Forty-seven (39%) of 120 patients with presumably operable lung cancer were found to have metastatic tumor in mediastinal lymph nodes by mediastinoscopy. Unnecessary thoracotomy was avoided in these 47 patients. Results of anterior mediastinal node biopsy were positive in 16 (38%) of 42 patients with primary tumors of the left lung who were evaluated by anterior mediastinoscopy without resection or division of costal cartilage. A case of gastric and lymph node extramedullary plasmacytoma is reported in a young Algerian female. Treatment with cyclophosphamide was ineffective, but rubidazone, followed by a maintenance therapy with prednisone, procarbazine, and vinblastine induced a complete remission. Immunofluorescence studies and biosynthesis experiments in vitro showed that the tumor cells produced and secreted IgA-kappa molecules, whereas no monoclonal immunoglobulin could be found in the serum and gastric juice.

Nonmelanomatous skin cancer usually occurs in fair-skinned persons on sites chronically exposed to sunlight. In a series of 35,000 patients with skin cancer seen at M. D. Anderson Hospital and Tumor Institute, 1,208 patients (13%) had squamous cell skin cancer of the extremities. A histopathological study of 703 surgical specimens from patients with adenocarcinoma of the rectum revealed invasion of veins by primary growth in almost 52 per cent. Protein pattern was studied using discontinuous polyacrylamide gel electrophoresis in 53 adenocarcinomas, 13 fibroadenomas and 66 samples of mammary tissue adjacent to neoplasias. Over a 28 year period, 77 patients with early stage anaplastic seminoma of the testis were treated by orchiectomy and lymphatic irradiation at three Army medical centers. With a median follow-up of 97 months, the 10 year actuarial survival is 96% of Stage I patients and 87% for Stage II patients. The length and radiologic type of tumor, esophageal axis and azygogram were examined in 208 patients with carcinoma of the thoracic esophagus who underwent esophageal resection from 1965 to 1975. An histologic examination was performed on all resected specimens. These examinations were used as diagnostic aids to determine the resectability of esophageal carcinoma before surgery. Resection results from 417 consecutive patients operated on between January 1, 1964, and December 30, 1969, were analyzed in March, 1976. This period was chosen to allow a five-year follow-up. Experience with 47 patients with head and neck carcinoma operated after preoperative megavoltage irradiation of about 3 000 rads over 3 weeks is reported. The 3-year survival for the series was 25/47 (53%), for the cases of Stages I--II 13/19 (68%) and Stages III--IV 12/28 (43%). Neoplastic cells could not be detected in 18% of the cases at operative biopsy. A case of sarcoidosis of the mediastinal lymph nodes with concomitant teratoembryonal carcinoma of the testis is presented. It is important to differentiate these entities from mediastinal metastases of testicular tumors and testicular sarcoidosis, respectively. In the past 18 years 34 patients (21 males and 13 females) with carcinoma of the tongue were treated in the author's clinic. Patients with T1N0 lesions were treated satisfactorily only with irradiation. Patients with advanced disease, such as T2-T4 with metastases, were classified into 2 groups. The records of 112 patients treated at the Ochsner Foundation Hospital with the diagnosis of bronchogenic carcinoma were reviewed. A new concept for defining the location of central versus peripheral tumors is presented. Criteria important in selection of patients for whom mediastinoscopy is likely to be helpful are cell type, location (peripheral versus central), and radiographic evidence of mediastinal metastasis.
