Prophylaxis against pulmonary embolism in patients undergoing total hip arthroplasty.

BACKGROUND: The optimal method of prophylaxis for the prevention of pulmonary embolism in patients undergoing total hip arthroplasty remains controversial. Guidelines appear to be contradictory. The purpose of the present study was to examine whether a best prophylactic agent exists for the prevention of postoperative pulmonary embolism and whether the type of anesthesia affects the rates of pulmonary embolism. METHODS: From 2001 to 2008, a total joint registry from a nationwide health maintenance organization was evaluated to determine the rates of pulmonary embolism, fatal pulmonary embolis…

Pelvic deformity influences acetabular version and coverage in hip dysplasia.

BACKGROUND: Although a wide variety of acetabular deformities in developmental dysplasia of the hip (DDH) have been reported, the morphologic features of the entire pelvis in DDH are not well characterized and their correlation with acetabular deformity is unknown. QUESTIONS/PURPOSES: We determined whether there was a rotational deformity of the entire innominate bone, and if so, whether it related to acetabular version and coverage. PATIENTS AND METHODS: We examined the morphologic features of the pelvis using CT for 50 patients with DDH (82 hips). Forty normal hips were used as controls. T…

Long-term outcome after traumatic anterior dislocation of the hip.

INTRODUCTION: Traumatic anterior dislocation of the hip joint is rare. Additional injuries to the hip due to dislocation are even more infrequent. Outcome is limited by osteoarthritic joint degeneration or the occurrence of avascular necrosis of the femoral head. METHOD: Anterior hip dislocation occurred in ten of 100 patients with traumatic hip dislocations (8 men, mean age: 43, 22-62years) at two major trauma centres, between January 2001 and December 2008. Four patients had impaction fractures of the femoral head and three patients had fractures of the anterior acetabular wall. One patient…

Neuropathic pain following hip resurfacing due to a transneural suture.

A 60-year-old man developed severe neuropathic pain and foot-drop in his left leg following resurfacing arthroplasty of the left hip. The pain was refractory to all analgesics for 16 months. At exploration, a PDS suture was found passing through the sciatic nerve at several points over 6 cm and terminating in a large knot. After release of the suture and neurolysis there was dramatic and rapid improvement of the neuropathic pain and of motor function. This case represents the human equivalent of previously described nerve ligation in an animal model of neuropathic pain. It emphasises that when…

Management of acetabular bone loss in revision total hip arthroplasty.

Most acetabular revisions can be managed with a hemispherical component with screw fixation. Areas of segmental bone loss that preclude acetabular component stability may be managed with structural allograft or second-generation porous metal augments. Acetabular cages have a limited application but can be a useful tool in the management of massive bone loss and pelvic discontinuity.

[Intermediate outcomes of alumina-on-alumina bearing total hip arthroplasty].

OBJECTIVE: To explore the intermediate outcomes of third-generation alumina-on-alumina total hip arthroplasty (THA). METHODS: A consecutive series of 165 primary alumina-on-alumina total hip arthroplasties were performed in 135 patients. Three patients died and 13 patients lost follow-up so that a total of 119 patients (146 hips) were available for study. The mean patient age was 53.4 ± 11.0 years old (range: 26 - 79). Hydroxyapatite. (HA)-coated press-fit acetabular cups were used in all cases. At femoral side, 123 cases were implanted with HA-coated uncemented stems and 23 cases implanted w…

Delayed fracture of a zirconium head after total hip arthroplasty.

We present a case of fracture of a zirconium head after a cemented total hip arthroplasty. The fracture occurred 81 months after the index operation without any history of trauma. The patient was thin, not participating in sports, and the zirconium head had 0-mm neck length. Preoperative radiographs demonstrated aseptic loosening of both components and significant polyethylene wear. Fracture pattern was unusual as the major fragment was half of the head. A revision was performed to a cementless arthroplasty. The authors have obtained the patient's informed written consent for print and electro…

Modern total hip arthroplasty in patients younger than 21 years.

Total hip arthroplasty (THA) is not commonly performed in adolescents. However, it may be the only option for pain control with continued mobility for advanced disease. We report our experience with modern alternative-bearing THA in patients younger than 21 years. Twenty-one THAs (18 patients) were followed. Preoperative and postoperative Harris hip scores were recorded, and radiographs were reviewed. Average follow-up was 49 months (range, 25-89). Underlying etiology was chemotherapy-induced osteonecrosis (33%), steroid-induced osteonecrosis (29%), sickle cell disease (24%), and chronic dislo…

Achilles allograft reconstruction for recurrent dislocation in total hip arthroplasty.

Recurrent dislocation secondary to posterior soft tissue deficiency is a challenging complication of total hip arthroplasty. We describe the use of an Achilles allograft sling to improve hip stability. Eight patients treated with the sling were followed an average of 5 years. Seven patients had no recurrent instability and good postoperative range of motion. One graft failed in a patient with a neuropathic hip. Cadaveric biomechanical testing was also performed to investigate the stiffness and torque to failure of the sling in 6 specimens. Allograft slings can be used to improve hip stability.…

Radiographic changes of the femoral neck after total hip resurfacing.

INTRODUCTION: Significant femoral neck narrowing following hip resurfacing arthroplasty has been observed. Several factors contributing to the physiopathology of femoral neck narrowing have been suggested. The aim of this study was to evaluate the femoral neck radiographic changes observed after hip resurfacing at a minimum follow-up period of 5 years and to determine their causes. PATIENTS AND METHODS: We conducted a prospective study of 57 hip resurfacing arthroplasties performed in 53 patients (30 men, 23 women) of mean age 49.2 years (32-65) at surgery. These patients were clinically revi…

Transient synovitis of the hip: Ultrasound appearance. Mini-pictorial essay.

We describe the ultrasound (US) appearance of transient synovitis. Transient synovitis of the hip typically occurs in 3 to 8-year-old children. The onset is clinically characterized by acute hip pain and limp with limited joint mobility, and the leg is usually held in a position of flexion and external rotation to avoid pain. US image is characterized by joint effusion in the hip joint anterior recess, as described in the literature. Our experience confirms the importance of the technique with which the US examination is performed. In order to obtain the best diagnostic information the hip mus…

Radiographic structural abnormalities associated with premature, natural hip-joint failure.

BACKGROUND: Significant controversy exists regarding the causes of premature, natural hip-joint failure. Identification of these causes may guide future investigations targeting prevention of this disorder. The aims of this study were to: (1) determine and characterize structural abnormalities associated with premature, natural hip-joint failure, and (2) analyze disease progression in the contralateral hips of patients with femoroacetabular impingement deformities. METHODS: We analyzed 604 patients (710 hips) from three different medical centers who underwent primary total hip arthroplasty at…

Anterior approach according to Ganz--surgical dislocation of hip joint.

The surgical technique of an anterior approach to the hip joint according to Ganz is described. The procedure preserves the deep branch of the medial circumflex artery, which, combined with greater trochanter osteotomy, allows for safe hip joint dislocation without a risk of avascular necrosis of the femoral head. This approach makes it possible to inspect the femoral head and the entire acetabulum. It is suitable for the treatment of early stages of coxarthrosis.

Pelvic dissociation in revision total hip arthroplasty: diagnosis and treatment.

Pelvic dissociation is a distinct but uncommon condition, which occurs in association with total hip arthroplasty, in which the superior aspect of the pelvis is separated from the inferior aspect by fracture. Because radiodense implants and cement can obscure pelvic discontinuity on plain radiographs, not all dissociations can be diagnosed preoperatively; therefore, a high index of suspicion for this condition should be maintained. In selected patients, CT angiography may be indicated. Successful treatment requires achieving initial stability of the socket, establishing conditions for long-ter…

Use of morphometry to quantify osteolysis after total hip arthroplasty.

BACKGROUND: Progressive osteolysis threatens the longevity of hip arthroplasties and radiographic review is recommended. Measurement of osteolytic lesions in a clinical setting has not been achieved easily in the past. Other radiologic investigations provide accurate information but cost and risk to the patient prohibit their use in routine review. QUESTIONS/PURPOSES: We developed a simple, reliable tool to be used in hip arthroplasty review to quantify osteolytic changes seen on plain film radiographs. METHODS: A morphometric grid was developed and tested on simulated and actual osteolytic …

Senescence in cells in aseptic loosening after total hip replacement.

Particle-induced osteolysis is a major cause of aseptic loosening after total joint replacement. The purpose of the current study was to evaluate cellular senescence of macrophages and giant cells in patients with aseptic hip loosening by determination of SA-β-Gal (SA-β-galactosidase), a reliable and frequently used indicator of cellular senescence. The level of senescence in capsule and interface membranes was significantly higher in patients with aseptic loosening in comparison to specimens from patients without aseptic loosening. Using Spearman's rank correlation, we found that the expressi…

Avoiding varus malreduction during cephalomedullary nailing of intertrochanteric hip fractures.

We describe a technique for preventing varus malreduction of intertrochanteric hip fractures during fixation with a cephalomedullary nail. When the entry reamer guidewire is positioned in the fracture line, passage of the entry reamer may simply push the proximal fragment medially and the distal fragment laterally rather than ream the accurate entry path. Subsequent nail insertion will result in a varus deformity of the proximal femur. The simple technique described in this report uses a brief period of over-distraction to wedge the entry reamer guidewire against the lateral edge of the proxim…

Functional results of isolated femoral revision of hip resurfacing arthroplasty.

BACKGROUND: Conversion of a failed femoral resurfacing component is reportedly a straightforward procedure; however, little has been published regarding the functional results following revision. Our primary aim was to compare the functional results for a group of patients who had had isolated femoral component revision after a failed hip resurfacing with those for a group of patients with a surviving hip resurfacing. Our secondary aim was to identify whether the mode of failure affects functional outcome. METHODS: Between 1997 and 2002, data were prospectively collected on 5000 Birmingham Hi…
