Treatment of acetabular fractures with quadrilateral plate injury - a comparison of two commonly used methods.

BACKGROUND: Acetabular fractures with quadrilateral plate (QLP) involvement have traditionally been treated by buttress plates applied through an Ilio-inguinal approach. Recently, many surgeons prefer the application of infra-pectineal plates through the modified Stoppa approach for this purpose. Whether one method can be preferred over the other is a matter of investigation. HYPOTHESIS: The research question was whether an infra pectineal plate applied through the modified Stoppa approach for acetabular fractures with QLP involvement would result in an equivalent or better outcome than a but

Clinical Outcomes Between Atrophic and Nonatrophic Mandibular Fracture in Elderly Patients.

The number of elderly patients with mandibular fracture is rapidly increasing. To improve outcome, it is important to understand the age-related characteristics of mandibular fracture. Thus, the aim of this study is to analyze the impact of atrophic change on mandibular fracture in elderly patients. The retrospective study was conducted in patients aged ≥65 years old, who underwent surgery for the treatment of mandibular fracture in our hospital from March 2006 until March 2015. Patient characteristics, such as age and gender, causes of injury, anatomic location of fracture, height of mandibul

A unified theory of bone healing and nonunion: BHN theory.

This article presents a unified clinical theory that links established facts about the physiology of bone and homeostasis, with those involved in the healing of fractures and the development of nonunion. The key to this theory is the concept that the tissue that forms in and around a fracture should be considered a specific functional entity. This 'bone-healing unit' produces a physiological response to its biological and mechanical environment, which leads to the normal healing of bone. This tissue responds to mechanical forces and functions according to Wolff's law, Perren's strain theory an

Subtrochanteric femur fracture after removal of screws for femoral neck fracture in a child.

Displaced femoral neck fractures are rare in children and are associated with a high rate of complications. Subtrochanteric fractures after cannulated screw fixation of femoral neck fractures in adults are well recognized, and there are several reports on the topic. However, there are no reports on complications related to hardware or subtrochanteric fractures after removal of the screws in the treatment of femoral neck fractures in children. Here we report the case of a 10-year-old boy who sustained a subtrochanteric fracture after the screw removal and healing that followed a femoral neck fr

Inflation osteoplasty: in vitro evaluation of a new technique for reducing depressed intra-articular fractures of the tibial plateau and distal radius.

BACKGROUND: Anatomic reduction of intra-articular fractures of the tibial plateau and distal radius can be difficult to achieve. Treatment goals are centered on restoring the native anatomy and articular congruency. Several surgeons in the USA have begun using an inflatable bone tamp to reduce these fracture patterns. The concept is built on the success of the tamp in kyphoplasty of the spine, but it has yet to be tested in the lab for use in the extremities. We performed an investigation into the safety and efficacy of using an inflatable bone tamp for intra-articular fracture reduction of th

Femoral allograft in the management of osseous hydatidosis presenting as femoral shaft non-union.

Osseous hydatidosis is a rare infection posing diagnostic and therapeutic challenge. We present a case of extensive hydatidosis of the femoral shaft with non-union of pathologic fracture. Patient had resection of involved femur and post-operative chemotherapy. Subsequently femur reconstruction was done using a femoral allograft. At 26 months patient has no recurrence and bony union is seen at host-allograft interface. We present this technique as an extended indication of allograft used in limb reconstruction.

[Treatment of a compound fracture of the mandible with a comminuted compound fracture of the maxilla (author's transl)].

A case is reported of a comminuted compound facial fracture associating a multisplintered comminuted compound fracture of the right palate, with a quadrifocal fracture of the mandible including severe displacement of fragments of a bi-subcondylar fracture. Treatment was difficult and consisted of producing a stable solid osteosynthesis of the body of the mandible allowing rapid extra-oral symphysial traction to reduce the condylar fractures for 3 weeks. Hospitalization lasted 33 days and the final buccal opening was 34 mm, with satisfactory dental articulation. The authors emphasize the need f

Operative management of ankle fractures in patients with diabetes mellitus.

BACKGROUND: Multiple studies have documented increased risks associated with treatment of ankle fractures in patients with diabetes mellitus. We reviewed our results in the largest series to date of this complex patient group to determine the frequency of complications. METHODS: Eighty-four patients with diabetes had open reduction and internal fixation using standard fixation techniques for acute, closed ankle fractures. The 51 men and 33 women had an average age was 49.3 (22 to 77) years. The average followup was 4.1 years (11 to 97 months). Seventy-five fractures were closed and nine were

Massive Osteolysis of the Femur (Gorham-Stout Disease) Requiring Total Femur Replacement: A Case Report.

CASE: Gorham-Stout disease (GSD) is a rare disorder marked by massive bone destruction. A 56-year-old woman with GSD presented to our hospital with a proximal femur fracture. She was initially diagnosed with a simple fracture and underwent open reduction and internal fixation using an intramedullary nail. However, the delayed diagnosis of GSD resulted in postponed appropriate treatment. Although the intramedullary nail helped prevent mechanical bone failure, extensive osteolysis continued, ultimately necessitating total femur replacement. CONCLUSION: This case highlights the importance of con

Open Surgical Treatments of Osteoporotic Vertebral Compression Fractures.

With an aging population, the osteoporotic vertebral compression fracture (OVCF) has become a constant concern for its physical and neurological complications, such as spinal kyphosis and refractory pains. Compared with traditional conservative treatments, the open surgery is more superior in some ways because of its direct decompression and correction. Various operation methods applying to different indications have been developed to deal with different fracture situations, including anterior, posterior, and combined surgery. In this review, we have concluded the latest developments of the su

Ruptured Extensor Pollicis Longus Tendon After a Nondisplaced Distal Radius Fracture in a Young Adult Soccer Player.

Forearm fractures of the distal radius are one of the most common fractures seen in the upper extremity, and they represent approximately 1/6 of fractures treated in the emergency department. Forearm fractures are associated with a rare but known complication of a delayed ruptured extensor pollicis longus (EPL) tendon. This sequela is more commonly seen in adults after a nondisplaced distal radius fracture, with much variability in the incidence ranging from 0.07% to 5%. By contrast, this complication in the pediatric population is almost exclusively seen after a displaced or unstable fracture

Treatment of a sacral fracture associated with an intrasacral meningocele: A case report.

We report a case of sacral fracture complicated by an intrasacral meningocele in a 37-year-old male. The patient had a left sacral fracture with bilateral fractures of the superior and inferior pubic rami. The sacrum was fixed with a posterior plate and both superior pubic rami were fixed with an intramedullary screw. However, computed tomography myelogram and magnetic resonance imaging showed the intrasacral meningocele with the leakage of the cerebrospinal fluid into the buttocks and developed delayed union. Reoperation utilizing a strong anterior and posterior internal fixation combined wit
