Gestational exposure to cigarette smoke imperils the long-term physical and mental health of offspring.

BACKGROUND: In this study, we sought to understand whether prenatal exposure to cigarette smoke would be associated with increased offspring hospitalization through age 22 years for various physical and mental health diagnoses. METHODS: We used multivariate logistic regression to investigate the relationship between gestational exposure to cigarette smoke and offspring hospitalization for physical and mental health conditions based on International Classification of Diseases (ICD; World Health Organization) diagnoses. RESULTS: When controlling for parental psychiatric status, maternal somati

Towards a renewed public health regulatory and surveillance role in water, sanitation and hygiene (Editorial).

Evidence from the burden of disease studies indicates that improving water, sanitation and hygiene (WSH) is a primary disease prevention measure. It prevents 57% of diarrheal deaths, contributes to prevention and management of neglected tropical diseases, and is the first line of defense against Cholera. Moreover, water and sanitation (W&S) services and hygiene contribute to programme initiatives including new-born, child and maternal health; infection prevention and control; universal health care; antimicrobial resistance; and health emergencies. Access to W&S services is recognized by the Un…

INTRODUCTION: Policy directions in the health sector are based on research which provides insight into implemented actions and helps to formulate improvement recommendations for health. In Cameroon, an innovative experience of anthropological research on newborn health was followed by a process of appropriating findings by the Ministry of Public Health aiming at improving the quality of care. We suggest reporting this Cameroonian experience of interdisciplinary collaboration and involved research. METHOD: Coinciding with the anthropological study on newborn care in hospital maternities and at

The impact of national health insurance on the utilization of health care services by pregnant women: the case in Taiwan.

OBJECTIVES: Substantially increased funding for health care services occurred in Taiwan after the implementation of a national health insurance plan in 1995. This study attempts to examine the impact of this national health insurance plan on the utilization of prenatal and intrapartum care services. METHODS: Nationally representative surveys of all pregnant women in Taiwan in 1989 (1,662 participants) and in 1996 (3,626 participants) were included in the analysis. We first compared the distribution of birth characteristics between the two surveys. We then calculated the rate of utilization of

[Influence of smoking during pregnancy on children's health--overview of epidemiologic studies].

The association between maternal smoking and retarded maternal condition and birth outcome is well know. Smoking during pregnancy increases risk of spontaneous abortion, placenta previa, abruptio placenta, preterm premature rupture of membranes, stillbirth, preterm delivery and sudden infant death syndrome. The recently conducted studies also indicate that prenatal exposure to tobacco smoke is a risk factor for respiratory infections, asthma, allergy, childhood cancer, and it has neurobehavioral consequences regarding children's health. The risk for most of these conditions has been found to i

Maternal body mass index in early pregnancy and offspring mortality up to early adulthood: A nationwide cohort study.

BACKGROUND: Obesity is increasingly common among women of childbearing age worldwide. Maternal obesity during pregnancy may have long-term implications for offspring health, but its association with offspring mortality remains not well understood. In this nationwide cohort study, we examined the associations of maternal body mass index (BMI) in early pregnancy with all-cause and cause-specific mortality in offspring. METHODS AND FINDINGS: We included 2,606,684 live births in Sweden during 1982-2014 (except for 1990 and 1991) and followed them from birth until death, emigration from Sweden, or

The Autism Spectrum Disorder and Its Possible Origins in Pregnancy.

Autism Spectrum Disorder (ASD) belongs to the group of neurodevelopmental disorders, and has a high prevalence, affecting 1 in 100 children according to data from the World Health Organization (WHO). To be diagnosed with ASD, the child must have persistent deficits in communication and social interactions, and restricted and repetitive patterns of behavior, interests, or activities. Despite its prevalence, the etiology of ASD is still uncertain, with multifactorial characteristics, including those associated with the gestational period, where maternal exposure to biological, chemical, or physi

Multimodal Early Birth Weight Prediction Using Multiple Kernel Learning.

In this work, a novel multimodal learning approach for early prediction of birth weight is presented. Fetal weight is one of the most relevant indicators in the assessment of fetal health status. The aim is to predict early birth weight using multimodal maternal-fetal variables from the first trimester of gestation (Anthropometric data, as well as metrics obtained from Fetal Biometry, Doppler and Maternal Ultrasound). The proposed methodology starts with the optimal selection of a subset of multimodal features using an ensemble-based approach of feature selectors. Subsequently, the selected va

A crucial role for maternal dietary methyl donor intake in epigenetic programming and fetal growth outcomes.

The fetal origins of health and disease framework has identified extremes in fetal growth and birth weight as factors associated with the lifelong generation of chronic diseases such as obesity, diabetes, cardiovascular disease, and hypertension. Maternal nutrition plays a critical role in fetal and placental development, in part by providing the methyl groups required to establish the fetus's genome structure and function, notably through DNA methylation. The goal of this narrative review is to describe the role of maternal dietary methyl donor (methionine, folate, and choline) and cofactor (

Safe abortion: a woman's right.

Complications of induced abortion sadly remain significant causes of maternal mortality and morbidity around the world, but only in countries that do not provide access to safe abortion services. This article presents a brief account of how high maternal mortality from induced abortion became history in the UK and the dire consequences to women's health that unsafe abortion still has in many countries of the world. It gives a brief overview of the methods available to evacuate the uterus, with particular reference to manual vacuum aspiration. The status of the law in different countries is dis

The Guadeloupean perinatal mortality audit: process, results, and implications.

In 1983, we implemented an ongoing perinatal mortality audit in Guadeloupe to identify factors underlying the current level of poor perinatal outcome and to stimulate increased provider motivation. The audit was part of an active research approach to document the specific nature of the perceived perinatal health problem, to modify provider behavior through the use of this acquired information, and, ultimately, to decrease avoidable perinatal deaths. We investigated all 320 perinatal deaths in Guadeloupe during 1984 and 1985. Nearly one-half of the perinatal deaths reviewed were determined to b

Differences in maternal lineages of New Zealand Black mice defined by restriction endonuclease analysis of mitochondrial DNA and by expression of maternally transmitted antigen.

Two substrains of New Zealand Black (NZB) mice have been compared with respect to expression of a maternally transmitted cell surface antigen, Mta, defined by cloned cytolytic T cells, and for restriction enzyme polymorphisms of mitochondrial DNA (mtDNA). These independent assays of maternal cytoplasmic inheritance provide strong evidence for genetic contamination of the NZB/BlPt substrain (NZB/Bl mice from Michael Potter's separate colony at the National Institutes of Health), in which the typical NZB immunologic abnormalities are at least partially ameliorated. The decisive data are the rest

Health Care Access Barriers Among Reproductive-Age Women in East Africa: Development and Validation of Machine Learning Prediction Models Using DHS Data.

BACKGROUND: The World Health Organization advises that every nation should take responsibility for guaranteeing access to health care services as a basic human right. However, due to financial constraints and geographical hurdles, only around half of the population in Africa has access to contemporary health care services. OBJECTIVE: This study aimed to predict barriers to health services and associated factors among reproductive-aged women in East Africa using machine learning algorithms and identify the best-performing predictive model. METHODS: Analysis of secondary data from 6 East Afric

Maternal-Fetal Crosstalk in Cardiovascular Programming: Linking the Intrauterine Environment to Lifelong Disease Risk.

Cardiovascular disease (CVD) is the leading cause of morbidity and mortality worldwide, accounting for a substantial proportion of global deaths. Increasing evidence indicates that cardiovascular susceptibility is shaped during fetal development, where the intrauterine environment plays a critical role. Maternal-fetal crosstalk, mediated largely through placental function, coordinates the transfer of metabolic, endocrine, and immune signals that are essential for normal cardiac and vascular development. Disruptions in maternal physiology-including metabolic disorders, hypertensive conditions,

Microchimerism in the human brain, quantitative assessment and single nuclei profiling establish cell types and diversity.

Bi-directional maternal-fetal exchange during pregnancy creates a long-term microchimerism (Mc) legacy in both individuals, but its presence and cellular fate in human brain are largely unknown. We studied surgically resected epilepsy brain specimens with targetable maternal polymorphisms using polymorphism-specific quantitative PCR. Maternal Mc was prevalent, detectable in 70% of patients, and often at striking quantities spanning temporal, frontal, parietal, and hippocampal regions. Next, we employed single nucleus RNA profiling using cellector , a genetic demultiplexing tool designed to det

Maternal Diabetes and Attention Deficit/Hyperactivity Disorder Association: A Narrative Review on Emerging Evidence, Research Gaps and Future Directions.

Maternal diabetes includes pre-gestational diabetes mellitus (PGDM) and gestational diabetes mellitus (GDM), with increasing prevalence globally. Attention Deficit Hyperactivity Disorder (ADHD) is a common childhood neurodevelopmental disorder. This narrative review synthesizes epidemiological and mechanistic evidence published mainly since 2023 to explore the association between maternal diabetes and ADHD in offspring. Pooled data show that maternal diabetes increases offspring ADHD risk by approximately 30%, with PGDM showing a stronger association than GDM, and poorly controlled diabetes or
