Pressure distribution over the palm region during forward falls on the outstretched hands.

Falls on the outstretched hands are the cause of over 90% of wrist fractures, yet little is known about bone loading during this event. We tested how the magnitude and distribution of pressure over the palm region during a forward fall is affected by foam padding (simulating a glove) and arm configuration, and by the faller's body mass index (BMI) and thickness of soft tissues over the palm region. Thirteen young women with high (n=7) or low (n=6) BMI participated in a "torso release experiment" that simulated falling on both outstretched hands with the arm inclined either at 20° or 40° from t

Upper-extremity phocomelia reexamined: a longitudinal dysplasia.

BACKGROUND: In contrast to longitudinal deficiencies, phocomelia is considered a transverse, intercalated segmental dysplasia. Most patients demonstrate severe, but not otherwise classifiable, upper-extremity deformities, which usually cannot be placed into one of three previously described phocomelia groups. Additionally, these phocomelic extremities do not demonstrate true segmental deficits; the limb is also abnormal proximal and distal to the segmental defect. The purpose of this investigation was to present evidence that upper-extremity abnormalities in patients previously diagnosed as ha

Incidence of scaphotrapezial arthritis following volar percutaneous fixation of nondisplaced scaphoid waist fractures using a transtrapezial approach.

PURPOSE: To investigate whether volar percutaneous screw fixation of scaphoid waist fractures via a transtrapezial approach causes degenerative changes at the scaphotrapezial (ST) joint at short- to medium-term follow-up. METHODS: A total of 34 patients were available for follow-up at a mean of 6.1 years (minimum follow-up, 3.7 y) after volar percutaneous fixation of acute scaphoid waist fractures via a transtrapezial approach. The clinical follow-up examination included assessment of pain using a visual analog scale, range of motion, grip strength, and key pinch strength. We obtained radiogr

Association between decreased bone mineral density and dynapenia in postmenopausal women: a case-control study.

OBJECTIVE: To analyze the association between decreased bone mineral density (BMD) and dynapenia in postmenopausal women. METHODS: An age-matched case-control study was conducted in postmenopausal women with and without dynapenia. Assessments were carried out by measuring dominant handgrip strength with a Jamar dynamometer; BMI-adjusted cut-off points were applied. BMD was evaluated according to the World Health Organization criteria ( T -score ≤-1 SD), using dual-energy x-ray absorptiometry (Hologic Horizon). The study included sociodemographic, anthropometric, body composition, biochemical,

Radioscapholunate Fusion Using a Volarly Placed Plate for Treating Post-Traumatic Radiocarpal Joint Arthritis after a Distal Radius Fracture - A Single-Centre Prospective Study.

Background: There is currently no consensus regarding optimal treatment strategies for treating radiocarpal arthritis. The purpose of this prospective study was to compare preoperative and postoperative wrist range of motion (ROM), residual pain, grip and pinch strength, functional results and fusion rates after radioscapholunate (RSL) fusion using a volarly placed plate. Methods: This single-centre prospective study was conducted from January 2022 to December 2024. This study included nine patients (six males and three females) with post-traumatic arthritis after a distal radius fracture who

Outcomes Following Surgical Interventions for Isolated Lunotriquetral Interosseous Ligament Injuries: A Systematic Review.

Ulnar-sided wrist pain can be attributed to various bony and ligamentous structures. The purpose of this review is to compare outcomes following surgical interventions for isolated lunotriquetral (LT) interosseous ligament injuries in adults. We assessed 202 procedures from 9 retrospective case series studies of low to moderate quality based on the Structured Effectiveness Quality Evaluation Scale. The comparative outcomes (ie, range of motion, pain, strength, quality of life, complications, return to work, and patient satisfaction) were aggregated and categorized under arthrodesis, capsulodes

Evaluation of grip strength in hook of hamate fractures treated with osteosynthesis. Is this surgical treatment necessary?

OBJECTIVE: The aim of this study was to evaluate the outcomes of open reduction and internal fixation (ORIF) in hamate hook fractures and review the literature on this surgical procedure. METHODS: We report the outcomes of ORIF of hamate hook fractures in 13 consecutive patients (12 men and 1 woman; mean age: 32 years (range, 22-48 years)). In eight patients (61%) the fracture was associated with ulnar nerve neuritis in Guyon's canal. We assessed the following clinical data: age, sex, mechanism of injury, side of the injured hand and associated lesions, fracture classification, average time f

Whole exome sequencing identifies FGF16 nonsense mutations as the cause of X-linked recessive metacarpal 4/5 fusion.

BACKGROUND: Metacarpal 4-5 fusion (MF4; MIM %309630) is a rare congenital malformation of the hand characterised by the partial or complete fusion of the fourth and fifth metacarpals. The anomaly occurs as an isolated trait or part of a genetic syndrome. METHODS: To search for disease-causing mutation, whole exome sequencing (WES) was performed on samples from a single trio. Before WES, molecular screening including gene sequencing and array comparative genomic hybridisation was applied. Validation of WES and segregation studies were done using routine Sanger sequencing. RESULTS: Exome seque

Kyphosis and decline in physical function over 15 years in older community-dwelling women: the Study of Osteoporotic Fractures.

BACKGROUND: Maintaining physical function is an important prerequisite for preserving independence in later life. Greater degrees of kyphosis in the thoracic spine are prevalent in older persons and accompanied by reduced physical function in multiple cross-sectional studies. It is unknown whether kyphosis predicts worse physical function over time. METHODS: We retrospectively assessed whether greater magnitude of kyphosis is associated with decline in self-reported and objectively measured physical function over 15 years. Digitized Cobb angle kyphosis (T4-T12) was derived from supine lateral

The Bilhaut-Cloquet procedure for Wassel types III, IV and VII thumb duplication.

Five cases of Wassel types III, IV and VII thumb duplication underwent a Bilhaut-Cloquet procedure. A stable and mobile metacarpophalangeal joint was achieved in all cases. Interphalangeal joint motion was limited but this joint was stable in all cases. The nail ridge in these thumbs was minimal. A strong, stable thumb of normal size and good appearance can result from the Bilhaut-Cloquet procedure. When one nail is 70% of normal width, a modified procedure using the whole of one nail will avoid the nail ridge, but the nail will still differ from normal.

Bilateral hands and feet postaxial polydactyly presenting with renal vascular anomalies.

The current Observation reports polydactyly of both the hands and feet in a 36 years old male cadaver that presented also with supernumerary renal vessels. The right kidney received the upper and lower supernumerary renal arteries from the abdominal aorta and was drained by the two renal veins. The anterior and posterior supernumerary renal veins opened separately into the inferior vena cava. Observations on the limbs revealed the presence of six digits in the four limbs. In the hands the extra digit contained two phalanges and rudimentary metacarpals that were pear-shaped. The pointed ends of

Distraction lengthening of the ulna in radial club hand using the Ilizarov technique.

Six children with radial club hand had distraction lengthening of the ulna using the Ilizarov technique at the mean age of 10 years. The mean lengthening achieved was 4.7 cm (46% of original ulna length). Complications included nocturnal pain, pin track infection and callus fracture or delayed union. Distraction lengthening of the ulna can enhance the ability to perform normal activities of daily living, such as reaching the perineum or driving a car, but complications are extremely common. The high rate of callus fracture in this series reinforces the need for regular radiographic review duri

Long-term results of radioscapholunate fusion following fractures of the distal radius.

Fifteen patients with radioscapholunate (RSL) fusion for traumatic lesions of the radiocarpal junction, whose short-term results have been previously reported, were reassessed after an average follow-up time of 8 years. Five patients had undergone wrist fusion because of non-union or early progressive arthritis. Of the ten wrists with retained mobility, eight continued to function satisfactorily. Two wrists were painful for reasons other than secondary midcarpal arthritis. Patient satisfaction was comparable in both groups with the wrist score better for wrists with residual motion. The surviv

Gunshot wounds to the extremities in children and adolescents.

Thirty-seven patients from 8 months to 17 years of age were evaluated prospectively and retrospectively after sustaining gunshot wounds to the extremities. This review supports the belief that higher velocity weapons and shotgun-wound injuries cause greater morbidity, with increased length of hospital stay and increased cost of care. Late growth abnormalities were related to recognized initial physeal injury and not to unrecognized physeal injury. Immediate neurologic deficits usually resolved partially or fully, without surgical treatment.

Long-term results following proximal row carpectomy.

The functional outcome, patient satisfaction, and x-ray film status for 23 wrists were examined an average of 6 years following proximal row carpectomy. Surgery was performed for Kienbock's disease, scapholunate dissociation with periscaphoid arthritis, and scaphoid nonunion not suitable for bone grafting. Twenty patients were satisfied with functional performance and pain relief. Wrist flexion-extension arc averaged 74 degrees, or 61%, of the opposite wrist at a later examination. Grip strength, corrected dor dominance, averaged 79% of the opposite side, reflecting an average improvement of 1

Posterior interosseous nerve palsies.

One hundred seventy patients with radial nerve disorders were reviewed at the Louisiana State University Medical Center over a 15-year period. Of these, 32 had involvement of the posterior interosseous nerve exclusively. Findings included weak wrist extension with a radial drift, inability to extend the fingers, paralysis of thumb extension, and weak thumb abduction. Causes included entrapment at the arcade of Froshe (14 patients), laceration (6 patients), fracture (6 patients), compression or contusion (3 patients), and loss associated with tumor (3 patients). The ratio of men to women was 2:

Acrometastases. A study of twenty-nine patients with osseous involvement of the hands and feet.

We studied the records of twenty-nine patients with forty-one metastatic lesions of the hand or foot. In five patients the lesions mimicked a benign condition, and inappropriate treatment was given. In eleven patients the acrometastases were the first indication of malignant disease, and four other patients had no known primary malignant lesion. Amputation of a phalanx, digit, or ray is recommended for most solitary phalangeal, metacarpal, or metatarsal lesions when the expected period of survival of the patient exceeds a few months.
