PURPOSE OF REVIEW: Adolescents with neurodevelopmental disorders (NDDs) often display challenging behaviors such as hypersexuality, severe irritability, and aggression. This review emphasizes current management strategies, focusing on the evaluation of problematic behaviors and considering both pharmacological and nonpharmacological options, as well as their level of evidence in the current literature.
RECENT FINDINGS: Hypersexuality in adolescents with NDDs may result from conditions such as precocious puberty, polycystic ovary syndrome, neurologic disorders, trauma, or medication effects. M

Inflammatory bowel disease (IBD), encompassing ulcerative colitis and Crohn's disease, is a group of chronic, immune-mediated disorders of the gastrointestinal tract that present substantial clinical challenges owing to their complex pathophysiology and tendency to relapse. A treat-to-target approach is recommended, involving iterative treatment adjustments to achieve clinical response, reduce inflammatory markers and achieve long-term goals such as mucosal healing. Lifelong medication is often necessary to manage the disease, maintain remission and prevent complications. The therapeutic lands

Carcinoid Heart Disease (CaHD) is defined as the constellation of all cardiac manifestations that occur in patients with carcinoid tumors. Cardiac manifestations are generally due to the paraneoplastic effects of vasoactive substances secreted by carcinoid tumors. These primarily cause cardiac valve dysfunction and resultant heart failure. Successful management of patients with CaHD requires a multidisciplinary team to address both the classical manifestations of carcinoid syndrome, as well as the additional manifestations of cardiac dysfunction. While the cornerstone of medical management for

OBJECTIVE: Persistence of hyperparathyroidism (HPT) after renal transplantation leads to undesirable outcomes such as increase in cardiovascular events, graft dysfunction, and increased mortality. Options for therapy include medical management with calcimimetic or operative management. The present study was undertaken to evaluate the natural history of HPT after renal transplantation and to determine risk factors for persistent HPT in the era of calcimimetic.
DESIGN: The study is a retrospective review of data from 74 consecutive patients who underwent renal transplantation at our institution

The opioid crisis is more severe in the United States than in any other country. This may be due, in part, to a cultural problem related to pain: Americans have come to expect quick, easy, physician-provided pain relief. Pharmaceuticals can neither cure injuries nor correct the underlying cause of any chronic musculoskeletal condition. Fortunately, people who regularly exercise have less pain, and guidelines for the management of painful chronic conditions already recommend exercise therapies over passive care. This suggests that self-care approaches emphasizing exercise are the logical, lowes

BACKGROUND: Invasive coronary function testing (CFT) is indicated in patients with refractory angina with non-obstructed coronary arteries (ANOCA). Despite this, questions remain regarding patient selection for testing, safety and integration of CFT within clinical pathways and the impact that endotyping has on long term management of these patients. This study aims to investigate the safety of CFT in patients presenting with anginal chest pain, the prevalence of ANOCA endotypes in the tested population, and the impact of CFT on prescribed medical therapy and secondary care resource utilisatio

BACKGROUND: Mild autonomous cortisol secretion (MACS) has an increasing prevalence with age and is associated with cardiometabolic comorbidity and increased mortality. Current guidelines recommend the majority of patients with MACS be managed conservatively, with select people referred for laparoscopic adrenalectomy. There are currently no licensed medications for MACS.
OBJECTIVE: We aimed to systematically review the literature to map out the current available evidence on the use of medical therapy in the treatment of MACS and to assess the research gap. Additionally, we sought to establish

BACKGROUND: Parkinson's disease is a primary neurological dysfunction syndrome in which the body's natural production of dopamine is reduced. Therapeutic intervention for this disease primarily includes medications that increase the body's natural production of dopamine. These medications can have side effects, requiring constant titration based on the patient's symptoms.
CASE PRESENTATION: We report a case study of a 75-year-old man diagnosed with Parkinson's disease who had a profound acute exacerbation of psychological symptoms following modifications to his dopaminergic treatment regimen.

Natural disasters and pandemics create major challenges for public health systems, requiring efficient medical responses under constrained conditions. Ensuring comprehensive healthcare in disaster-stricken areas is critical, as conventional medical resources may be insufficient in some settings. Japanese traditional (Kampo) medicine (JTM) offers a potential solution for enhancing healthcare delivery in these scenarios. This work was motivated by the desire to facilitate the international dissemination of the integration of Western medicine and traditional medicine. Japan has integrated JTM, in

BACKGROUND: Neurocysticercosis is particularly severe when affecting the subarachnoid space. While antiparasitic therapy effectively controls the infection, it can lead to significant complications, especially in subarachnoid neurocysticercosis (SUBNCC). This study aims to characterize a cohort of patients with SUBNCC, with a focus on their clinical course depending on therapeutic interventions.
METHODS: We conducted an observational, retrospective study involving patients diagnosed with SUBNCC at a tertiary hospital between November 1985 and July 2022. The primary endpoint was to delineate t

Cerebral venous sinus thrombosis (CVST) is an uncommon condition accounting for 0.5-1% of all strokes. It occurs more commonly in women, particularly in the age group of 20-40 years of age due to pregnancy and oral contraceptive use. Systemic anticoagulation is recommended as first line treatment but 10-20% of patients deteriorate despite medical treatment and require surgical or endovascular interventions. We summarize a 41-year-old female with a past medical history of acute disseminated encephalomyelitis who presented with headaches and worsening mental status. Further workup confirmed infe

INTRODUCTION: This resource is a web-based and self-contained self-learning module (SLM) on binge-eating disorder (BED) that runs utilizing Articulate software. Certain skills and knowledge are required to both interview and manage patients diagnosed with the disorder. Because BED may be encountered by medical students, this module can help inform the learning process.
METHODS: Software was utilized to develop an SLM for third-year medical students utilizing video clips demonstrating the evolution of a clinical BED case from presentation to follow-up over 10-12 weeks. The focus is on differen

PURPOSE: Extracranial arterial dissections (EAD) are uncommon in the general population but represent a relatively frequent cause of ischemic stroke in young adults. Most EAD have a favorable prognosis and are managed conservatively with antithrombotic therapy, with current guidelines supporting either antiplatelet or anticoagulant treatment. Endovascular stenting is reserved for selected high-risk cases, and this article reports a multicenter experience with this approach and reviews the available literature.
MATERIALS AND METHODS: Eighty-four EAD patients who underwent stenting were include
