Telemedical experiences at an Antarctic station.

Wintering-over in Antarctica represents a physician's most remote and inaccessible scenario, apart from a space station. Because of the harsh and unpredictable winter weather, Antarctic stations are typically inaccessible for over six months of the year. Telephone and fax communication, and recently other forms of telemedicine, have provided vital links to specialists. The author was the sole physician for more than 250 people wintering-over during the 1995 austral winter at McMurdo Station. There were several instances of serious or life-threatening illness where the author relied on telecons

The 'sandwich' zygomatic osteotomy: technique, indications and clinical results.

The classical approach to lateral midface hypoplasia is reconstruction with onlays. Dislocation and asymmetry, early and late infection, and extrusion are possible complications with alloplastic implant materials. Unpredictable resorption, irregular contours and asymmetry are problems that can arise with autogenous, homogenous and hetergenous onlay grafts. We describe a technique by which the zygomatic body is luxated laterally and ventrally after a combined oblique-horizontal and vertical osteotomy. The resultant greenstick fracture at the temporo-zygomatic suture together with minimal latera

Arthroscopically assisted proximal extensor mechanism realignment of the knee.

An arthroscopically assisted proximal extensor mechanism realignment procedure has been designed and performed on 27 knees in 24 patients. The study group has been followed a minimum of 18 months. The procedure consists of a lateral retinacular release and an arthroscopically controlled plication of the medial patellar retinaculum and oblique fibers of the vastus medialis. The indications for this procedure include recurrent patellar instability and acute initial patellar dislocation with a concomitant fracture of either the patella or lateral margin of the intertrochlear sulcus. Using a simpl

Traumatic axial dislocations of the carpus.

Among 1140 patients treated in the last 16 years for any type of carpal fracture, dislocation, or subluxation, 16 (1.4%) patients seen with a traumatic axial disruption of both the carpus and metacarpus were identified. Most of these were crush or blast injuries. According to the direction of instability, the cases were classified into three groups: axial-ulnar disruptions (4 patients), axial-radial disruptions (11 patients), and combined axial-radial-ulnar disruption (1 patient). Treatment consisted of open reduction and percutaneous Kirschner-wire fixation in most cases. The results were les

Transient loss of femoral head Tc-99m diphosphonate uptake with prolonged maintenance of femoral head architecture.

Nine pediatric patients presented with synovitis and suspected Legg-Perthes disease, hemarthrosis, acute slipped capital femoral epiphysis, prolonged traumatic hip dislocation, or transcervical femoral fracture. All had hip scintigraphs that showed loss of Tc-99m diphosphonate uptake in the affected femoral head. Radiographic changes associated with aseptic necrosis failed to appear during the initial follow-up period of three to 14 months. Repeat scintigraphy showed complete revascularization in six cases and partial revascularization in two. Reambulation at 1.5 to eight months after presenta

Early complications of acute anterior dislocation of the shoulder in the middle-aged and elderly patient.

Shoulder function following acute anterior dislocation may be impaired by rotator cuff rupture, fracture of the greater tuberosity and/or nerve injury. Twelve such cases seen at a referral shoulder clinic are described. The most common symptoms were loss of movement (100 per cent of patients) and pain (66 per cent). Some loss of active abduction was present in all cases and muscle wasting in 33 per cent. Operation was indicated in 9 patients. There were 2 good, 3 fair and 4 poor results following "late' operation performed as a salvage procedure. Rotator cuff rupture may be obscured by a circu

Pathological changes in the lumbar spine of pigs: gross findings.

The lumbar vertebral columns from 60 sows and 30 slaughter weight pigs were examined grossly for pathological changes. Asymmetry of lumbar articular facets and minor periarticular osteophytes were seen in the slaughter weight group. Degeneration of intervertebral discs or vertebral osteophytes were not present. In contrast, 38% of 60 sows had vertebral osteophytes and 40% had degeneration of intervertebral discs. Extensive ankylosing spondylosis was present in two sows. Other vertebral lesions observed in sows include asymmetry and arthrosis of articular facets, fissures and areas of cavitatio

[Surgical stabilization of multiple rib fractures can shorten inpatient stay].

Flail chest is a common complication in patients with blunt chest wall traumas resulting in high mortality rates. In this case report a 43-year-old woman was stepped on by a horse, thereby receiving multiple rib fractures and pneumohaemothorax. She was not able to wean from epidural analgesia after ten days and maintained a "thoracic floating feeling". In opposition to the non-operative management previously preferred, a surgical stabilization was then performed in the patient, who was discharged only five days later. Thus, surgical fixation of multiple rib fractures may reduce inpatient stay.

Anatomical reduction of marginal impaction in posterior wall acetabular fractures ensures mid-term satisfactory outcomes.

This study aimed to compare the postoperative radiological and functional outcomes of posterior wall acetabular fractures with and without marginal impaction. Patients who underwent surgical treatments for posterior wall acetabular fractures at a single level I trauma center between January 2013 and June 2022 were enrolled. Seventeen patients with marginal impaction were classified into group I, and the remaining 24 were classified into group II. Immediate postoperative reduction quality was confirmed using computed tomography (CT), final follow-up results were evaluated using simple x-ray. He

A Young Patient with Multiple Pathological Fractures.

A 26-year-old male was asymptomatic and active until the age of 19 years. At the age of 20 years, he underwent lithotripsy for ureteral stones. As he grew older, he experienced bone pain, recurrent bilateral shoulder dislocations, and long bone fractures following minor trauma for which he was operated. Currently, he presented with another long bone fracture following minor trauma. Investigations showed increased serum calcium (11.5 mg/dL), 24-hour urinary calcium (420 mg/24 hours; 100-300 mg/day), and intact parathyroid hormone (PTH) (1688 pg/mL; 10-55 pg/mL). Serum phosphorus was 2.0 mg/dL (

[Osteochondral fractures in childhood and adolescence].

BACKGROUND: Osteochondral fractures (OCF) are traumatic shearing injuries to portions of cartilage and bone. The most common cause is patellar dislocation, with the main localisation being the medial patellar facet and the lateral femoral condyle. They can occur in all age groups. DIAGNOSIS: Clinically, there is a painful knee joint effusion (haemarthrosis) with a "dancing patella". This is usually accompanied by restricted movement and/or a locking phenomenon. In addition to the standard X‑ray of the knee in three planes (lateral, anterior-posterior and tangential patella), an MRI should be

Surgical management of acute combined injuries of the ipsilateral wrist and elbow joints.

BACKGROUND: Combined injuries of ipsilateral wrist and elbow joints are rare in clinical practice, characterized by multiple joint dislocations or/and fractures and varying manifestations. As there are still no clinical guidelines and no consensus on the standard treatment, this study aimed to explore the surgical intervention and complications of this kind of combined injuries. METHODS: This retrospective study was conducted in a single center. A total of 13 patients with acute combined injuries of the ipsilateral wrist and elbow joints receiving surgical treatment from August 2013 to May 20

The outcome of spine metastatic after surgery: a case series.

UNLABELLED: Spine metastases occur because of the spread of the primary tumour that causes back pain and neurological deficits in the sufferer and has a high risk for surgery. CASE PRESENTATION: In this case series, all three patients had the same initial symptoms; back pain and lower limb weakness and had a history of previous primary tumours that metastasized to the spine. The MRI showed a tumour mass on T11 accompanied by a burst fracture in the first patient, L4 in the second patient, and T3 in the third case accompanied by a dislocated fracture. The three reported patients underwent a po

Primary total hip arthroplasty in patients with sequelae of poliomyelitis.

INTRODUCTION: Total hip arthroplasty (THA) in patients with residual poliomyelitis (RP) is a surgical challenge. Dysplastic morphology, osteoporosis and gluteal weakness hinder orientation, increase fracture risk and reduce implant stability. The aim of this study is to describe a series of patients with RP treated by THA. MATERIAL AND METHODS: Retrospective descriptive study of patients with RP treated with THA between 1999 and 2021 in a tertiary hospital, with clinical and radiological follow-up and functional and complication evaluation until present or death, with a minimum of 12months.

A novel asymmetric horizontal splitting fracture of the lumbar spine: pediatric case report of a lateral bending, flexion and distraction fracture.

BACKGROUND: There are many lateral bending fracture cases presented in the literature that mostly involve facet dislocations or corpus collapse. In this report, we aim to describe a novel asymmetric lateral bending, flexion and distraction fracture, propose a mechanism, and delineate its clinical importance. CASE DESCRIPTION: A 13-year-old girl arrived at our trauma center 12 hours after a head-on truck collision. She had paraplegia, and her imaging revealed a spinal cord avulsion at the T10 level and a horizontal fracture at the L4 spinous process, left lamina, left pedicle, and left posteri

The Use of Standard Cemented Femoral Stems in Total Hip Replacement After Failed Internal Fixation of Intertrochanteric Femoral Fractures.

BACKGROUND: Total hip replacement (THR) after failed internal fixation of intertrochanteric femoral fractures is challenging. The aim of this study is to show the reliability of using standard cemented femoral stems in this operation. METHODS: This work included 107 THRs performed in 107 patients after failed treatment of intertrochanteric femoral fractures. The etiology of failure included 67 cases of failure of fixation, 16 cases of nonunion, 15 cases of avascular necrosis, and 9 cases of post-traumatic osteoarthritis. There were 48 males and 59 females. The mean age was 66 years (range 58-
