An en bloc avulsion fracture of tibial tuberosity and Gerdy's tubercle in an adolescent basketball player: a case report.

We report a case of en bloc avulsion fracture of tibial tuberosity and Gerdy's tubercle, which has never been reported. A 14-year-old boy suffered from an acute pain in his left knee during running just before a jump. Simple radiographs showed an avulsion of the tibial tuberosity. On CT scans, the fractured fragment was attached not only to patellar tendon but also to iliotibial band (ITB) via Gerdy's tubercle. MRI evaluation revealed no intra-articular associated pathology. Open reduction and internal fixation with three cannulated screws were performed under lateral parapatellar approach to

Dislocation of the shoulder joint with ipsilateral humeral shaft fracture: two case reports.

INTRODUCTION: A combination of the shaft fracture of the humerus with shoulder dislocation is a rarely documented injury. Moreover, few reports describe a long-term outcome after a follow-up period over 10 years. The present article is the first report of long-term clinical results obtained with this combined injury. MATERIALS AND METHODS: We present two cases of the dislocation of the shoulder joint with ipsilateral humeral shaft fracture. One is an anterior dislocation and the other is posterior. The anterior dislocation was closely reduced on the date of injury, but the posterior dislocati

Antegrade Kirschner Wire Transfixation of Interphalangeal and Metacarpophalangeal Joint in a Comminuted Thumb Proximal Phalangeal Fracture.

Comminuted proximal phalangeal fractures of the thumb are inherently unstable injuries requiring surgical fixation to provide stability and restore function. Current fixation methods include plate and screw fixation, which is associated with stiffness (in 64% of cases), extensor lag, and hardware complications requiring further operations. Moreover, intramedullary screws carry the risk of cartilage damage and fracture displacement during insertion. These modalities requiring extensive soft tissue dissection correlate with poor functional outcomes due to fibrosis and vascular disruption. A simp

A Twist in the Shoulder Tale: Proximal Humerus Fracture Following Trapezius Transfer - A Case Report.

INTRODUCTION: Proximal humerus fractures constitute approximately 4-6% of all fractures, commonly affecting the elderly due to low-energy trauma and osteoporotic bones. Two-part surgical neck fractures are the most frequent subtype. In younger individuals, high-energy trauma is a usual cause. However, a proximal humerus fracture in a patient with a history of trapezius muscle transfer for brachial plexus injury is an extremely rare clinical scenario, with limited literature available on its management and rehabilitation. CASE REPORT: A 30-year-old right-hand-dominant male presented with acute

Suture Anchor Repair for Distal Patellar Tendon Avulsion in Tibial Tubercle Fractures: A Technical Description and Report of Two Cases.

Tibial tubercle avulsion fractures, though rare, pose a significant challenge in pediatric orthopedics, particularly in athletic adolescents. For nondisplaced fractures, conservative treatment involves the use of braces or casts, while displaced fractures necessitate operative intervention, often through screw fixation. Concomitant soft tissue injuries should also be identified and addressed operatively to ensure complete repair of the extensor mechanism. This paper introduces a method for open reduction and internal fixation (ORIF) of tibial tubercle fractures with suture anchor repair of the

Unusual case report of concomitant lunate and scaphoid fractures without associated dislocation in a 16-year-old male.

INTRODUCTION AND IMPORTANCE: Lunate fractures without associated dislocations are rare injuries, combination scaphoid-lunate fractures without an associated dislocation are even more rare of which few are reported in the literature. CASE PRESENTATION: This case report describes a 16-year-old otherwise healthy male with ipsilateral scaphoid and lunate fractures after punching a goalpost with his left hand, for which he had surgical management. This patient had a successful treatment with two headless compression screws through a single extended carpal tunnel approach. Surgery was performed wit

KNEE PERIPROSTHETIC FRACTURES IN THE ELDERLY: CURRENT CONCEPT.

Periprosthetic fractures around total knee arthroplasty in elderly represent an emerging cause of implant revision and their incidence seems destined to further increase in the upcoming years, considering the ever-increasing number of implanted prostheses. These are complex injuries with very high complication rates. It has been estimated that the incidence of femoral periprosthetic fractures after T.K.A. ranged between 0,3 to 2,5%, but increases up to 38% when considering revision T.K.A. Patient-related risk factors for T.K.A. periprosthetic fracture (T.K.A.P.F.) include osteoporosis, age, fe

Single-Point Fixation for Noncomminuted Zygomaticomaxillary Complex Fractures-A 20-Year Experience.

PURPOSE: Zygomaticomaxillary complex (ZMC) fractures occur often. However, no clinical consensus has been reached regarding the number of fixation points required when performing open reduction and internal fixation (ORIF). The objective of the present study was to explore the utility of single-point fixation in the management of noncomminuted ZMC fractures. PATIENTS AND METHODS: We analyzed the data from a retrospective case series of 211 patients treated during a 20-year period. RESULTS: The mean length of follow-up was 3.4 months. Of the 211 patients, 162 with noncomminuted ZMC fractures

A working paradigm for managing mandibular fractures under regional anesthesia.

OBJECTIVES: Isolated mandibular fractures contribute to approximately 45% of maxillofacial traumas. Improper management of mandibular fractures can cause myriad potential complications and can lead to serious functional and aesthetic sequelae. The objective of the study is to design a stepwise approach for managing isolated mandibular fractures using open reduction and internal fixation (ORIF) with regional anesthesia on outpatient basis. MATERIALS AND METHODS: Patients with isolated mandibular fractures presenting to the department of maxillofacial surgery were selected for ORIF under region

Surgical treatment of sacral fractures following lumbosacral arthrodesis: Case report and literature review.

Sacral fractures following posterior lumbosacral fusion are an uncommon complication. Only a few case series and case reports have been published so far. This article presents a case of totally displaced sacral fracture following posterior L4-S1 fusion in a 65-year-old patient with a 15-year history of corticosteroid use who underwent open reduction and internal fixation using iliac screws. The patient was followed for 2 years. A thorough review of the literature was conducted using the Medline database between 1994 and 2014. Immediately after the revision surgery, the patient's pain in the bu

The fifth metatarsal base: anatomic evaluation regarding fracture mechanism and treatment algorithms.

Fractures occurring within the 1.5-cm proximal portion of the fifth metatarsal are commonly considered avulsion fractures. The exact mechanisms of such fractures are controversial. The present study focused on determining the likely mechanism of fracture according to the exact anatomy to allow for more successful treatment. The research sample included 10 frozen cadaveric specimens. The lateral band of the plantar fascia, peroneus brevis, and articular surface were identified and separated from their attachments, thereby splitting the fifth metatarsal base into zones A, B, and C. In zone A, th

Treatment of reverse oblique and transverse intertrochanteric fractures with use of an intramedullary nail or a 95 degrees screw-plate: a prospective, randomized study.

BACKGROUND: Intertrochanteric fractures are composed of different anatomic patterns that vary in their degree of stability following open reduction and internal fixation. A particularly unstable group is classified as AO/OTA 31-A3, with the fracture pattern described as reverse oblique or transverse. The purpose of this study was to compare the results of intramedullary fixation with those of plate fixation for these intertrochanteric fractures in elderly patients. METHODS: Thirty-nine elderly patients with AO/OTA 31-A3 intertrochanteric fractures of the femur were randomized into two treatme

Standard or Fin SIGN® nail? which option is better for the treatment of femoral fractures in low and middle-income countries?

PURPOSE: Femoral fractures are common in low and middle-income countries (LMIC), predominantly caused by high-energy trauma. The surgical implant generation network (SIGN®) program offers two different intramedullary nails in LMIC which are designed to be used without image intensifier free of charge for the patients: the SIGN standard nail (SSN®) and the SIGN Fin nail (SFN®). This study aimed to compare the results of the SSN® and the SFN® for the treatment of middle and distal shaft femoral fractures through a retrograde approach. MATERIAL AND METHODS: This was a retrospective, descriptive,

Retrograde intramedullary nailing for periprosthetic supracondylar fractures of the femur after total knee arthroplasty.

BACKGROUND: Periprosthetic supracondylar fractures of the femur after total knee arthroplasty are not common but are usually difficult to treat due to the advanced age of patients and frequently accompanying osteoporosis. Retrograde intramedullary nailing can be effective in promoting healing of these fractures by providing sufficient stability, but the number of beneficiaries is small due to its limited applicability and the postoperative function has rarely been assessed. This study evaluated the efficacy of retrograde intramedullary nailing for the treatment of periprosthetic supracondylar

[Complex injury of the elbow joint].

High-velocity trauma now often results in complex injuries to the upper extremity, and especially the elbow joint. These can lead to both an enormous reduction in the range of motion of the shoulder, elbow and wrist joints, in severe cases with complete loss of upper extremity function. A complex injury is defined as a fracture and/or dislocation of the elbow in association with (1) a serial injury of the upper extremity, (2) a severe soft tissue trauma, or (3) concomitant injury to vessels or nerves. Serial fractures, in particular can lead to enormous problems with treatment and are often as

Immobilization Following Operative Treatment of Lateral Condyle Fractures: Early Motion Is Safe.

BACKGROUND: Immobilization is required following operatively treated lateral condyle fractures to provide adjunct stability. However, there is no clear consensus regarding the optimal duration of postoperative immobilization. This study aims to compare an early motion (EM) protocol of four weeks or less of immobilization to a six-week or longer immobilization period, evaluating the differences in postoperative range of motion (ROM), healing, and complications. METHODS: We performed a retrospective, single center cohort study of acute, isolated, lateral condyle fractures in pediatric patients

Does Surgical Repair Benefit Pipkin Type I Femoral Head Fractures?: A Systematic Review and Meta-Analysis.

Background: Femoral head fractures are rare injuries with or without traumatic dislocations. The management of these fractures is crucial to prevent the development of severe complications and to achieve optimal functional outcomes. Wide treatment options for Pipkin 1 femoral head fractures range from fragment excision, fixation following open reduction with internal fixation, or conservative treatment such as close reduction alone after fracture dislocation. However, the best decision making remains controversial not only due to lack of large trials, but also inconsistent results reported. Th
