[Modifications of fronto-orbital osteotomy as an approach to the anterior and middle skull base].

Various extracranial, intracranial and combined extra-intracranial approaches have been described for the surgical therapy of tumours of the anterior and medial cranial base. A combined extra-intracranial approach is indicated in cases in which the cranial base tumour spreads out intracranially and at the same time into the main nasal cavity, the paranasal space and/or the orbits. These tumours of the skull base can be surgically removed by two standardised modifications of the fronto-orbital osteotomy--the fronto-orbito-nasal and the fronto-orbito-zygomatic osteotomy. The advantage of these s…

[A 44-year-old patient with cardial carcinoma and paraneoplastic orbital disease].

HISTORY AND CLINICAL FINDINGS: Bilateral orbital involvement with chemosis, protrusion and reduced vision developed in a 44-year-old man 12 weeks after carcinoma of the cardia with lymph node metastases and peritoneal carcinomatosis had been diagnosed and palliative chemotherapy with a cycle of leucovorin and 5-fluorouracil had been concluded. INVESTIGATIONS: Computed tomography excluded a retrobulbar metastasis. Thyroid hormone concentration was normal as was the thyroid autoantibody titre. There was also no evidence on sonography of endocrine orbital disease. These findings established the …

Subclinical Sjögren's syndrome: a significant 67gallium accumulation in the orbits and parotid glands.

An 8-year-old girl with hypergammaglobulinemia showed an abnormal 67gallium accumulation in the orbits and parotid glands. Although she did not have any subjective siccant complaints, reported typical histopathological and sialographic changes suggesting Sjögren's syndrome (SjS) were observed in the salivary glands. Gallium scintigram might be a valuable and non-invasive diagnostic tool in the diagnosis of children with SjS without sicca symptoms.

The incidence of diplopia following coronal and translid orbital decompression in Graves' orbitopathy.

PURPOSE: Firstly, to assess the incidence of induced diplopia following orbital decompression in patients with Graves' orbitopathy. Secondly, to assess patient satisfaction after orbital decompression. Thirdly, to determine the factors that contribute to the variable reported incidence of diplopia complicating decompression surgery. METHODS: We present a retrospective analysis of the alterations of ocular motility in a consecutive series of 81 patients with Graves' orbitopathy who underwent orbital decompression by either a coronal or a translid approach. We assessed patient satisfaction by a…

[Acute orbital myositis and idiopathic inflammatory pseudotumor in children: three cases].

UNLABELLED: Orbital pseudo-tumors account for approximately 8% of the orbital tumors. They are scarcely described in children and raise diagnostic dilemas, especially when they present as an isolated extra-ocular muscle swelling called idiopathic orbital myositis (IOM). CASE REPORTS: A diagnosis of inflammatory orbital pseudotumors (IOPT) was made in three children aged 7, 13 and 14 years from clinical and CT scan and/or MRI findings associated with histological data in two of them. Two of these IOPT presented as IOM. Analysis was made in a effort to eliminate intra-orbital tumors, especially…

Nonsyndromal craniosynostosis: longitudinal outcome following cranio-orbital reconstruction in infancy.

This paper presents a prospective longitudinal outcome study on patients with nonsyndromal craniosynostosis who were treated with the contemporary craniofacial surgical techniques of suture release, cranial decompression, and cranial and orbital reconstruction and reshaping in infancy. Diagnosis, surgical treatment, and long-term results and complications are reviewed. Preoperative and long-term postoperative intracranial volumes in these patients were evaluated and compared with age and gender match controls throughout the period of the study. From July 1, 1990, to July 1, 1994, 25 patients w…

Orbital floor implant migration across the ethmoidal sinuses and nasal septum.

PURPOSE: To report an unusual case of orbital floor implant migration across the ethmoidal sinuses and nasal septum. METHOD: Case report. A 61-year-old woman with a history of right orbital floor fracture repair 25 years earlier is described. RESULTS: The patient presented with sinus congestion and difficulty breathing through the right nostril. Computed tomographic scan disclosed medial migration of the right orbital floor implant across the ethmoidal sinuses and nasal septum. The patient underwent transorbital and transnasal endoscopic surgery with removal of the implant. CONCLUSIONS: Whe…

Orbital venous drainage into the anterior cavernous sinus space: microanatomic relationships.

OBJECTIVE: Microsurgical anatomy and relationships in the region of the superior orbital fissure and anterior part of the cavernous sinus were studied, with special attention to the venous structures. METHODS: Microanatomic dissections were performed in 24 specimens from human cadavers, fixed in formalin, with the aid of a microdrill and operating microscope. RESULTS: A protocol of dissection was developed, enabling plane-by-plane exposure of the cranial nerves passing from the cavernous sinus to the superior orbital fissure and farther, on disclosure of the junction between the ophthalmic v…

Frozen section--guided surgical debridement for management of rhino-orbital mucormycosis.

PURPOSE: To present a new method of surgical treatment for rhino-orbital mucormycosis. METHOD: We treated a patient with extensive surgical debridement of mucormycosis with intraoperative frozen section guidance. The patient did not require exenteration. RESULT: Our patient survived with normal vision and no diplopia. CONCLUSIONS: Frozen section-guided surgical debridement in rhino-orbital mucormycosis may provide an alternative to traditional radical surgical excision. In selected cases, this new method of treatment may cause less morbidity than do traditional treatments.

The role of plain radiography in the management of suspected orbital blow-out fractures.

We reviewed the indications for radiography and its impact on management in 100 consecutive patients referred to the Radiology Department from the Accident and Emergency Department of a large eye hospital in order to assess the utility of orbital plain radiographs in patients suspected of having blow-out fractures. We assessed whether administration of antibiotics or referral for surgical or orthoptic treatment was influenced by clinical features, radiographic findings or both. Ophthalmologists interpreting the radiographs appeared to be guided by the presence or absence of physical signs. No …

Asymptomatic posterior orbital cellulitis resulting from ethmoid/maxillary sinusitis.

BACKGROUND: Although vision-threatening sinusitis complications are not uncommon, an unusual case of asymptomatic posterior orbital cellulitis resulting from maxillary and ethmoidal sinus infection is presented. METHODS, RESULTS: An 11-year-old girl was brought to us for a routine optometric examination, reporting only blurred distance vision. A mild afferent pupillary defect was the key to detection and diagnosis of a vision-threatening posterior orbital cellulitis. CONCLUSIONS: A literature review of relevant anatomic and pathologic mechanisms, as well as radiologic recommendations, follow…

Fronto-ethmoidal and orbital osteomas with intracranial extension. Report of two cases.

Osteomas involving the anterior cranial base are quite rare lesions. Intracranial and orbital extension of these tumors, causing neurological and ophthalmological symptoms and signs is a rare event. Two such cases are reported. In the first one, the tumor arose from the frontal sinus and extended upward to the medial and superior wall of the right orbital cavity and the lower part of the right frontal convexity. In the second case the osteoma arose from the left orbital roof and extended to the lower part of the homolateral frontal convexity and the splenoid wing. After the operation the sympt…

Connectional networks within the orbital and medial prefrontal cortex of macaque monkeys.

The intrinsic cortico-cortical connections within the orbital and medial prefrontal cortex (OMPFC) were demonstrated with retrograde and anterograde tracers injected into each of the architectonic areas that constitute this region. Although many of the connections linked neighboring areas, others selectively connected relatively distant areas. Most, but not all, of the connections were reciprocal. Altogether, the connections formed at least two distinct networks within the OMPFC. The "orbital" prefrontal network linked most of the areas within the orbital cortex, with very few connections to m…

Extradural deadspace after infant fronto-orbital advancement in Apert syndrome.

Infant fronto-orbital advancement is presently a management cornerstone of the craniosynostosis in Apert syndrome. This procedure is based on the premise that the expanding brain rapidly advances in support of, and to remodel, the new frontal facade. This series of infant patients with Apert syndrome undergoing fronto-orbital advancement with early postoperative computerized tomography assessment reveals a large extradural deadspace persistent through the first postoperative week, not obliterated consistently until the fourth week. Early deadspace effacement results from expansion of the prefr…

Congenital ptosis: a good cosmetic result with redefinition and suturing of the orbital septum.

A surgical technique employing orbital septum sutures during ptosis surgery in children is described. A retrospective study of 16 children (age range 6 months to 14 years) undergoing surgery for congenital ptosis over a 6 year period was undertaken with regard to cosmetic outcome. All surgery was performed by one consultant ophthalmic surgeon with the patient under general anaesthesia. A standard levator resection was undertaken, following which the orbital septum was redefined and sutured with interrupted 5-0 catgut. This resulted in a well-defined lid crease post-operatively, with a good cos…
