Tic douloureux, Parkinson's disease and the herpes connection.

Several cases encountered in psychiatric practice are described in which herpetic involvement of the trigeminal nerve appeared to be crucial to the pathophysiology of the patient's illness. A brief history of herpes is discussed in terms of the anatomy and physiology of the trigeminal nerve. Particular attention is given to the effects of intraneuronal herpes on gasserian ganglion and spinal nuclei discharge thresholds, as well as viral pathways directly impacting the substantia nigra. Evidence characterizing Parkinson's disease as one of several important manifestations of CNS herpes is cover

Gradual discontinuation of lithium augmentation in elderly patients with unipolar depression.

Recently, the therapeutic benefit of lithium augmentation in old age has come into question. These data, in light of the documented high incidence of side effects after lithium use in elderly patients, resulted in the design of a prospective, placebo-controlled lithium augmentation withdrawal study in elderly patients with unipolar depression. Twelve eligible geriatric patients (10 women and 2 men; mean [+/-SD] age, 76.2 +/- 5.7 years) with DSM-III-R unipolar depression receiving adjunct lithium therapy were randomized to receive continued lithium augmentation or matching placebo (withdrawal r

["Phobic postural vertigo". A further differentiation of psychogenic vertigo conditions seems necessary].

Due to the results of an interdisciplinary study on patients with vertigo as the chief symptom and on the background of psychodynamic theories concerning anxiety disorders the term of phobic postural vertigo (Brandt & Dieterich 1986) is discussed. It becomes obvious that phobic postural vertigo is a generalizing term which encompasses different forms of psychogenic vertigo. The authors plead for a more differentiated diagnosis and subgroup oriented classification of vertigo caused by psychiatric disorders.

[Paroxetine-induced neuroleptic malignant syndrome].

We report on a 22-year-old schizophrenic patient who attempted suicide and suffered an epidural hemorrhage. 16 days after the neurosurgical operation. After several weeks of treatment with promethazine, 1 day after intake of paroxetin he partially lost consciousness and developed extrapyramidal symptoms and vegetative disorders. Hyper-Ck-aemia up to 680 U/l was observed. Malignant neuroleptic syndrome (MNS) was diagnosed, which led to withdrawal of paroxetin and promethazine. He was put on dantamacrine and amantadine until the symptoms resolved. To date there have been few reports on MNS under

[Postpartum obturator nerve syndrome: case report and review of the nerve compression syndrome during pregnancy and delivery].

Compression of a peripheral nerve or nerve trunk can occur during pregnancy and delivery. The injury may be caused by the fetal head, the application of forceps, trauma or hematoma due to cesarean section, or improper positioning in leg holders. Often, no cause of the injury is found. The most common nerve compression syndromes during pregnancy and delivery are carpal tunnel syndrome, femoral neuropathy, and post partal foot drop. Obturator neuropathy, meralgia paraesthetica, tarsal tunnel syndrome, and syndrome of the rectus abdominis muscle occur less frequently. Symptoms, such as paraesthes

[Frequent problems with antibiotic chemotherapy of bacterial CNS diseases. Case reports and recommendations for initial treatment].

The initial antibiotic treatment of bacterial meningoencephalitis has major implications for the course of the disease. The current spectrum of infectious agents must be considered. Antibiotics have to reach sufficient concentrations in the blood and cerebrospinal fluid for a bactericidal action to occur at the site of infection. Frequent problems arising in this condition are: (1) the non-observation of listerial infections, (2) the use of antibiotics not sufficiently active against gram-positive bacteria in pneumococcal meningitis, (3) the development of meningitis following the use of cipro

Denial of hemiplegia: an investigation into the theories of causation.

A comprehensive study of denial of hemiplegia, with tests chosen to cover the varied proposed hypotheses, was undertaken. Twenty patients with denial of hemiplegia following acute CVA were studied. Their results were compared with those from two patient 'control' groups. A neurological examination and neuropsychological assessment were carried out in all patients and CT brain scan performed in 21 CVA cases. The findings are discussed in relation to previously proposed theories of causation concerning denial of hemiplegia which include personality factors, cognitive ability and the presence/abs

MRI and angiographic diagnosis of lupus cerebral vasculitis.

The authors present a patient with striking angiographic findings of systemic lupus erythematosus (SLE) cerebral vasculitis. Although lupus is often cited as part of the differential diagnosis for angiographically apparent cerebral vasculitis, true SLE cerebral vasculitis is rare in pathological and radiological series. The clinical spectrum and pathological findings of CNS lupus with particular attention to cerebral vasculitis are briefly discussed.

A case of delayed loss of facial nerve function after acoustic neuroma surgery.

We report on the uncommon delay of facial palsy in the course of a patient who underwent surgery for a left sided acoustic neuroma. Facial nerve function was preserved without clinical or electrophysiological deficit during the immediate postoperative phase and then lost almost totally over two days. This began on the seventh postoperative day. Recovery became visible under hypervolemic hemodilution and glucocorticoids a few days later and was complete after three months. Vasospasm in combination with neural edema might account for this uncommon delay of postoperative facial nerve deterioratio

Sensorimotor gating in boys with Tourette's syndrome and ADHD: preliminary results.

Deficits in sensorimotor gating, defined by prepulse inhibition (PPI), have been associated with subcortical dopaminergic overactivity in animal and clinical studies. Utilizing supraorbital nerve electrical stimulation, we produced adequate blink responses and measured decreases in amplitude resulting from electric prestimuli just above sensory threshold. Seven boys comorbid for attention-deficit hyperactivity disorder (ADHD) and a tic disorder had significantly reduced PPI, compared to 14 screened controls and seven boys with ADHD alone. If independently replicated, these results may reflect

Prenatal exposure to alcohol and marijuana: effects on motor development of preschool children.

Gross motor development of preschool children prenatally exposed to alcohol and marijuana was assessed as part of a longitudinal study. Most mothers in the study were light to moderate users and discontinued or decreased use of alcohol and marijuana after the first trimester of pregnancy. The women were of lower socioeconomic status, half of the sample was African-American, and most were single. Gross motor development was evaluated with balance and ball-handling items at 3 years. Balance items included walking on a line, walking on a balance beam, standing on one foot, standing on tiptoes, an

Ataxia without telangiectasia masquerading as benign hereditary chorea.

We report a nonconsanguineous family in whom two (of three) sons developed isolated chorea in early childhood, suggesting a diagnosis of benign hereditary chorea (BHC). However, cerebellar ataxia and oculomotor apraxia, without telangiectasia, subsequently developed. Chromosome analysis showed increased radiosensitivity in both brothers and translocations in the younger one. We conclude that ataxia with chromosomal instability may masquerade as BHC in some patients.

[X chromosomal bulbospinal neuropathy (X-BSN, Kennedy syndrome): an illness with repetitive triplet sequences. Case report, differential diagnosis and molecular genetics aspects].

X-chromosomal recessive bulbospinal neuronopathy (X-BNS, Kennedy's disease) is an important differential diagnosis of amyotrophic lateral sclerosis. We present the data of ten own patients along with a review of the literature on this uncommon disease which is caused by an expanded CAG-repeat in the androgen receptor gene. This mutation probably affects the transcription regulating activity of the androgen receptor in neurons. Signs and symptoms of X-BSN can be derived from partial insensitivity for androgens and a mixed, mainly motor neuronopathy. The clinical diagnosis is based on: 1. lower

Meningiomas of the cerebellopontine angle.

Meningiomas of the cerebellopontine angle (CPA) represent a clinically and surgically interesting entity. The opportunity of complete surgical excision and the incidence of impairment of nerval structures largely depend on the tumour biology that either leads to displacement of surrounding structures by an expansive type of growth or to an enveloping of nerval and vascular structures by an en plaque type of growth. As the origin and the direction of growth are very variable, the exact tumour extension in relation to the nerval structures and the tumour origin can be identified sometimes only a

Brain perfusion SPECT in infantile neuronal ceroid-lipofuscinosis (INCL). Comparison with clinical manifestations and MRI findings.

We studied brain perfusion in 19 patients with infantile neuronal ceroid-lipofuscinosis (INCL), aged 13 months to 11 years, using 99mTc-HMPAO single photon emission computed tomography (SPECT). SPECT findings were compared with clinical manifestations and MRI findings. The typical SPECT findings at an early stage of INCL were bilateral anterior frontal, posterior temporoparietal and occipital hypoperfusion. Initially cerebral hypoperfusion was localized and symmetrical, whereas atrophic findings were more generalized. Reduction in cerebellar perfusion appeared later, as did cerebellar atrophy.

The mental nerve blink reflex in the diagnosis of lesions of the inferior alveolar nerve following orthognathic surgery of the mandible.

The purpose of this study was to evaluate the diagnostic value of a new modification of the blink reflex test with stimulation of the distribution of the mental nerve in iatrogenic lesions of the inferior alveolar nerve. The test was performed on 23 patients undergoing orthognathic surgery of the mandible, most of them (20) with bilateral sagittal split osteotomies. The function of the inferior alveolar nerve was studied preoperatively, and 2 weeks, 2 months, 6 months and 1 year postoperatively with both mental nerve blink reflex test and clinical neurosensory testing. The objective electrophy

Loss of circadian blood pressure variability in complete tetraplegia.

Blood pressure (BP) and heart rate (HR) were monitored over 24 h utilising an ambulatory blood pressure monitoring (ABPM) system in 33 subjects with spinal cord injury: 11 patients with complete tetraplegia, 13 patients with incomplete tetraplegia and nine patients with complete paraplegia. Measurements were analysed for overall levels of BP and HR, presence of a physiological day/night cycle and events of autonomic dysreflexia. Paraplegic patients exhibited normal BP and HR levels with a physiological circadian rhythm. In complete tetraplegic patients the circadian rhythm was abolished for BP

[Acute spinal subdural hematoma after attempted spinal anesthesia].

This is a report of a case of a subdural haematoma with resulting paraplegia after attempted spinal anaesthesia. Epidural and subdural haematomas are rare complications after central neural blockade. The complication described here was the result of an unsuccessful attempt to puncture the spinal channel. The patient was a 72-year-old woman with a fracture of the left femoral neck, which it was intended to stabilize operatively. Findings that made lumbar spinal puncture difficult were severe overweight, and lordosis and scoliosis of the lumbar spine resulting from degenerative changes. Spinal a
