Proportion of Colonized Person-time With Streptococcus pneumoniae and Duration of Colonization With Individual Serotypes in Young Peruvian Children.

Few studies have characterized the proportion of time a child spends with nasopharyngeal pneumococcal colonization, thus at risk for pneumococcal disease. In this prospective study conducted in the early 7-valent pneumococcal conjugate vaccine implementation period, young Peruvian children exhibited pneumococcal colonization during half of the observed person-time. Specific serotypes exhibited variable colonization duration, ranging from 17.5 to 100.3 days.

A unique case of herpetic keratitis manifesting as recurrent preseptal cellulitis.

Preseptal cellulitis is typically caused by nasopharyngeal bacteria and can usually be treated with empiric antibiotics. We present a case of herpes simplex virus (HSV) 1 keratitis manifesting as recurrent and treatment refractory preseptal cellulitis. This is a rare presentation that is infrequently reported in the literature. Due to the potential for permanent vision loss, it is important to consider herpes viruses as an etiology in immunocompromised patients with preseptal cellulitis not responding to standard treatment.

Anatomy and physiology of velopharyngeal closure and insufficiency.

The velopharynx is a complex structure that is responsible for separation of the oral and nasal cavities during speech production and swallowing. Incompetence of this mechanism can lead to hypernasality, with nasal air emission and incomprehensible speech, as well as nasopharyngeal regurgitation. There can be a significant social stigma associated with velopharyngeal dysfunction, and surgical treatment can be curative in many cases. Knowledge of the normal anatomy and physiology of the velopharyngeal complex is essential when planning for surgical repair.

Docetaxel-induced nail toxicity: a case of severe onycholysis and topic review.

Docetaxel is a commonly-used anti-cancer chemotherapeutic agent given its efficacy in a large variety of solid tumors. It is associated with various adverse effects one of which is nail toxicity. We report a case of severe onycholysis as a result of treatment with docetaxel in a patient who suffered from metastatic nasopharyngeal carcinoma. The case report will be followed by a discussion on the possible mechanism and preventive strategies for taxane-induced nail toxicity.

Extragonadal teratomas presenting at birth and originating from 3 different sites--a report of 3 cases.

Three cases of extragonadal teratomas presented at birth and all the cases arisen from three separate sites are reported in the present study. A huge sacrococcygeal teratoma is being reported and the other two retroperitoneal and nasopharyngeal teratomas, are reported for the rarity of their location in neonatal period. Teratomas of all three babies were mature benign on histopathology and surgical removal sufficed as mode of treatment. No recurrence was noted on follow-up.

Right to left intracardiac shunt: a unique presentation of metastatic cardiac disease.

A 54 year old man presented with signs and symptoms of severe hypoxemia. He had a history of a nasopharyngeal epidermoid carcinoma that was thought to have been totally eradicated 2 years earlier. He was found to have a right to left interatrial shunt through a patent foramen ovale due to tricuspid valve obstruction by a solitary cardiac metastasis. No other such reported case of a metastatic cardiac lesion could be found.

En-bloc irradiation of tumours of the head and neck and their lymphatics. II. Early results and side effects.

The results and side effects of en-bloc irradiation of ear, nose and throat tumours and their lymphatics showed that the technique had been successful in nasopharyngeal carcinoma, but poor in carcinoma of the hypopharynx. Marked early and late radiation side effects were relatively common in high age and with certain tumour sites, whereas no correlation could be demonstrated with total absorbed dose, fractionation, cumulative radiation effect or major surgery.

Impact of whole-herd vaccination on a caprine coxiellosis outbreak: a longitudinal study of Coxiella burnetii shedding, serology, and host microbiota.

This longitudinal study investigated C. burnetii dynamics and the associated microbiota after vaccination in aborted and normal-delivery goats and bucks following a Q fever outbreak. Fecal, milk, vaginal, preputial, nasopharyngeal, environmental (qPCR and 16S rRNA), and blood samples (16S rRNA and serology) were analyzed. Before vaccination, over 70% of animals were seropositive, with qPCR positivity in all females and half of the males. Seroconversion remained complete until the second kidding season and reached approximately 90% by the third kidding season. After primary vaccination, a signi

Trends, levels, and projections of Head and Neck Cancer in China between 2000 and 2021: Findings from the Global Burden of Disease 2021.

Head and neck cancer (HNC), a condition that is both disfiguring and potentially fatal, has become a critical public health challenge. This study seeks to evaluate the trends in HNC burden and predict its future trajectory in China. Utilizing data from the 2021 Global Burden of Disease database, we focused on the incidence, mortality, and disability-adjusted life years related to lip and oral cavity, nasopharyngeal, and laryngeal cancers within the country. We analyzed changes in incidence and mortality rates using the estimated annual percentage change, age-period-cohort (APC) analysis, and d

Back to the future? Drastic drop in serotype 19A carriage in daycare centers within two years after a second switch to PCV13 in Belgium.

Pneumococcal conjugate vaccines (PCVs) reduce Streptococcus pneumoniae infection and carriage. After switching from PCV13 to PCV10 in 2015-2016, Belgium switched back to PCV13 in 2019. Building on our systematic monitoring of childhood nasopharyngeal carriage since 2016, here, we analyze the serotypes of S. pneumoniae and other pathogens in children attending daycare centers (DCCs) from 2018 to 2021. From the period of 2018-2019 to 2020-2021, we included a total of 2,741 nasopharyngeal swabs collected from children aged 6 to 30 months. We identified S. pneumoniae, Haemophilus influenzae, Morax

Respiratory Syncytial Virus Elicits Glycolytic Metabolism in Pediatric Upper and Lower Airways.

Respiratory syncytial virus (RSV) is the leading cause of lower respiratory tract viral infection in infants and causes around 60,000 in-hospital deaths annually. Emerging evidence suggests that RSV induces metabolic changes in host cells to support viral replication, presenting a potential target for therapeutic intervention. To investigate RSV-driven metabolic changes in situ, we combined positron emission tomography (PET), live-cell bioenergetics, and metabolomic profiling in the upper and lower airways of children. PET imaging revealed persistent, hyper-glycolytic regions in the lungs of R

Unveiling the uncommon: Cholesterol granuloma in the nasopharynx of a young patient.

BACKGROUND: Cholesterol granulomas are benign cystic lesions primarily found in the petrous apex of the temporal bone, often presenting diagnostic challenges due to their similarity to other lesions. Known for containing cholesterol-rich fluid, these granulomas can cause chronic inflammation and a range of symptoms, from mild headaches to severe neurological issues. CASE PRESENTATION: We report a case of a 14-year-old Iranian female with recurrent flu-like symptoms, nasal congestion, dysarthria, and obstructive sleep apnea. Examination revealed a cystic mass in the nasopharynx, and imaging co

Pilus-mediated co-aggregation with Lactobacillus crispatus increases meningococcal susceptibility to antimicrobial agents by interfering with microcolony formation.

BACKGROUND: Neisseria meningitidis asymptomatically colonizes the nasopharyngeal mucosa, but occasionally, the bacteria disseminate to cause sepsis and meningitis. In the epithelial cell layer, the pathogen co-colonizes with other resident inhabitants, such as Lactobacillus species that are part of the nasopharyngeal-oral microbiota. In this study, we investigated the interaction between lactobacilli and meningococci. RESULTS: We demonstrated that Lactobacillus crispatus strain MV24 can co-aggregate with meningococci, whereas all other Lactobacillus strains tested did not co-aggregate. The bi
