For 273 patients evaluated for disseminated Mycobacterium avium complex infection, a total of 1,047 mycobacterial blood cultures (MBCs) were submitted; the M. avium complex was recovered from 140 (13%) of the specimens. Results for the paired MBCs were highly concordant: in 392 of 462 (85%) culture sets, both MBCs were negative, in 53 of 462 (11%) sets, both MBCs were positive, and in only 17 of 462 (4%) sets was one culture positive and the other negative. Rationally designed combinations of rifampicin/thiacetazone plus isoniazid and/or ethambutol are highly effective in the treatment of patients (including HIV-pos.) infected with multiply resistant mycobacteria of the M. avium complex (MAC). Clinical results are very promising. The high efficacy of these combinations is due to the synergistic potentiation of single drug activities. As soon as rifabutin is marketed it should replace rifampicin in the combination treatment of patients with highly rifampicin-resistant MAC bacteria.

The possibility of synergy between immunotherapy with recombinant interferon-gamma (IFN-gamma) and chemotherapy with rifampin (RMP) and dapsone (DDS) against Mycobacterium leprae was examined in nude mice. Activation of the respiratory metabolic channel (F.O.R.), responsible in part for the bactericidal effect, was measured in vitro by a chemoluminescence test on circulating phagocytes. Sera from patients with lepromatous and tuberculoid leprosy were examined in immunoblot assays for antibodies to Mycobacterium tuberculosis culture filtrate antigens. Antibodies to 30- and 31-kilodalton proteins were present in 88 and 81%, respectively, of 16 patients with lepromatous disease and absent in 16 patients with tuberculoid disease. Antibodies to a 32-kilodalton protein were found in 12 and 38% of lepromatous and tuberculoid patients, respectively. These reactivities may be useful for distinguishing lepromatous and tuberculoid leprosy.

Since 1983, primary resistance of M. leprae to DDS and rifampicin has been evaluated in new cases of lepromatous leprosy observed in the Cap-Verde region in Senegal. The beta-lactamases from the 3 biovariants of M. fortuitum were compared on the basis of substrate profiles, susceptibility to enzyme inhibitors, and inducibility in the presence of selected beta-lactams.
