult chickens than in newborns. This indicates that developmental modifications of mechanical transmission in the external and m

Auditory brain-stem responses in hydrocephalic patients.

Auditory brain-stem response (ABR) was measured in 40 patients (80 ears) with confirmed hydrocephalus. Eighty-eight percent of these patients showed some form of ABR abnormality. Responses indicative of brain-stem dysfunction consisted of prolonged I-V interwave latency (38%), reduced V/I amplitude ratio (33%), and abnormalities in wave-shape of components III (27%) and V (53%). In addition, 70% of the patients had elevated ABR thresholds; 45% had responses in excess of 20 dB HL and the remaining 25% had no ABR activity. The etiology of the hydrocephalus, head circumference and brain-stem symp

Alteration of brain stem auditory evoked responses following cardio-respiratory arrest and resuscitation.

BAERs were recorded in a patient who suffered spontaneous cardio-respiratory arrest after testing had begun. During the arrest the BAERs initially decreased in amplitude and then could not be recorded. After restoration of cardiac activity there was immediate recovery of waves I and II. Subsequent potentials also immediately reappeared but they did not recover their original peak latency values during the 50 min following resuscitation.

Focal EEG abnormalities in Heidenhain's variant of Jakob-Creutzfeldt disease.

Serial EEGs obtained during a six-week period from a patient with Heidenhain's variant of Jakob-Creutzfeldt disease demonstrated periodic complexes confined to the occipital regions that at no time became generalized. The focal character of the discharges correlated with the site of maximal disease in the occipital cortex, suggesting that cortical damage is a necessary substrate for the production of periodic complexes in Jakob-Creutzfeldt disease.
